Use four liang of licorice root (Gancao, 甘草) with nodes, and one bowl of long-flowing water. Roast the licorice root, soak it in the water until the water is completely absorbed, then grind it into a fine powder. Add a small amount of soap pod ash (Zaojiao Hui, 皂角灰). Divide into four doses. Mix with decoction and take all at once; this is highly effective. Another prescription: Licorice root nodes (Gancao Jie, 甘草节), Angelica dahurica (Baizhi, 白芷), and Coptis chinensis (Huanglian, 黄连) in equal parts. Coarsely crush, decoct in water. If the abscess has ruptured, apply dragon bone (Longgu, 龙骨), calcined alum (Kubai Fan, 枯白矾), and red kaolin (Chishizhi, 赤石脂) externally. A case of a patient with upper cough and lower kidney abscess rupture: Use Scrophularia ningpoensis (Xuanshen, 玄参), roasted Phellodendron amurense (Huangbai, 黄柏), Indigo naturalis (Qingdai, 青黛), rhinoceros horn (Xijiao, 犀角 - *note: modern substitute often used*), Hawthorn fruit (Shanzha, 山楂), licorice root nodes, Medicated leaven (Shenqu, 神曲), wheat bran (Mai Fu Pi, 麦麸 - *interpreted from context "Mai"*), peach kernel (Taoren, 桃仁), and Forsythia suspensa (Lianqiao, 连翘). Grind into powder and make pills. Prescription for inguinal abscess (Biandu, 便毒): Gardenia jasminoides (Shanzhi, 山栀), Rhubarb (Dahuang, 大黄), Frankincense (Ruxiang, 乳香), Myrrh (Moyao, 没药), Angelica sinensis (Danggui, 当归) (each 5 fen), Trichosanthes kirilowii seed (Gualou Ren, 栝蒌仁) (2 qian), Hematite (Daizheshi, 代赭石) (1 qian). Prepare as one dose for decoction. Another prescription: Momordica charantia seed (Mubiezi, 木鳖子), Rhubarb, Trichosanthes kirilowii seed, Gentiana scabra (Caolongdan, 草龙胆), peach kernel. Decoct strongly, leave exposed overnight, and take warm early in the morning. Another prescription: Silkworm cocoon (Baijiangcan, 白僵蚕), Sophora flower (Huaihua, 槐花), grind together into powder, mix with wine and take. One variation adds wine-processed Rhubarb. Another prescription: Semen Phaseoli (Lishi, 蠡实) root, three cun, equal part fresh ginger, grind finely, mix with hot water, take on an empty stomach. Another prescription: Rhubarb, Oyster shell (Muli, 牡蛎) (each 2.5 qian), one Trichosanthes kirilowii peeled, licorice root peeled. Coarsely cut as one packet, decoct in water, take on an empty stomach.
When heat reaches the extreme stage in the blood level, initially use Citrus reticulata green peel (Qingpi, 青皮) and licorice root nodes. Later, nourish the blood. For initial leg swelling, use Ginseng (Renshen, 人参), Astragalus membranaceus (Huangqi, 黄芪), Poria cocos (Fuling, 茯苓) (each 2 qian), and forty-eight Trichosanthes kirilowii seeds. Make two packets, add bamboo sap (Zhulü, 竹沥), and drink while hot. If pain persists at the Huantiao acupoint (Huantiao, 环跳), guard against the formation of deep-seated abscess (Fugu Yong, 附骨痈) (see detailed explanation in *Yiyao*).
Tiequan San (Iron Ring Powder) treats carbuncles, sores, and toxic swellings: Frankincense, Myrrh (each half liang), Rhubarb, Coptis chinensis, Phellodendron amurense, Arisaema erubescens (Nanxing, 南星), Pinellia ternata (Banxia, 半夏), Saposhnikovia divaricata (Fangfeng, 防风), Notopterygium incisum (Qianghuo, 羌活), soap pod, licorice root nodes, Aconitum carmichaelii lateral root (Caowu, 草乌), Donkey-hide gelatin (Ejiao, 阿胶) (added separately, each one liang above). Grind into powder, mix with vinegar to form a paste. Boil in a clay pot over fire until black. Apply to the affected area with a goose feather. For cold conditions, use heat application; for heat conditions, use cold application. For deep-rooted carbuncles (Dingchuang, 疔疮), must use needle and knife to rotate and break open. Apply toad venom (Chansu, 蟾酥) on the head of the sore, then use herbal medicine internally. Wild chrysanthemum ground into powder, mixed with wine and drunk until intoxicated. Upon waking, pain stops and heat dissipates; no need to remove the carbuncle root, it will heal itself. Method of extracting pus through the skin for various toxic swellings: Donkey hoof (roasted one liang, finely cut), buckwheat flour (roasted one liang), white salt (half liang), Aconitum carmichaelii lateral root (four qian, peeled). Grind into powder, mix with water, knead into cake shape. Slowly roast until slightly yellow, remove fire toxicity, grind into powder, mix with vinegar to form paste. Spread on white paper and apply to the affected area. Moisture exits naturally through hair follicles, swelling recedes spontaneously. For treating "Heaven Snake Head" (tip of finger swelling), use wild eggplant (Ye Luosu, 野落苏, i.e., Huangsi Cao), honeysuckle vine (Jinyinhua Teng, 金银花藤, i.e., Yang'er Teng), Wuyezicai, kudzu vine, Tianqiaomai. Cut finely, each one-tenth part. Decoct strongly with good rice vinegar. Fumigate first, then wash. Another prescription: Mix human feces with yellow mud, pound it, wrap around the affected area for relief. For treating "Heaven Fire Dan" (erysipelas), stir-fry yeast (Qu, 曲) with soil, grind finely, mix with sesame oil and apply. Another prescription: Pheasant feathers and goose feathers burned to ash, mix with sesame oil and apply. For treating all carbuncles, roots, stems, leaves, and flowers of Purple-stemmed Chrysanthemum can be used. Crush and extract juice, drip into mouth and drink. White wax has astringent properties (seen in *Yiyao*). To treat carbuncles and sores, place one layer of honeycomb (Lufengfang, 露蜂房) containing alum on a stone. Melt and calcine with fire, grind into powder, mix with oil and apply. Another prescription: Licorice root soaked in manure greatly treats toxic swellings; details are under the Winter Warmth section. When treating carbuncles and sores, one must differentiate by meridians. The Six Yang meridians and Six Yin meridians have variations of abundant qi and scarce blood, or abundant blood and scarce qi; they cannot be treated uniformly. Shaoyang meridian has abundant qi and scarce blood; muscles are difficult to regenerate. It is appropriate to prevent and expel toxins; purgative medicines should not be used lightly. My cousin, who suffered from excessive mental labor, was nearly fifty years old. There was red swelling like a chestnut on the outer side of his left arm. I said do not underestimate it. First give strong Ginseng decoction. Getting slight sweating is good. After taking dozens of packets, it stopped. More than ten days later, during a storm that uprooted trees, a red line appeared on the sore, circling the back to the right hypochondrium. I said large doses of Ginseng decoction plus Ligusticum wallichii (Xiong, 川芎) and Atractylodes macrocephala (Zhu, 白术) tonics are necessary. He took them for two months and recovered. Brother Li's son, thirty years old, had experienced continuous worry and hardship, indulged in lust, and worked hard. There was red swelling like a chestnut on the lower outer side of his left leg. One doctor gave Chengqi Tang (Big Order Downward Decoction) for two doses to purge. Another doctor taught to use Jiedu Tang (Detoxification Decoction) to purge. I examined him and said the pulse was large and excess. He subsequently died. The buttocks are located behind the lower abdomen and below, which is the utmost yin within yin. The pathway is distant, the position is remote. Although the Taiyang meridian has abundant blood, qi is difficult to circulate for long, and blood rarely reaches there. For those developing sores after middle age, preventive tonification is necessary. Without accumulated tonification efforts, disaster often occurs after the sore forms a scab, or manifests after half a year. Incompetent doctors fail to observe this, leading to mistakes. Be cautious and vigilant. When treating carbuncles and swellings, treatment should be differentiated based on whether it is a swelling or ulcer. Do not hastily use Wuxiang Lianqiao Tang (Five Fragrant Forsythia Decoction). Before rupture, support the interior and disperse; after rupture, tonify qi and blood. Press the swollen area with hand; if hot, there is pus; if not hot, there is no pus.
