A woman suffered from dead blood (stagnant blood), accompanied by food retention, phlegm-fluids condensing into masses. These pathogenic factors sometimes moved between the two hypochondria, or caused borborygmus and epigastric discomfort in the abdomen, accompanied by vertigo and fever, with symptoms appearing intermittently (for treatment, see Volume 5, under the section "Masses").
For treating pain in the Sea of Blood (uterus) in women: Dang Gui (Radix Angelicae Sinensis) 1 qian, Gan Cao (Radix Glycyrrhizae) and Mu Xiang (Radix Aucklandiae) each 5 qian, Xiang Fu (Rhizoma Cyperi) 2 qian, Wu Yao (Fructus Linderae) 1.5 qian. Prepare as one dose, decoct in water, and take before meals.
For women suffering from pain due to blood and qi disorders, grind E Zhu (Rhizoma Curcumae) with wine and administer. A woman had a blood mass in her abdomen the size of a plate; however, she was pregnant and could not tolerate harsh medications. The prescription consisted of Xiang Fu (Rhizoma Cyperi) 4 liang, Tao Ren (Semen Persicae) boiled in vinegar 1 liang (skin and tip removed), Hai Shi (Lapis Calcitae) boiled in vinegar 2 liang, Bai Zhu (Rhizoma Atractylodis Macrocephalae) 1 liang. These were made into pills using Shen Qu (Massa Medicata Fermentata) paste. For women suffering from unbearable stabbing heart pain due to blood and qi disorders, administer Mu Xiang (Radix Aucklandiae) powder mixed with wine. When blood and qi rush into the head and brain, causing dizziness, fainting, and loss of consciousness, use the tender heart of Cang Er (Fructus Xanthii), dry it in the shade, grind into powder, and mix with wine for administration.
A woman suffered from pain caused by a Jia mass (abdominal mass) in her abdomen, or blockage due to qi attacking and obstructing. The prescription included: Xiang Fu (Rhizoma Cyperi) 1 liang (boiled in vinegar), Dang Gui (Radix Angelicae Sinensis) 1 liang, Bai San Leng (Rhizoma Sparganii) 1 liang (processed), Hei San Leng (Rhizoma Sparganii) 1 liang (processed), Hei E Zhu (Rhizoma Curcumae) 1 liang, Mo Yao (Resina Myrrhae), Ru Xiang (Resina Olibani), Chuan Xiong (Rhizoma Chuanxiong) each 5 qian, Kun Bu (Thallus Laminariae), Zao Hai (Thallus Sargassii) each 1 liang (stir-fried), Bing Lang (Semen Arecae) 5 qian, Qing Pi (Pericarpium Citri Reticulatae Viride) 1 liang (pith removed), Gan Qi (Cortex Cinnamomi processed with honey/charred) 5 qian (stir-fried until smoke ceased), Mu Xiang (Radix Aucklandiae), Chen Xiang (Aquilariae Lignum Resinatum), Suo Sha (Fructus Amomi Rotundus) each 5 qian. Grind all ingredients into fine powder, make pills using rice vinegar paste, sized like paulownia seeds. Take 60-70 pills per dose, on an empty stomach, washed down with plain water or salt water. Avoid raw, cold, and greasy foods.
For treating waist and abdominal pain caused by blood and qi disorders: Use Dang Gui (Radix Angelicae Sinensis) and Yuan Hu (Rhizoma Corydalis Yanhusuo) in equal parts, ground into coarse powder. Take 3 qian per dose, decoct with 3 slices of fresh ginger.
Formula for treating diseases caused by all types of stagnant blood: Xiang Fu (Rhizoma Cyperi) 4 liang (boiled in vinegar), Tao Ren (Semen Persicae), Wa Long Zi (Concha Arcae) 2 liang (boiled in vinegar for one day and one night), Mu Dan Pi (Cortex Moutan), Da Huang (Radix et Rhizoma Rhei) steamed in wine, Dang Gui (Radix Angelicae Sinensis), Chuan Xiong (Rhizoma Chuanxiong), Hong Hua (Flos Carthami) each 5 qian. Grind into fine powder and make pills using steamed wheat dough.
For amenorrhea and cramping abdominal pain: Tai Wu Yao (Rhizoma Linderae) 2 liang, Dang Gui (Radix Angelicae Sinensis), E Zhu (Rhizoma Curcumae) each 1 liang. Grind into powder, take 2 qian on an empty stomach with wine.
A woman had missed two menstrual cycles, accompanied by abdominal pain and fever. By promoting blood circulation and cooling the blood, the disease would resolve spontaneously once menstruation resumed. Use Si Wu Tang (Four Substances Decoction) added with Huang Qin (Radix Scutellariae), Hong Hua (Flos Carthami), Tao Ren (Semen Persicae), Xiang Fu (Rhizoma Cyperi), Yuan Hu Suo (Rhizoma Corydalis Yanhusuo), etc.
A woman in her forties, pale-faced, emaciated, and irritable, experienced a mass forming under her breasts below the ribs three months after encountering significant distress. The mass gradually enlarged, covering the heart region, causing mild pain and chest oppression. Her appetite decreased by three-quarters, and she felt bitter taste in her mouth every morning. Her pulses in both hands were faint, short, and choppy. I deduced that her menstruation had ceased and was very concerned, advising against treatment initially. However, at midnight, observing that she could still go out to see doctors, dress up, and speak normally, I surmised she still had Stomach Qi. Thus, I prescribed a large dose of Ren Shen (Radix Ginseng), Bai Zhu (Rhizoma Atractylodis Macrocephalae), Dang Gui (Radix Angelicae Sinensis), and Chuan Xiong (Rhizoma Chuanxiong), assisted by qi-regulating herbs. This large dose was administered four times throughout the night. Externally, a large Hu Po Gao (Amber Plaster) was applied to the mass to prevent further growth. After more than a month, having taken over a hundred doses of tonifying medicine, her diet recovered to half of normal. She continued with the previous medication. Another month later, her pulse became fuller. She took the previous medicine along with over a hundred grains of Run Xia Wan (Moistening Down Pills). Menstruation lasted less than two days and then stopped. The choppy pulse reduced by four-fifths. Given the hot weather, when menstruation occurred, the blood was purple. She continued with the previous medicine plus San Leng (Rhizoma Sparganii), swallowing Run Xia Wan, assisted by Yi Qing Wan (Suppress Green Pill) 15 grains. Another month passed, and suddenly the mass had diminished by half. Menstruation arrived on time, though half a day shorter than usual. Appetite was good and normal, but eating meat did not feel satisfying. I instructed her to stop medication and wait until spring when Wood Qi is vigorous to reassess. In June of the following year, she reported that overnight the mass reappeared, increasing by half a finger's width compared to before. The pulse was slightly wiry, with the left side slightly weaker than the right, and the rate was peaceful. She stated that after full meals, the mass felt slightly oppressive, but subsided after digestion. I suspected that emotional agitation had triggered it, which proved true. I used the previous formula added with stir-fried Huang Qin (Radix Scutellariae) and Chuan Lian Zi (Fructus Toosendan) [Note: Text says Chao Lian, likely Chai Hu or Huang Lian contextually, but literal is Lian], assisted by small amounts of Mu Tong (Caulis Akebiae) and Sheng Jiang (Rhizoma Zingiberis Recens). San Leng was removed. Decoct and swallow Run Xia Wan. Apply Amber Plaster externally. Half a month later, menstruation occurred, and the qi mass dissipated. This condition resulted from Lung Metal being scorched by Fire, allowing Wood to slightly overcome Earth. Earth failed to transport, leading to interference between clear and turbid qi. The outline of the old mass remained because blood and qi had not fully recovered. Slight retention of turbid qi caused the old mass to recur. Tonifying blood and qi prevents Lung from receiving evil, subdues Wood, rectifies Earth, promotes the movement of turbid qi, and dissipates the mass.
A maid, melancholic and prone to worry, aged forty, had not menstruated for three months. A mass appeared in the center of the lower abdomen, gradually growing to the size of a steamed bun. All pulses were choppy and heavy. Pressing the mass gently caused severe pain. Palpation revealed it protruded half a cun above the skin. She was given Qian Jin Xiao Shi Wan (Millennium Stone-Dissolving Pills) one to four times. She suddenly said, "My nipples are dark and secreting fluid; I fear I am pregnant." I replied, "A choppy pulse does not indicate pregnancy." I gave two more doses. The pulse became slightly larger and open. I realized, "The medication was too harsh!" I ordered cessation of drugs. I prescribed Si Wu Tang (Four Substances Decoction) with doubled Bai Zhu (Rhizoma Atractylodis Macrocephalae), assisted by Chen Pi (Pericarpium Citri Reticulatae) and Zhi Gan Cao (Radix Glycyrrhizae Preparata). After thirty doses, waiting for the pulse to become full, I administered Xiao Shi Wan (Stone-Dissolving Pills) one to four times again. She suddenly said the mass had receded by one circle, so I told her to stop. Half a month later, menstruation began with severe pain, expelling nearly half a sheng of black blood containing dozens of particles resembling pepper seeds. The mass reduced by half. She came to request more medicine. I explained, "The mass has ruptured; do not attack it further. Just observe dietary restrictions, and the next menstruation will cause it to disappear completely." This indeed happened.
