Kun virtue is vast, all things rely on it for growth; this is the constant principle of nature. For a woman who has just given birth, sufficient blood may not necessarily be depleted, foul blood may not necessarily accumulate, and the zang-fu organs may not necessarily be cold. Why use medicine? If one diligently cares for diet and daily life, what illness would there be? If other illnesses occur, one should seek the cause of onset, determine which meridian is affected, treat qi disorders by regulating qi, and treat blood disorders by regulating blood. Why does *Ju Fang* (Formulary of the Bureau) fail to examine carefully and instead universally prescribe the formula Heishen San (Black Spirit Powder)?
I often see postpartum women without illness, and I must instruct them to discard Heishen San (Black Spirit Powder), large chicken eggs, fire salt, and various meats. Instead, provide white rice gruel for management, occasionally offering some stone-head fish (Yellow Croaker), boiled until bland and light. After one month, then offer small amounts of meat. Chicken eggs must also be cracked open and boiled; this greatly nourishes the stomach and dispels phlegm. Postpartum conditioning prescription: Dang Gui (Radix Angelicae Sinensis, 1 qian), Chuan Xiong (Rhizoma Ligustici Chuanxiong, 1 qian), Bai Zhi (Radix Angelicae Dahuricae), Guan Gui (Cortex Cinnamomi Cassiae), E Zhu (Rhizoma Curcumae Zedoariae), Mu Dan Pi (Cortex Moutan Radicis, all 5 fen), Fu Ling (Poria, 1 qian), Gan Cao (Radix Glycyrrhizae Uralensis, 3 fen). Decoct and take orally. Add Yan Hu Suo (Rhizoma Corydalis Yanhusuo) for abdominal pain; add Huang Qin (Radix Scutellariae Baicalensis) and Chai Hu (Radix Bupleuri) for fever; add Suo Sha (Fructus Amomi Rotundus) and Chen Pi (Pericarpium Citri Reticulatae) for poor appetite.
Qing Hun San (Clear Soul Powder) treats postpartum blood syncope: Su Mu (Santalum Album Lignum), Ren Shen (Radix Ginseng, half liang), Tong Bian (Child's Urine). The above three ingredients are decocted together with water and wine and taken orally. Postpartum blood syncope arises from deficient fire carrying blood upward gradually. Burn Lu Jiao (Cornu Cervi Pantotrichum) to ash to remove fire toxicity, grind into an extremely fine powder, mix with good wine, and administer via gavage to induce immediate awakening; this method moves blood very rapidly. Another prescription: finely cut Jiuye (Folium Allii Tuberosi), place in a bottle with a spout, pour hot Cu (Vinegar) over it, quickly seal the mouth, insert the spout into the mother's nostril to cure dizziness. Before delivery, the mother tends to have stagnation; after delivery, the mother tends to be deficient. Postpartum care should focus on greatly tonifying blood and qi. Even if there are miscellaneous symptoms, treat the root first. In postpartum conditions, all diseases should not be treated with diaphoretic methods.
Postpartum deficiency tonification: Ren Shen (Radix Ginseng), Bai Zhu (Rhizoma Atractylodis Macrocephalae, each 1 qian), Huang Qin (Radix Scutellariae Baicalensis, half qian; another version says Huang Qi [Radix Astragali Membranacei]), Chen Pi (Pericarpium Citri Reticulatae, 5 fen), Chuan Xiong (Rhizoma Ligustici Chuanxiong, 5 fen), Zhi Gan Cao (Radix Glycyrrhizae Preparata, 3 fen), Dang Gui Shen Wei (Radix Angelicae Sinensis body and tail, 5 fen). Add Gan Jiang (Rhizoma Zingiberis Preparatum, 3 fen) if there is heat; add Fu Ling (Poria, 1 qian).
Postpartum blood stasis resolution: Hua Shi (Talcum, 3 qian), Mo Yao (Resina Myrrhae, 3 qian), Qilin Jie (Dragon's Blood, 2 qian; if unavailable, substitute Mu Dan Pi [Cortex Moutan Radicis], 1 qian). Grind into powder, make pills with vinegar paste. For postpartum lochia retention, grind Wu Ling Zhi (Excrement Trogopteri, 3 qian) into powder, make pills with Shen Qu (Massa Medicata Fermentata), and swallow with Bai Zhu (Rhizoma Atractylodis Macrocephalae) and Chen Pi (Pericarpium Citri Reticulatae) decoction. Qilin Jie (Dragon's Blood) and Wu Ling Zhi (Excrement Trogopteri) are excellent for resolving postpartum blood stasis. For postpartum lochia non-discharge with lower abdominal pain, use Wu Ling Zhi (Excrement Trogopteri) and Xiang Fu (Rhizoma Cyperi Rotundi) powder mixed with vinegar to form pills; for severe cases, add Tao Ren (Semen Persicae), unremoved tips. Postpartum abdominal pain with fever indicates必有 (must have) evil blood, which should be removed. Use Zengsun Siwu Tang (Modified Four Substances Decoction) for postpartum fever.
Seven or eight days postpartum, fever caused by great fright, accompanied by retching and vomiting, excessive phlegm, sweating upon vomiting: use Bawu Tang (Eight Substances Decoction) plus Huang Qin (Radix Scutellariae Baicalensis); if lower abdomen and pain coexist, add Gui Zhi (Ramulus Cinnamomi Cassiae). Postpartum wind-stroke must never be treated as ordinary wind. For postpartum wind-stroke, use Jingjie Sui (Cacumen Schizonepetae) and Chao Dang Gui (Fried Radix Angelicae Sinensis), equal parts ground to powder, take 2-3 qian per dose, washed down with Dou Lin Jiu (Bean Drip Wine). (Also treats blood syncope.)
For postpartum blood confusion and syncope, take Qing Hun San (Clear Soul Powder): Ze Lan Ye (Herba Lycopi Japonici), Ren Shen (Radix Ginseng, each 2.5 qian), Jing Jie (Cacumen Schizonepetae, 1 liang), Chuan Xiong (Rhizoma Ligustici Chuanxiong, half liang), Gan Cao (Radix Glycyrrhizae Uralensis, 2 qian). Grind to powder, mix with half decoction and half wine, and take orally. For postpartum abdominal pain or spontaneous diarrhea, take Qing Liu Wan (Green Six Pills), swallowed with decoction of spleen-tonifying and blood-nourishing herbs. For postpartum diarrhea, use Bai Zhu (Rhizoma Atractylodis Macrocephalae), Chuan Xiong (Rhizoma Ligustici Chuanxiong), Fu Ling (Poria), Gan Jiang (Rhizoma Zingiberis Preparatum), Huang Qin (Radix Scutellariae Baicalensis), Hua Shi (Talcum), Chen Pi (Pericarpium Citri Reticulatae), Bai Shao Yao (Radix Paeoniae Alba). Stir-fry, crush, decoct, and take. For severe postpartum fever, Gan Jiang (Rhizoma Zingiberis Preparatum) must be used; use Fu Ling (Poria) lightly to leach out heat through urination; all bitter-cold diaphoretic drugs are forbidden. Someone asks: Why use Gan Jiang (Rhizoma Zingiberis Preparatum) for severe heat? Answer: This heat is not excess heat; it is generated by blood-moving herbs generating blood. Do not use alone; must be used with yin-tonifying herbs. This is the wonder of natural creation; only the most divine can understand it!
Postpartum fever and aversion to cold are both due to blood and qi deficiency. If the left hand pulse is insufficient, blood-tonifying herbs should exceed qi-tonifying herbs; if the right hand pulse is insufficient, qi-tonifying herbs should exceed blood-tonifying herbs. For postpartime aversion to cold, fever, and abdominal pain, one should remove evil blood. Yi Mu Cao (Herba Leonuri Japonici), also known as Chong Wei Zi (Fructus Leonuri Japonici), treats various diseases before and after delivery. When taking Siwu Tang (Four Substances Decoction) postpartum, do not use Bai Shao Yao (Radix Paeoniae Alba), because its sour and cold nature destroys the vitalizing qi. Those with robust constitution may still use it.
For postpartum agalactia, use Tong Cao (Medulla Tetrapanacis), Qu Mai (Herba Portulae), Jie Geng (Radix Platycodonis), Qing Pi (Pericarpium Citri Reticulatae Viride), Chai Hu (Radix Bupleuri), Bai Zhi (Radix Angelicae Dahuricae), Chi Shao Yao (Radix Paeoniae Rubra), Tian Hua Fen (Radix Trichosanthis), Lian Qiao (Fructus Forsythiae), Gan Cao (Radix Glycyrrhizae Uralensis). Decoct in water, drink slowly while full after meals, and massage the breast with one hand. For postpartum aversion to cold, fever, and agalactia, if breastfeeding is not desired, to suppress lactation, use Mai Ya (Fructus Hordei Germinatus, 2 liang), stir-fried and ground to powder, mixed with water, taken in four doses.
For postpartum edema, the main treatment must be greatly tonifying blood and qi, with slight addition of Cang Zhu (Rhizoma Atractylodis Lanceae) and Fu Ling (Poria) to allow water to pass naturally. Postpartum,败血 (败坏之血/defective blood) flows into meridians due to deficiency, rotting into water, causing swelling of limbs and face. Do not use water-draining drugs. First, use Wupi San (Five Barks Powder) plus Mu Dan Pi (Cortex Moutan Radicis) for 3-5 doses, then use Ju Fang Tiaojing San (Bureau Formula Regulating Menstruation Powder) for 20-30 doses. Once effective, blood moves freely and swelling subsides. Wupi San (Five Barks Powder): Wujia Pi (Cortex Acanthopanacis Senticosi), Digupi (Cortex Lycii Radicis), Shengjiang Pi (Pericarpium Zingiberis Recens), Sangbaipi (Cortex Mori Radicis), Fuling Pi (Poria cum Ligno), plus Mu Dan Pi (Cortex Moutan Radicis), decoct and take. Tiaojing San (Regulating Menstruation Powder): Dang Gui (Radix Angelicae Sinensis), Rou Gui (Cortex Cinnamomi Cassiae), Hu Po (Electrum, each 1 qian), She Xiang (Moschus), Xi Xin (Herba Asari, each 5 fen), Mo Yao (Resina Myrrhae, 1 qian), Chi Shao (Radix Paeoniae Rubra, 1 qian). Grind to powder, take 5 fen, add a little Ginger Juice, mix with warm wine.
For postpartum bleeding that does not stop, use Pu Huang (Pollinis Typhae, 3 liang), water 3 sheng, decoct to 1 sheng and take. For postpartum blood syncope with chest oppression and breathlessness, use Hong Hua (Flos Carthami Tinctorii, 1 liang), grind to powder, divide into two doses, decoct in 2 chan of wine to 1 chan, and take together. For trismus, pry open and administer. For various postpartum wind syndromes, use Cang Er Cao Zhi (Juice of Herba Xanthii Japonici), half chan, take warm. Toothache can also be treated. For postpartum millet-grain-like rash, heat like fire, mash Tao Ren (Semen Persicae), mix with December pig fat, and apply. For postpartum blood syncope near death, make Ban Xia (Rhizoma Pinelliae Ternatae) powder pills with water, size of soybeans, insert into nostrils to revive.
To expel dead fetus or retained placenta after delivery, use She Xiang (Moschus, half qian), Guan Gui (Cortex Cinnamomi Cassiae) powder (3 qian), wash down with warm wine; within moments, the placenta will be expelled like a hand pushing it out. One case: minor abortion,有形物 (tangible matter) not expelled, use Siwu Tang (Four Substances Decoction) plus Xiao (Natrii Sulfas). One woman, age 18, difficult labor, delivered after seven days, diarrhea, thirst, panting, red face with purple spots, lower abdominal distension, urinary retention. Use Niu Xi (Radix Achyranthis Bidentatae), Tao Ren (Semen Persicae), Dang Gui (Radix Angelicae Sinensis), Hong Hua (Flos Carthami Tinctorii), Mu Tong (Caulis Akebiae Trifoliatae), Hua Shi (Talcum), Gan Cao (Radix Glycyrrhizae Uralensis), Bai Zhu (Rhizoma Atractylodis Macrocephalae), Chen Pi (Pericarpium Citri Reticulatae), Fu Ling (Poria). Decoct, mix with Yimucao Gao (Leonurus Paste). Later replaced with Du Niu Xi (Radix Achyranthis Asperae), decoct to thick paste, one bowl, drink. After about one double-hour period, profuse diarrhea of one bucket, urination cleared, cured. For thirst, use Sijunzi Tang (Four Gentlemen Decoction) plus Dang Gui (Radix Angelicae Sinensis), Niu Xi (Radix Achyranthis Bidentatae), mix with Yimu Gao (Leonurus Paste).
One woman, postpartum illness due to fright and worry, heavy head, sensation of heaviness dropping in chest and heart, fear, feeling like floating in waves, unsettled. Use: Zhi Shi (Fructus Aurantii Immaturus), Mai Ya (Fructus Hordei Germinatus), Shen Qu (Massa Medicata Fermentata), Bei Mu (Bulbus Fritillariae Cirrhosae), Hou Sha (Rhizoma Zingiberis Officinalis var. Roscoe? Note: Text says 侯莎, likely typo for 厚朴 or similar, but keeping original entity name as per instruction "keep original"), Jiang Huang (Rhizoma Curcumae Longae, each 1.5 qian), Ban Xia (Rhizoma Pinelliae Ternatae, 2 qian), Tao Ren (Semen Persicae), Mu Dan Pi (Cortex Moutan Radicis), Gua Lou Zi (Semen Trichosanthis, each 1 qian), Hong Hua (Flos Carthami Tinctorii, 5 fen). Grind to powder, make pills with Ginger Cake. After taking, chest mass dissipated, but fright/unsettled feeling remained. Next use: Chen Sha (Cinnabaris), Yu Jin (Radix Curcumae Longae), Huang Lian (Rhizoma Coptidis Chinensis, each 3 qian), Dang Gui (Radix Angelicae Sinensis), Yuan Zhi (Radix Polygalae Tenuifoliae), Fu Shen (Poria cum Radix, each 2 qian), Zhen Zhu (Margarita), Ren Shen (Radix Ginseng), Sheng Gan Cao (Radix Glycyrrhizae Uralensis), Chang Pu (Rhizoma Acori Tatarinowii), Niu Huang (Calculus Bovis), Xiong Dan (Fel Ursi), Chen Xiang (Lignum Aquilariae Resinatum, each 1 qian), Hong Hua (Flos Carthami Tinctorii, 5 qian), Jin Bo (Lamina Aurea, one piece), Dan Xing (Folium Evodiae Preparatum, 3 qian). Grind to powder, make pills with Pig Heart Blood. After taking, fright/unsettled feeling reduced. Next use: Zhi Shi (Fructus Aurantii Immaturus), Ban Xia (Rhizoma Pinelliae Ternatae), Jiang Huang (Rhizoma Curcumae Longae), Shan Zha (Fructus Crataegi, Shen Qu (Massa Medicata Fermentata), Mai Ya (Fructus Hordei Germinatus), Chen Pi (Pericarpium Citri Reticulatae), Shan Zhi (Fructus Gardeniae Jasminoidis, each 5 qian), Bai Zhu (Rhizoma Atractylodis Macrocephalae, 1 liang). Grind to powder, make pills with Ginger Cake. Take this to assist stomach, digest food, resolve phlegm. Next use: Niu Huang (Calculus Bovis, 2 qian), Chang Pu (Rhizoma Acori Tatarinowii, 2.5 qian), Zhu Sha (Cinnabaris), Yu Jin (Radix Curcumae Longae, each 3 qian), Yuan Zhi (Radix Polygalae Tenuifoliae), Hu Po (Electrum, each 2.5 qian), Zhen Zhu (Margarita), Hong Hua (Flos Carthami Tinctorii), Chen Xiang (Lignum Aquilariae Resinatum, each 1 qian), Huang Lian (Rhizoma Coptidis Chinensis), Ren Shen (Radix Ginseng), Dan Xing (Folium Evodiae Preparatum, each 5 qian), Dang Gui (Radix Angelicae Sinensis). Grind to powder, make pills with Pig Heart Blood. Take this to calm heart and spirit. Next use: Gan Qi (Larix resin processed, 3 qian stir-fried until smoke gone), San Leng (Rhizoma Sparganii Stoloniferi), E Zhu (Rhizoma Curcumae Zedoariae, each 7.5 qian), Cang Zhu (Rhizoma Atractylodis Lanceae), Qing Pi (Pericarpium Citri Reticulatae Viride), Chen Pi (Pericarpium Citri Reticulatae), Zhen Sha (Sand?), Nong Po (Cortex Magnoliae Officinalis), Dang Gui (Radix Angelicae Sinensis, each 1 liang), Sheng Xiang Fu (Rhizoma Cyperi Rotundi, 2 liang). Grind to powder, make pills with Steamed Bread Dough. If these formulas were not taken, after Dao Cang (Inversion of Stomach) therapy, take decoction: Bai Zhu (Rhizoma Atractylodis Macrocephalae, 4 qian), Chen Pi (Pericarpium Citri Reticulatae), Huang Qin (Radix Scutellariae Baicalensis), Bai Shao Yao (Radix Paeoniae Alba), Xiang Fu Zi (Rhizoma Cyperi Rotundi, each 2 qian), Fu Ling (Poria, 1.5 qian), Dang Gui (Radix Angelicae Sinensis), Mai Dong (Radix Ophiopogonis Japonici), Qing Pi (Pericarpium Citri Reticulatae Viride, each 1 qian), Zhi Ke (Fructus Aurantii, 6 fen), Chen Xiang (Lignum Aquilariae Resinatum), Sheng Gan Cao (Radix Glycyrrhizae Uralensis, each 5 fen). Divide into six doses, to remove chest fullness, clear heat, and leach dampness. Treats women's Erzhen Tong (Placental Pain), decoct Tang Zi (Fructus Chaenomelis Speciosi) strongly, mix with brown sugar, take immediately effective.
Before and after delivery, mostly blood deficiency. One woman, nearly thirty years old, new delivery in January, left leg and right hand convulsions, panting unable to sleep, black qi rising from mouth/nose/face, pulse floating-stringy and deep-choppy, right hand more severe. Suspected spleen dampness syndrome. Asked: Did you have extreme thirst craving water during pregnancy? She said: At three months gestation, liked drinking soup/tea/water. So used Huang Qin (Radix Scutellariae Baicalensis), Jing Jie (Cacumen Schizonepetae), Mu Xiang (Radix Aucklandiae), Hua Shi (Talcum), Bai Zhu (Rhizoma Atractylodis Macrocephalae), Bing Lang (Arecae Semen), Chen Pi (Pericarpium Citri Reticulatae), Cang Zhu (Rhizoma Atractylodis Lanceae), Gan Cao (Radix Glycyrrhizae Uralensis), Shao Yao (Radix Paeoniae Alba). After four doses, added Tao Ren (Semen Persicae), took four more doses. Belly made gurgling sounds, stool passed, looked like crystal blocks, large ones like egg yolks, small ones like dozens of tadpoles. Then convulsions stopped, breathing eased. Removed Jing Jie (Cacumen Schizonepetae), Bing Lang (Arecae Semen), Hua Shi (Talcum) from prescription, added Dang Gui Shen (Radix Angelicae Sinensis body), Fu Ling (Poria), gave her to regulate blood vessels, took ten doses and was safe.
I once saw bladder rupture due to careless midwifery, leading to dribbling disease. Xu's wife, young adult, got this disease. I thought: External muscle/bone wounds can be repaired; although the bladder is inside, perhaps it can be treated too. Pulse was very deficient. Realized: Difficult labor people are mostly blood deficient; after difficult labor, qi and blood are especially deficient. Therefore used strong tonifying drugs, with Zhu (Atractylodes) and Shen (Ginseng) as sovereign, Tao Ren (Semen Persicae), Chen Pi (Pericarpium Citri Reticulatae), Huang Qi (Radix Astragali Membranacei), Fu Ling (Poria) as ministers, decocted with Pig/Sheep Bladder Soup, given at extreme hunger, one liang per dose. After one month, recovered. Probably because qi and blood suddenly grew, the bladder could heal; if delayed slightly, success might be difficult.
