Whenever purgatives are used to promote bowel movement, if the bowels remain unopened despite the use of drugs such as *Rhei Radix et Rhizoma* (Dahuang) and *Crotonis Fructus* (Badou), it is due to the mutual obstruction of Yin and Yang Qi, resulting in the failure of Water and Fire to ascend and descend. For cases with heat, *Laifu Dan* (Return-to-Life Pill) combined with *Sanhuang Wan* (Three-Yellows Pill) should be administered, taken down with a decoction infused with *Menthae Haplocalycis* (Bohe). For cases without heat, *Yangzheng Dan* (Nourish-Righteousness Pill) combined with *Shenbao Wan* (Spirit-Preserving Pill) should be administered, taken down with a decoction of *Citri Reticulatae Pericarpium* (Chenpi).
The harmonious interaction between the Spleen and Kidney allows for the differentiation of water and grain. Although the Spleen Qi may be strong, insufficiency of Kidney Qi leads to undigested food passing into the Large Intestine, causing diarrhea (*Sunxie*). The treatment method involves using *Ershen Wan* (Two-Geniuses Pill), compounded from *Psoraleae Fructus* (Guzhi) and *Myristicae Semen* (Roudoukou); alternatively, *Buhuanjin Zhengqi San* (Unchanging-Gold Rectifying-Energy Powder) can be used to swallow *Anshen Wan* (Peace-Kidney Pill). When the Qi of the Spleen and Kidney communicates smoothly, water and grain are naturally digested and transformed; this is referred to as "subtle integration and condensation."
There is Spleen-diarrhea and Kidney-diarrhea. Symptoms of Spleen-diarrhea include heavy and adherent limbs, a sensation of obstruction in the epigastrium (Zhongwan), a sallow complexion, and slight abdominal distension. Symptoms of Kidney-diarrhea include aversion to cold in the skin and pores, soreness and pain in the lower back, coughing accompanied by a darkened complexion, and pain in the umbilical and abdominal regions. To treat Spleen-diarrhea, herbs such as *Atractylodis Rhizoma* (Cangzhu), *Atractylodis Macrocephalae Rhizoma* (Baizhu), *Magnoliae Officinalis Cortex* (Houpo), *Zingiberis Rhizoma* (Ganjiang), *Aquari Flos* (Muxiang), and raw *Myristicae Semen* (Roudoukou) are employed. To treat Kidney-diarrhea, *Psoraleae Fructus* (Buguzhi), along with *Anshen Wan* and *Zhenling Dan* (Vibrating-Spirit Pill), are used.
For various types of diarrhea, formulas such as *Buhuanjin Zhengqi San* and *Chushi Tang* (Dampness-Removing Decoction) can be used to dispel wind-dampness and stabilize the intestines and stomach; these are considered superior treatments. Next in hierarchy are agents that warm the Spleen and nourish the Stomach, which constitute the orthodox method. However, if these methods prove ineffective, and abdominal pain migrates up and down, or there is dull pain in the umbilical region, soreness and pain in the lower back, weakness in the bones and joints, a withered and haggard complexion, and a weak Chi pulse, where lies the disease? It is said: this is Kidney-diarrhea. One should primarily use fried *Psoraleae Fructus* (Guzhi), raw dried *Zingiberis Rhizoma* (Ganjiang), *Cinnamomi Cortex* (Guangui), *Aquari Flos* (Muxiang), and *Angelicae Sinensis Radix* (Danggui), while still employing *Anshen Wan* as an adjunct. If Kidney-Yin is exhausted and Heart-Fire flares upward, leading to dry thirst, polyuria, and excessive drinking, this indicates the separation of Yin and Yang, a condition that astute physicians regard with concern.
In adults and children, if diarrhea and dysentery persist indefinitely, they may later transform into white pus dripping down, or appearing at the end of the stool. If warming the Spleen fails to cure it, the principle dictates warming the Kidney. Since the Kidney governs bone marrow, white pus is another name for bone marrow. Essentially, initial white dysentery has a fishy odor; after purgation and prolonged duration with salivation, it no longer qualifies as dysentery. The current white pus lacks any fishy odor, the complexion is slightly black, and physical strength is emaciated, clearly indicating Kidney deficiency. The appropriate prescription includes fried *Psoraleae Fructus* (Guzhi), *Angelicae Sinensis Radix* (Danggui), *Aquari Flos* (Muxiang), *Zingiberis Rhizoma* (Ganjiang), and *Cinnamomi Cortex* (Guangui), followed by small doses of *Zhenling Dan* or *Yuhua Bai Dan* (Jade-White Pill) as auxiliaries. Nevertheless, there is another syndrome in women that resembles dysentery but is not dysentery, characterized by the discharge of white pus and sudden severe pain in the heart and abdomen, accompanied by unbearable roaring sounds. This requires promoting blood circulation through blood-invigorating agents, assisted by *Yangzheng Dan*.
