During the three months of winter, the Black Bone Warm Disease occurs. Its origin lies in the interaction and accumulation between the Taiyang (Greater Yang) and Shaoyin (Lesser Yin) meridians. This leads to blockage and congestion throughout the Sanjiao (Triple Burner), with Yin Toxin circulating internally. When the Zang-Fu organs are invaded by exogenous pathogenic qi, the disease arises.
In Kidney Warm Disease, the Yin and Yang Toxic Qi present opposing pathological manifestations. If the Fu organs are deficient, they are injured by Yin Toxin, presenting as internal heat with external cold; the patient desires warmth from fire and thirst for water, or experiences severe lower back pain as if broken. Conversely, if the Zang organs are excess, they are damaged by Yang Toxin, presenting as cutting pain in the chest and hypochondrium resembling knife stabs, abdominal distension, and profuse diarrhea following excessive use of cold-natured medicines (this formula should not be prepared as a decoction powder). Kushen Shigao Tang (Sophora Root and Gypsum Decoction).
Herbs: Kushen (Radix Sophorae Flavescentis), Sheng Gegen (Raw Radix Puerariae Lobatae) [each 2 liang], Shigao (Gypsum Fibrosum), Shi Dihuang (Fresh Rhizoma Rehmanniae) [each 4 liang], Zhizi Ren (Semen Gardeniae), Yichen (Herba Artemisiae Scopariae), Mangxiao (Natrii Sulfas) [each 1.5 liang], Xiangchi (Fermen Sojae Preparatum), Congbai (Bulbus Allii Cepae) [each 0.5 sheng].
Chop the herbs, add 8 sheng of water, and boil down to 3.5 sheng. Strain the dregs, add Mangxiao, and stir until dissolved. Take one zhan warm, three to four times daily.
Bian Que stated: Moxibustion on the Spleen, Liver, and Kidney meridians treats Dan Du (Carbuncle Poison) and Black Bone Warm Disease.
Zhimu Jieji Tang (Anemarrhena Muscle-Releasing Decoction) treats Warm-Heat Disease with symptoms such as headache, vexing pain in bones and muscles, dry mouth, and mental oppression. It is also indicated for epidemic toxic diseases in summer presenting with exterior cold and interior heat; or for cases where residual heat remains after purgation; or for cases where spontaneous bowel movement has occurred but deficiency heat, vexation, and thirst persist.
Herbs: Mahuang (Herba Ephedrae), Gancao (Radix Glycyrrhizae) [each 1 liang]; Zhimu (Rhizoma Anemarrhenae), Gegen (Radix Puerariae Lobatae) [each 1.5 liang]; Shigao (Gypsum Fibrosum) [3 liang].
Chop the herbs, add 3 sheng of water, and boil down to 1 sheng. Strain the dregs and take one zhan warm. If purgation or spontaneous bowel movement has already occurred and deficiency heat remains, remove Mahuang and increase Gegen to 3 liang. For patients who sweat spontaneously, add Gegen according to this method. For those with no sweat and difficulty inducing sweating, increase Mahuang to 1.5 liang. For cases complicated by diarrhea due to transformation, remove Mahuang and add Baiwei (Radix Cistanches) and Renshen (Radix Ginseng) [each 1 liang], add 4 sheng of water, and boil down to 1.5 sheng.
Hidden heat lodged in the Stomach causes chest fullness and rebellious Qi, leading to hiccups (Yue). Severe purgation may also cause stomach deficiency-cold, resulting in hiccups.
For Warm Disease with fever caused by sudden ingestion of cold water, Maogen Gegen Tang (Imperata and Pueraria Decoction) is used.
Herbs: Maogen (Radix Imperatae), Gegen (Radix Puerariae Lobatae) [each 0.5 sheng].
Add 4 sheng of water, boil down to 2 sheng, strain, and take one zhan warm.
If Warm Disease heat persists and thick quilts are suddenly removed causing cold exposure, Cold Toxin enters the Stomach and accumulates without dispersing, leading to transformation. Zipi Yinzi (Catalpa Bark Drink) is used.
Boil Catalpa bark juice alone and drink it slowly for best results.
If Warm Disease involves accumulation of cold drinks with cold knotting in the Stomach and heat entering the Kidneys, transforming into high fever and constipation (Da Bi), Zipi Tang (Catalpa Bark Decoction) is administered. A sign of heat in the Kidneys is that after recovery, the skin on the soles of the feet tends to peel off, and hair becomes sparse or falls out.
For Warm Disease with stomach cold causing transformation, Maogen Jupi Tang (Imperata and Tangerine Peel Decoction) is used.
Herbs: Baimaogen (Radix Imperatae) [0.5 sheng], Jupi (Pericarpium Citri Reticulatae) [1.5 liang], Guizhi (Ramulus Cinnamomi), Gegen (Radix Puerariae Lobatae) [each 1 liang].
Chop the herbs, add 3 sheng of water, boil down by half, strain, and take one zhan warm. Discontinue after hiccups stop. If there is slight heat, reduce Guizhi by 0.5 liang. (If slight heat is present, it is appropriate to remove Guizhi and add Shengjiang [Rhizoma Zingiberis Recens] 2 liang.)
For Warm Disease with fever caused by sudden ingestion of cold water, Pipa Maogen Tang (Loquat Leaf and Imperata Decoction) is used.
Herbs: Pipaye (Folium Eriobotryae), Maogen (Radix Imperatae) [each 0.5 sheng].
Add 4 sheng of water, boil down by half, strain, and drink one or two zhan slightly warm. (The above four formulas must not be prepared as decoction powders.)
Warm Disease refers to Heat entering the Kidneys. The Kidney dislikes dryness; excessive heat causes Kidney dryness, leading to thirst, prompting the patient to drink water to self-rescue.
Weiyu Tang (Melissa Decoction) treats Winter Warmth and Wind-Cold Strike in spring, with symptoms including fever, vertigo and headache, dry throat and stiff tongue, chest pain, chest and heart oppression, and stiffness in the waist and back.
Herbs: Weiyu (Rhizoma Polygonati Odorati), Baiwei (Radix Cistanches), Mahuang (Herba Ephedrae), Duhuo (Radix Angelicae Pubescentis), Da Xingren (Semen Armeniacae Amarum, raw), Xiong (Radix Chuanxiong), Gancao (Radix Glycyrrhizae), Qingmuxiang (Radix Aucklandiae), Gegen (Radix Puerariae Lobatae) [each 1 liang]; Shigao (Gypsum Fibrosum) [1.5 liang].
Chop the herbs, add 5 sheng of water, and boil down to 2.5 sheng. Strain and take one zhan orally to induce sweating. If alternating chills and fever occur, add Puxiao (Natrii Sulfas) 0.5 liang and Dahuang (Radix et Rhizoma Rhei) 1.5 liang, adding Puxiao powder at the end.
