Taiyang (Greater Yang) disease presents with stiffness of the neck. If there is no sweating and the pulse is floating and tight, Ma Huang Tang (Ephedra Decoction) is indicated; if there is sweating and the pulse is floating and slow, Gui Zhi Tang (Cinnamon Twig Decoction) is indicated. Jing syndrome (Spasm disorder) manifests as independent head shaking, sudden lockjaw, and neck stiffness, treated with Xiao Xu Ming Tang (Minor Continuation of Life Decoction). Jie Xiong syndrome (Chest Binding syndrome) presents with neck stiffness, resembling Rou Jing (Soft Spasm); after purgation, the condition harmonizes, treated with Da Xian Xiong Wan (Major Chest-Binding Pill).
Internal pain causes head shaking, which is a manifestation of Interior syndrome. Feng Jing (Wind Spasm) causes independent head shaking. When Heart Qi is exhausted, the head shakes, the complexion is dark like soot, and fixed staring indicates death. Wind pathogen governs movement; thus, head shaking mostly belongs to Wind. The pulse for Wind must be Xian (Wiry). (Shen Shu Tang (Divine Efficacy Art Decoction) can be modified with Tian Ma (Gastrodia Rhizome), Qiang Huo (Notopterygium Rhizome), Fang Feng (Saposhnikovia Root), and Jiang Can (Bombyx Batryticatus)).
Xuan (Dizziness) refers to vertigo and blurred vision, caused by deficiency in the Upper Jiao (Upper Burner) after sweating, vomiting, or purging. Shaoyang (Lesser Yang) opens into the eyes and resides between Exterior and Interior; as Exterior pathogens gradually enter the Interior, Yang deficiency in the Exterior leads to visual dizziness. If excessive sweating leads to delirious speech and dizziness, it indicates death.
After Taiyang disease has been treated with sweating, vomiting, or purging, both Exterior and Interior are deficient, inevitably leading to vertigo and faintness, treated with Zhen Wu Tang (True Warrior Decoction) modified with Chuan Xiong (Ligusticum Rhizome) and Tian Ma (Gastrodia Rhizome). If Taiyang disease persists after sweating, presenting with palpitations in the epigastrium, dizziness, muscle twitching, and body tremors threatening to fall, treat with Zhen Wu Tang (True Warrior Decoction) modified with Chuan Xiong (Ligusticum Rhizome) and Tian Ma (Gastrodia Rhizome). Yangming (Yang Brightness) disease presents with dizziness, aversion to cold absent, ability to eat but with cough, treated with Fu Ling Bai Zhu Gan Cao Sheng Jiang Tang (Poria, Atractylodes, Licorice, and Fresh Ginger Decoction) modified with Chuan Xiong (Ligusticum Rhizome) and Tian Ma (Gastrodia Rhizome). Shaoyang (Lesser Yang) disease presents with visual dizziness and vertigo, with a Xian (Wiry) and rapid pulse, treated with Xiao Chai Hu Tang (Minor Bupleurum Decoction) modified with Chuan Xiong (Ligusticum Rhizome) and Tian Ma (Gastrodia Rhizome). Cases complicated by Blood deficiency use Si Wu Chuan Ma Tang (Four Substances, Ligusticum, and Gastrodia Decoction). Cases complicated by Phlegm-Fire add Jiu Huang Qin (Wine-processed Scutellaria Root) and Zhu Li (Bamboo Sap). Cases complicated by Qi deficiency use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) modified with Chuan Xiong (Ligusticum Rhizome) and Tian Ma (Gastrodia Rhizome). In Shaoyin (Lesser Yin) disease, if diarrhea stops but dizziness and frequent faintness occur, it indicates death.
Body pain is a symptom present in all six meridians, differentiated into Exterior/Interior, Cold/Heat, and Wind-Damp. For Taiyang (Greater Yang), sweating is appropriate; if the pulse becomes Chen Chi (Deep and Slow) after sweating, warming the Interior is appropriate; [for Yangming Heat], Bai Hu Tang (White Tiger Decoction) is appropriate; for Interior Cold with Exterior Heat, rescue the Interior first then attack the Exterior; for Cold in the Three Yin meridians, the pulse is Deep; for Wind in the Three Yang meridians, the pulse is Floating; for Dampness invasion, the body is heavy and painful, unable to turn; for Yin Du (Yin Toxin), the body suffers severe pain as if beaten by a club.
Taiyang (Greater Yang) disease with body pain, no sweating, aversion to cold, and a Floating Tight pulse is treated with Ma Huang Tang (Ephedra Decoction). Body pain with sweating, aversion to wind, and a Floating Slow pulse is treated with Gui Zhi Tang (Cinnamon Twig Decoction). Shaoyin (Lesser Yin) disease with body pain, vomiting, diarrhea, and cold limbs is treated with Si Ni Tang (Frigid Extremities Decoction). Cold extremities, diarrhea, body pain as if beaten by a club, and vomiting/reversal are treated with Zhu Yu Si Ni Tang (Cornus Frigid Extremities Decoction). Whole-body pain, fever, yellow complexion, heat binding in the Interior, free urination, and constipation indicate Blood Stasis (Xue Yu) (Mild cases: Xi Jiao Di Huang Tang (Rhinoceros Horn and Rehmannia Decoction); Severe cases: Tao Ren Cheng Qi Tang (Peach Kernel and Order the Qi Decoction)). Wind-Damp causing whole-body pain, heaviness, inability to turn, and difficult urination is treated with Wu Ling San (Five Ingredient Powder with Poria) modified with Cang Zhu (Atractylodes Rhizome) and Qiang Huo (Notopterygium Rhizome). Huo Luan (Cholera-like disorder) with body pain and a Deep pulse is treated with Gui Zhi Tang (Cinnamon Twig Decoction). Exterior and Interior both Cold, with clear undigested stool diarrhea, and body pain requires rescuing the Interior first (Si Ni Tang / Frigid Extremities Decoction) then rescuing the Exterior (Gui Zhi Tang / Cinnamon Twig Decoction).
