Pediatric patients with *Shanghan* (Cold Damage) presenting with fever, spontaneous sweating, and frequent crying should be treated with **Gegen Shaoyao Tang** (Kudzu and Peony Decoction).
*Radix Puerariae* (Gegen, Kudzu Root) (3 fen), *Radix Paeoniae Alba* (Shaoyao, White Peony Root), *Radix Glycyrrhizae* (Gancao, Licorice Root), *Radix Scutellariae* (Huangqin, Scutellaria Root), *Ramulus Cinnamomi* (Guizhi, Cinnamon Twig) (each 0.5 liang).
Grind into a coarse powder. For each dose, take 3 qian, add one *zhan* of water, decoct until reduced to 0.6 *zhan*, filter out the dregs, and take warm in two doses, administered sequentially. If the heat is severe, remove *Guizhi* and add *Rhizoma Cimicifugae* (Shengma, Black Cohosh Rhizome) 0.5 liang. If there is no sweating, add *Herba Ephedrae* (Mahuang, Ephedra Herb) 1 liang. If there is wheezing, add *Semen Pruni Armeniacae* (Xingren, Apricot Seed) 0.5 liang.
Pang Anshi stated: Pediatric *Shanghan* initially presents with persistent high fever. After being treated with purgative decoctions or pills, symptoms such as neck stiffness, upward deviation of the eyes, tongue protrusion, and convulsions occur, resembling epilepsy. Over time, this leads to choking sensations and loss of voice. Physicians often misdiagnose this as "Jingfeng" (Wind-Alarm), repeatedly administering substances such as *Cinnabar* (Zhusha), *Mercury* (Shuiyin), *Realgar* (Niuhuang - note: usually refers to Calculus Bovis but here likely mineral/animal mixtures), *Calomel* (Lvfen), *Radix Euphorbiae Lathyridis* (Badou, Croton Seed), and *Succus Bambusae* (Zhuli, Bamboo Sap). These medications prove ineffective. This condition arises because improper purgation causes toxic pathogens to knot in the chest and heart region. Internal heat generates phlegm-fluids, which encapsulate the ingested medicines, preventing their dispersion and circulation. Therefore, use **Dangxian San** (Phlegm-Dispersing Powder).
*Sublimated Arsenic Sulfide* (Fenshuai, Realgar sublimation product) (1 qian), *Calomel* (Nifen, Mercurous Chloride) (2 boxes), *Radix Euphorbiae Lathyridis* (Yuanhua, Croton Flower) (1 fen).
Grind into a fine powder. Administer with warm grain water. One-year-old infants take 0.5 qian; those with severe conditions may take another dose. If white sediment settles at the bottom, it is Calomel, which should be completely expelled. After some time, the child falls asleep and expels black-yellow phlegm-fluids encapsulating cinnabar-like particles, forming granules. The voice returns immediately, and the child stabilizes.
Pang Anshi stated: Pediatric "Jie Xiong" (Chest-Knotting Syndrome) presents similarly, but the child can cry. Physicians also often treat it as "Jingfeng." The pulse is floating and slippery. Pressing the epigastrium with fingers causes pain and crying. It is appropriate to administer **Banxia Huanglian Gualou Tang** (Pinellia, Scutellaria, and Trichosanthes Fruit Decoction) according to the situation. Purging yellow phlegm-fluids will lead to recovery. (The formula is found in the Chest-Knotting section.)
When pediatric *Shanghan* generates steaming wind-heat, leading to epilepsy with convulsions of the hands and feet and unconsciousness, use **Shepi Tang** (Snake Skin Decoction).
*Herba Ephedrae* (Mahuang, Ephedra Herb), *Radix et Rhizoma Rhei* (Dahuang, Rhubarb Root), *Concha Ostreae* (Muli, Oyster Shell), *Radix Scutellariae* (Huangqin, Scutellaria Root) (each 4 qian); *Lapis Aquae Frigidae* (Han Shuishi, Cold Water Stone), *Alumen Rubrum* (Baishizhi, White Alunstone), *Lapis Haematitae* (Chishizhi, Red Ochre), *Gypsum Fibrosum* (Shigao, Gypsum), *Flos Astrali* (Zhiwuying - *Note: Zhiwuying usually refers to Zinc Ore or similar, context implies mineral antipyretics*), *Talcum* (Huashi, Talc) (each 8 qian); *Radix Ginseng* (Renshen, Ginseng Root), *Ramulus Cinnamomi* (Guizhi, Cinnamon Twig), *Os Draconis* (Longchi, Dragon Tooth) (each 2 qian); *Radix Glycyrrhizae* (Gancao, Licorice Root) (3 qian); *Periostracum Bombyx Batryticatus* (She Tuipi, Snake Molt) (1 qian).
Grind into a coarse powder. For each dose, take 4 qian, add 1.5 *zhan* of water, decoct to 0.7 *zhan*. Take warm in two doses. If heat is prominent, take three doses. Decocting with water mixed with half *Succus Bambusae* (Bamboo Sap) is particularly effective.
After pediatric *Shanghan*, if there is heat in the stomach causing irritability, anorexia, evening tidal fever, flushed cheeks, restlessness, and vomiting, use **Lugen Tang** (Reed Rhizome Decoction).
*Rhizoma Phragmitis Communis* (Sheng Lugen, Fresh Reed Rhizome), *Radix Imperatae* (Sheng Maogen, Fresh Cogongrass Root), *Poria* (Chi Fuling, Red Poria), *Radix Scutellariae* (Zi Qin, Scutellaria Root), *Radix Ophiopogonis* (Maidong, Ophiopogon Root), *Radix Glycyrrhizae* (Gancao, Licorice Root), *Rhizoma Zingiberis Recens* (Shengjiang, Fresh Ginger) (each 1 fen); *Frumentum Tritici* (Xiaomai, Wheat Grain), *Oryza Sativa Glutinosa* (Nuomi, Glutinous Rice) (each 200 grains).
Cut finely. Add 1 sheng 2 he of water, decoct to 6 he. Filter out dregs and divide into three doses for immediate effect.
For pediatric *Shanghan* with cough, phlegm obstruction in the chest and diaphragm, and wheezing sounds in the throat, use **Shexian Tang** (Belamcanda Decoction).
*Rhizoma Belamcandae* (Shexian, Belamcanda Rhizome), *Herba Ephedrae* (Mahuang, Ephedra Herb), *Herba Asteris Tatarici* (Ziwan, Aster Root), *Cortex Cinnamomi* (Guixin, Cinnamon Bark), *Rhizoma Pinelliae Preparatum* (Banxia, Processed Pinellia Rhizome) (each 0.5 liang); *Radix Glycyrrhizae* (Gancao, Licorice Root) (1 fen).
Grind into a coarse powder. For each dose, take 3 qian, add 1.5 *zhan* of water and a little juice of *Rhizoma Zingiberis Recens* (Fresh Ginger Juice). Decoct to 0.6 *zhan*, filter out dregs, add half a spoonful of honey to dissolve evenly. Take warm in two doses.
Another formula, **Tiantingli Tang** (Sweet Lepidium Decoction).
*Semen Lepidii seu Descurainiae* (Tiantili, Lepidium Seed, stir-fried), *Semen Pruni Armeniacae* (Xingren, Apricot Seed, stir-fried), *Herba Ephedrae* (Mahuang, Ephedra Herb) (equal parts).
Grind into a coarse powder. For each dose, take 2 qian, add 1 *zhan* of water, decoct to 0.5 *zhan*. Take warm, adjusting the dose as needed.
After pediatric *Shanghan*, if night sweats, body heat, and dry throat occur, use **Xi Jiao Huang [Missing Character] Tang**.
*Corneus Rinocerotis* (Xijiao, Rhino Horn), *Sclerotium Poriae* (Fushen, Poria with Wood), *Radix Ophiopogonis* (Maidong, Ophiopogon Root), *Radix Astragali* (Huangqi, Astragalus Root - *assuming 'Huang' refers to Huangqi based on context of deficiency/heat*) or *Radix Scutellariae* (Huangqin, Scutellaria Root), *Radix Ginseng* (Renshen, Ginseng Root) (each 0.5 liang); *Radix Glycyrrhizae* (Gancao, Licorice Root) (1 fen).
Grind into a coarse powder. For each dose, take 3 qian, add 1 *zhan* of water, decoct to 0.5 *zhan*. Take warm.
If pediatric *Shanghan* interior syndrome is unresolved, leading to fright, delirious speech, mania, and tidal fever, use **Gouteng Dahuang Tang** (Uncaria and Rhubarb Decoction).
*Caulis Uncariae cum Uncis* (Goutengpi, Uncaria Stem with Hook), *Radix Angelicae Sinensis* (Danggui, Dang Gui Root), *Radix Glycyrrhizae Preparata* (Zhi Gancao, Prepared Licorice Root), *Radix Paeoniae Alba* (Shaoyao, White Peony Root) (each 0.5 liang); *Radix et Rhizoma Rhei* (Dahuang, Rhubarb Root) (3 fen).
Grind into a coarse powder. For each dose, take 3 qian, add 1 *zhan* of water, decoct to 0.6 *zhan*. Take warm. Stop when bowel movement is achieved. If difficult to purge, administer intermittently with **Yinchen Wan** (Artemisia Capillaris Pill). (The formula is in the Shanghan section.)
This formula not only treats *Shanghan* but also addresses pediatric food stagnation, induced fright/epilepsy, refusal to nurse, and lingering low-grade fever ("Wenzhuang"). It can be administered according to judgment, stopping when bowel movement is achieved.
