(Attached: Wood Kidney, Scrotal Eczema)
Scrotal pain is generally caused by the accumulation and downward flow of damp-heat and phlegm turbidity. It is mostly triggered by cold stagnation, essentially involving phlegm-fluids, food retention, and dead blood (blood stasis). The pathology primarily belongs to the Liver Meridian and is related to the Kidney. Purging methods are strictly contraindicated. For severe pain, tonics such as Renshen (Ginseng Radix et Rhizoma) and Baizhu (Atractylodis Macrocephalae Rhizoma) should not be used. If dampness predominates, Moxibustion on Dazhun (LR1, Great Dun) acupoint is suitable for hernia; this point is located in the hair margin between the big toe nail and the skin, approximately the width of a leek leaf. For pain caused by food retention and blood stasis, use Zhizi (Gardeniae Fructus), Taoren (Persicae Semen), Shanzha (Crataegi Fructus), Zhizi (Zanthoxyli Fructus or Zhi Shi - Citri Reticulatae Pericarpium), and Wuzhuyu (Evodiae Fructus), decocted with fried ginger juice and flowing water, taken as a powder suspension. Another prescription adds Huixiang (Foeniculi Fructus) and Fuzi (Aconiti Lateralis Radix Praeparata). In cases with fluid retention causing swelling and pain, or those with deficiency, Renshen (Ginseng Radix et Rhizoma) and [Baizhu] (Atractylodis Macrocephalae Rhizoma) should serve as the sovereign drugs, assisted by draining and guiding agents. The pulse in such cases is typically deep, tight, or wide and empty. Tenderness that is indistinct upon palpation indicates deficiency; one must use Guizhi (Cinnamomi Ramulus), fried Zhizi (Gardeniae Fructus), and Wu Tou (Aconiti Radix), finely cut and fried, ground into powder, mixed with ginger juice to form pills. Take 30-40 pills per dose with ginger soup; this strongly alleviates pain.
Dai Yun stated: Hernia fundamentally belongs to the Jueyin Liver Meridian. I often see vulgar theories attributing it to Qi issues in the lower parts of the Small Intestine or Bladder, which are all false statements.
[Prescriptions]
Treating various hernias with rapid pain relief.
Zhi Shi (Citri Reticulatae Pericarpium, 15 slices, alternatively Ju He - Tangerine Seed), fried Zhizi (Gardeniae Fructus), fried Shanzha (Crataegi Fructus), fried Wuzhuyu (Evodiae Fructus) in equal proportions. If dampness is excessive, add Pao Lizhi He (Litchi Seed).
Grind all ingredients into powder, mix with wine paste to form pills. Alternatively, take the powder decocted in ginger water, or mix 1-2 qian with long-flowing water, taken on an empty stomach.

Zhizi Ren (Gardeniae Semen) is used to drain damp-heat, while Wu Tou (Aconiti Radix) disperses cold constraint. Both herbs act on the Lower Jiao, and Wu Tou's action is accelerated by Zhizi, ensuring rapid efficacy without lingering in the Stomach.

I once treated a patient who drank water after illness and suffered severe pain in the left testicle. We moxa-ed Dazhun (LR1) Acupoint. I happened to have Moyao Gao (Waist Rubbing Plaster) containing Wutou (Aconiti Radix), Fuzi (Aconiti Lateralis Radix Praeparata), Dingxiang (Syzygii Aromatici Flos), and Shexiang (Moschus). I applied it to the transverse bone end above the scrotum, covered it with a heated cloth, and the pain ceased immediately. By morning, the swelling subsided. I previously had citrus retention. Once, while hiking and very hungry, I ate oranges and taro. The oranges disturbed my old retention, and the taro stagnated Qi, causing immediate swelling of the right testicle with chills and fever. I first took 1-2 doses of Stomach-regulating medicine. The next morning, I administered acupuncture at Shenque (CV8); Qi reached the Lower Jiao inducing vomiting. Feeling the retention loosen and vomit out, I then harmonized Stomach Qi and unblocked meridians, resulting in recovery.
[Appendix] "Wood Kidney" occurs when Heart Fire descends, ensuring Kidney Water is warm; when True Yang descends, Kidney Qi is harmonious and warm. With warmth and harmony, how could "wood-like hardness" occur? It happens because internal damage from desires leads to Kidney deficiency and exhaustion. Yin and Yang fail to interact, Water and Fire fail to balance, allowing deep-seated cold to stagnate. This results in distension, severe pain, stubborn numbness, and hard masses—a inevitable progression. Do not use purely drying-hot medicines; instead, use warming-dispersing and warming-draining methods to expel pathogens. As pathogens dissipate internally, Ying and Wei Qi circulate freely, akin to a cold valley returning to spring, achieving rapid results without forced effort.
