To treat sudden apoplexy (stroke), characterized by loss of consciousness, hemiplegia, deviation of the mouth and eyes, trembling of the hands and feet, dysarthria (slurred speech), numbness and itching of the limbs, mental confusion, dizziness of the head and eyes, excessive phlegm and saliva, spasm of the sinews and vessels, as well as foot-leg weakness (Hong Kong foot) rendering the patient unable to walk or flex and extend.
Ingredients: Pheretima (Fangji), Cinnamon Heart (Guixin), Scutellaria Root (Huangqin), Apricot Seed (Xingren, peeled, tips removed, and stir-fried), Peony Root (Shaoyao), Licorice Root (Gancao, honey-fried), Szechwan Lovage Rhizome (Chuanxiong), Ephedra Stem (Mahuang, nodes removed), Ginseng Radix (Renshen). Each ingredient is used in an amount of one qian.
Crush the above herbs and divide into two doses. Add two cups of water, five slices of fresh ginger, and two jujubes. Decoct until eight-tenths of the liquid remains. Take hot after meals, with a slight delay. If there is mental vacillation, add Polypora (Fushen) and Polygala Root (Yuanzhi). If there is vexing pain in the bone joints with heat signs, add Peony Root and remove Aconite Root (Fuzi). If there is heart vexation and frequent fright, add Rhinoceros Horn (Xijiao) [Note: Substitute with Buffalo Horn in modern practice]. If there is heavy pain in the bone joints with cold-dampness, double the dosage of Cassia Twig (Guigui) and Aconite Root (Fuzi). If there is vomiting, regurgitation, and abdominal distension, add Pinellia Tuber (Banxia) and double the dosage of Ginseng Radix (Renshen). If there is restlessness, oppression, and constipation, remove Aconite Root (Fuzi), double the dosage of Peony Root (Shaoyao), and add Bamboo Sap (Zhuli). If there is diarrhea due to cold in the zang-organs, remove Pheretima (Fangji) and Scutellaria Root (Huangqin), and add Atractylodes Rhizome (Baizhu). If there is spontaneous sweating, remove Ephedra Stem (Mahuang) and Apricot Seed (Xingren), and add Atractylodes Rhizome (Baizhu). If there is weakness in the feet and knees, add Achyranthes Root (Niuxi) and Dendrobium Stem (Shihu). If there is body pain, add Gentian Root (Qinjiao). If there is abdominal pain, add Peach Kernel (Taoren) and Eucommia Bark (Duzhong, stir-fried with ginger).
In treating stroke, if one does not examine the manifestations of the Six Channels (Taiyang, Yangming, Shaoyang, Taiyin, Shaoyin, Jueyin) for appropriate modifications, treatment will be of no benefit. The methods for modification are detailed below.
To treat Taiyang stroke (disease pattern caused by pathogenic invasion of the Taiyang channel) with no sweat and aversion to cold:
Double the dosage of Ephedra Stem (Mahuang), Apricot Seed (Xingren), and Saposhnikovia Root (Fangfeng) in this formula.
To treat Taiyang stroke with sweat and aversion to wind:
Double the dosage of Cinnamon Twig (Guizhi), Peony Root (Shaoyao), and Apricot Seed (Xingren) in this formula.
To treat Yangming stroke with body heat, no sweat, no aversion to cold, but thirst:
Add Anemarrhena Rhizome (Zhimu) and Gypsum Fibrosum (Shigao), each 1.4 qian, and remove Aconite Root (Fuzi).
To treat Yangming stroke with body heat, sweat, and aversion to wind:
Add Kudzu Root (Gegen), Cassia Twig (Guigui), and Polyporus (Fuling), each 1.5 qian.
To treat Taiyin stroke with no sweat and cool body:
Double the dosage of Aconite Root (Fuzi) and add Dried Ginger (Ganjiang), 1 qian.
To treat Shaoyin stroke with sweat and no heat:
Double the dosage of Cinnamon Twig (Guizhi), Aconite Root (Fuzi), and Licorice Root (Gancao) in this formula.
