以上少阴发温热例。
Various examples of warm diseases and heat diseases by Zhang Zhongjing have traditionally been mixed into the six-meridian examples of cold damage (Shanghan). This has led later generations to mistakenly treat cold damage with Huangqin Tang (Scutellaria Decoction) and Baihu Tang (White Tiger Decoction), or to mistakenly refer to syndromes treated by these decoctions as cold damage. This is entirely due to the confusion of categories and boundaries, causing ignorance for millennia. Since Zhang Zhongjing of Changsha, only Liu Wansu (Zishou Zhen) alone grasped the secret, yet he obscured the clear truth, not calling it "warm-heat" but still "cold damage." No wonder famous physicians of his time often made errors. Now, I separate the various clauses on warm-heat diseases into this chapter for scholars to read with a purified mind. It is like opening a jade box; in an instant, the fiery wheels and sword trees transform into a cool, clear earth.
In Taiyang disease, fever accompanied by thirst and aversion to cold absent indicates Warm Disease (Wenbing).
"Fever, thirst, and absence of aversion to cold" outlines that Warm Disease originates from within. Generally, if initially there is no aversion to cold but rather fever, irritability, and thirst, and within three to four days abdominal fullness or diarrhea occurs, this is Warm Disease.
If there was initial aversion to cold and fever for three to four days, followed by the exterior pathogen transmitting inward becoming irritability and thirst, this is Cold Damage where heat pathogen transmits inward manifesting as interior excess.
If sweating therapy has already been applied and the body becomes burning hot, it is named Wind-Warmth (Fengwen). In Wind-Warmth disease, the pulse is floating at all positions (cun, guan, chi), there is spontaneous sweating, heaviness of the body, excessive sleepiness, snoring breath, and difficulty speaking. If purgation is incorrectly applied, it leads to dysuria, staring eyes, and incontinence of urine and stool. If fire therapy (such as moxibustion or heat fumigation) is incorrectly applied, slight cases result in yellowing of the skin, while severe cases resemble convulsions or epilepsy with intermittent spasms. If heat fumigation is used, one error may lead to eye disorders, and a second error will shorten life expectancy.
This clause follows the previous one, stating "if sweating has been applied and the body becomes burning hot, it is named Wind-Warmth," indicating that Wind-Warmth arises from misapplying sweating to Warm Disease, which is distinct from Wind-Warmth caused by secondary wind invasion. The syndrome of Wind-Warmth involves both Taiyang and Shaoyin, with floating pulses at both cun and chi positions. Kidney water pulse should normally be deep; Wind-Warmth carries the pathogen upward from Taiyang, uprooting the foundation and disturbing the branches. The latent pathogen has been stagnant in the body for a long time; when the season arrives in the second lunar month, the pulse reveals itself first. Upon onset, exterior heat is in Taiyang and interior heat is in Shaoyin, both acting simultaneously. Reckless sweating without taboo causes burning heat to double, which is called Wind-Warmth. Both exterior and interior of Wind-Warmth show floating pulses. Symptoms include spontaneous sweating and body heaviness, indicating kidney water pathology; excessive sleepiness, snoring, and difficulty speaking indicate kidney organ pathology. These kidney signs appear simultaneously, signaling extreme danger. Ancient medical laws warn: "Wind-Warmth treatment focuses on Shaoyin; do not induce sweating. Death from sweating is murder by the physician." Unexpectedly, at the onset of fever, without careful pulse differentiation, rash sweating violates this major prohibition. Once wind pathogen leaves the kidney outward, aiding yang with yang, heat has no end. Incorrect purgation damages bladder qi transformation, leading to dysuria, staring eyes, and incontinence, cutting off Taiyang organ and channel qi. Incorrect fire therapy slightly damages Ying-Qi causing stasis-heat jaundice; severely, intense heat generates wind resembling convulsions and spasms. Fire-heat disrupts the spirit and disturbs the meridians. Zhang Zhongjing repeatedly warned against cauterizing needles and moxibustion in Cold Damage; for Warm syndromes, this must especially be avoided. Slight fire injury causes yellowing, disrupting spirit and meridians; this is the heaviest syndrome. Slightly lighter fire errors affect Shaoyin channel connecting to the throat, causing dry painful throat and even hemoptysis, often fatal. One error (sweating) delays death; a second error is beyond divine rescue. Therefore, treating Warm Disease relies on differentiating pulse and symptoms before sweating, preparing remedies and preventing danger. Once sweating occurs, disaster strikes suddenly, leaving no room for action. *Neijing: Cilre Lun* states: After Warm disease affects Ying-Qi, it connects internally to the Kidney. *Pingre Lun* states: Warm disease specifically discusses grain qi; if Kidney essence is sufficient, sweating occurs and life is preserved; if Kidney deficiency is severe and heat attacks, death ensues. The meaning is profound and complete. Zhang Zhongjing further elaborates on Zang-Fu theory, discussing Kidney and referencing Bladder. Injury to urination affects Bladder qi transformation; severe cases involve staring eyes and incontinence. Mingmen Kidney essence cannot illuminate, Shen-Shui (spiritual water/pupil function) is cut off. High pupil position indicates Taiyang insufficiency; fixed upward gaze indicates Taiyang exhaustion. Taiyang qi exhaustion means the little toe cannot flex or extend, characteristic of stroke-like sudden wind syndrome. Bladder exhaustion is unrecognized by people, hence the key to Wind-Warmth lies in the Bladder. If Kidney Zang is about to exhaust, does not Bladder exhaust first?
When Taiyang and Shaoyin combine, spontaneous diarrhea occurs; administer Huangqin Tang. If vomiting occurs, administer Huangqin Jia Banxia Shengjiang Tang (Scutellaria Decoction plus Pinellia and Fresh Ginger).
This refers to Taiyang-Shaoyin combined disease, clearly not the sequential transmission Shaoyin syndrome, but undoubtedly a combined Warm Disease syndrome. Because the patient's Middle Qi is originally deficient, heat pathogen cannot vent outward, so it attacks inward causing spontaneous diarrhea. Huangqin Tang disperses exterior and interior heat, differing significantly from Cold Damage treatment methods. Note: Huangqin Tang is the main formula for Warm Disease, essentially Guizhi Tang replacing Guizhi with Huangqin and removing Shengjiang. Guizhi treats exterior wind-cold; Huangqin treats interior wind-heat, an unchangeable rule. Shengjiang is acrid and dispersing, unsuitable for warm-heat, hence removed. However, for phlegm-fluid retention above the diaphragm, ginger and banxia are necessary, which does not exceed Cold Damage treatment methods. ○Note: Warm Disease onset should immediately use Huangqin Tang to clear heat. When Cold Damage transmits to Shaoyang, heat gradually enters the interior, then Huangqin can assist Chaihu for harmonization. This is the sequence of exterior-interior and cold-heat.
Yangming disease, pulse floating and tight, dry throat, bitter taste, abdominal fullness with panting, fever, sweating, no aversion to cold but aversion to heat, body heaviness. If sweating is induced, irritability, mental confusion, and delirious speech occur. If cauterizing needles are added, fright, restlessness, and insomnia result. If purgation is used, the stomach becomes empty, rebellious qi agitates the diaphragm, heart feels uncomfortable (Ao Nao), and tongue coating appears; administer Zhizi Chi Tang (Gardenia and Fermented Soybean Decoction). If thirsty for water, dry mouth and tongue, administer Baihu Jia Renshen Tang (White Tiger Decoction plus Ginseng). If pulse is floating with fever, thirsty for water, and dysuria, administer Zhu Ling Tang (Polyporus Decoction).
This is latent Qi (Fuqi) triggered by external pathogen, hence floating-tight pulse. However, floating-tight pulse with fever, sweating, no aversion to cold but aversion to heat indicates Warm Disease, similar to Yangming Cold Damage. Added dry throat, bitter taste, abdominal fullness, panting, and body heaviness clearly indicate warm-heat signs. Thus, sweating, purgation, and cauterization are contraindicated; Huangqin Tang and Baihu Tang are appropriate. Additionally, wind-cold rebellious qi in the diaphragm causes slippery tongue coating, prohibiting Huangqin-type herbs; thus, emesis with Zhizi Chi Tang is indicated. This treats Taiyang without harming Yangming. If prior symptoms add dry mouth and tongue, Baihu Tang clears heat and generates fluids. If adding fever, irritability, thirst, and dysuria, heat consumes Yangming fluids, requiring Zhu Ling Tang to guide heat out and nourish dryness. Ultimately, external cold pathogen in the stomach makes Huangqin/Baihu cold herbs difficult to use, so alternative paths are sought to disperse cold. ○For Cold Damage dysuria: floating pulse indicates Qi level, use Wu Ling San; sinking pulse indicates Blood level, use Zhu Ling Tang. For Warm-Heat dysuria: floating pulse indicates Exterior syndrome, use Zhu Ling Tang; sinking pulse indicates Interior syndrome, use Chengqi Tang. Cold Damage transmits from Qi to Blood; Warm-Heat emerges from Blood to Qi; do not let one hinder the other.
Yangming disease with profuse sweating and thirst: Zhu Ling Tang is contraindicated. Profuse sweating dries the Stomach; Zhu Ling Tang further promotes urination.
Taiyang Cold Damage affecting the root has Wu Ling San for dual resolution; Yangming Warm-Heat has Zhu Ling Tang to guide heat and nourish dryness. However, it is contraindicated when sweating is profuse and thirst exists. Yangming Stomach governs Jin-Ye (body fluids); sufficient fluids mean no thirst, insufficient means thirst. Yangming heat excess consumes fluids first; profuse sweating depletes them externally, and promoting urination depletes them internally, leading to immediate fluid loss. Only if pulse is floating with fever, thirst, dysuria, AND sweating is minimal, can Zhu Ling Tang be used. If pulse is floating with fever, thirst, dry mouth/tongue, AND sweating is profuse, Baihu Jia Renshen Tang is indicated (method detailed in previous clause). If pulse is sinking, steaming heat, severe thirst, and yellow-red scanty urine, Chengqi Tang purgation is required to urgently save Yin.
Combined disease of Three Yangs, pulse floating and large reaching the Guan position, desire to sleep, sweating upon closing eyes.
Warm-heat Qi reaches the exterior from within, so Combined Three Yangs is common. This clause describes Warm Disease, noting sweating only upon closing eyes; thus, frequent heavy sweating outside of heat disease is known. With interior-exterior heat and pathogen in all directions, the floating-large Guan pulse indicates Yangming heat. Sweating upon closing eyes belongs to Shaoyang; treatment should modify Xiao Chai Hu Tang, or Huangqin Tang plus Chai Hu is suitable. Assuming heat disease shows floating-large pulse, desire to sleep, and night sweats, this indicates original Qi deficiency, so heat intensity is not severe; Baihu Jia Renshen Tang is appropriate.
Combined disease of Three Yangs: abdominal fullness, body heaviness, difficulty turning over, insensitivity of mouth, dirty face, delirium, urinary incontinence. Sweating worsens delirium; purgation causes sweat on forehead and cold limbs. If spontaneous sweating occurs, administer Baihu Tang.
This describes heat disease combined with [mixed pathogens]. In summer heat, latent stagnant pathogens often erupt with summer heat into the Three Yangs, disturbing the Middle Jiao. Sweating here doubly depletes fluids, worsening delirium. Purgation traps Yang pathogen inward, causing cold limbs. Heat cannot vent, causing forehead sweat. Since neither sweating nor purgation is suitable, only Baihu Tang clears heat without hindering exterior-interior, making it urgent. Mistaking cold limbs for Yang deficiency is fatal.
Cold Damage with floating-slippery pulse: Exterior has cold, Interior has heat; administer Baihu Tang.
World editions state "Exterior has heat, Interior has cold," which is certainly a transcription error, unnoticed for millennia. I clarify this publicly. Baihu Tang treats heat/summer heat diseases; its nature is greatly cold. How could Interior cold take it? Examining floating-slippery pulse, slippery implies excess. This clearly indicates latent pathogen emerging to the exterior. As heat pathogen initially attacks muscle-skin, exterior Qi cannot overcome it, showing false cold externally ("Exterior has cold"). Latent pathogen emission is incomplete, interior heat remains strong ("Interior has heat"). Since there is no dry-stool solid heat, Baihu Tang disperses stagnant-emerging pathogen. Some say it should be "Exterior heat, Interior excess," similar to "excess" character; this is plausible. If truly Interior excess, Chengqi Tang would be used, not Baihu Tang. Note: This originally discusses Heat Disease. Why did Zhang Zhongjing call it Cold Damage? The hidden intent is for deep inquiry. Heat Disease results from winter failure to store essence, Yang Qi venting, sudden cold damage causing pathogen latent in marrow. In summer, heavy sweating releases heat pathogen, so it is termed Cold Damage. Latent pathogen emerges from marrow, passes through Pericardium to approach Yangming. Then Shi Gao saves Yangming heat, Zhi Mu clears Shaoyin source, Gan Cao and Jing Mi protect Pericardium and Lung Qi. Thus, Qi deficiency requires Renshen. Later physicians mistakenly use Baihu Tang for Winter Cold Damage fever. I reveal this to warn against rash Baihu Tang use in winter.
Cold Damage with slippery pulse and厥 (Jue/cold limbs): Interior has heat; administer Baihu Tang.
Slippery is Yang pulse, so Jue is Yang-Jue. Interior heat stagnation and blazing cause external aversion to cold and cold limbs, often with blue lips, face, and nails. Baihu Tang or Zhuye Shigao Tang clears stagnant heat for cure. ○This explicitly states Interior heat, confirming the previous "Exterior heat, Interior cold" is erroneous. Wang Shuhe mixed this into Jueyin Chapter due to slippery pulse and Jue; I reclassify it here.
Cold Damage with floating pulse, fever, no sweating, unresolved exterior: Baihu Tang is contraindicated. Thirst for water, no exterior syndrome: Baihu Jia Renshen Tang is indicated.
Baihu Tang clears heat but does not resolve exterior. If Heat Disease has slight acute cold pathogen with aversion to cold, headache, body ache (exterior signs), it is unusable. Must have surging/large pulse, or rapid pulse, irritability, heat, dryness, thirst. If slight non-seasonal exterior pathogen existed previously, after 2-3 days it disperses from exterior, revealing Heat Disease pulse/symptoms. Irritability/thirst for water indicates massive fluid consumption, beyond Baihu Tang's capacity; Renshen must be added to assist Jin-Qi for heat dispersion.
Cold Damage with no significant heat, dry mouth/thirst, irritability, slight aversion to cold on back: Baihu Jia Renshen Tang is indicated.
Intense internal latent heat causes dry mouth and irritability. True Yang cannot overcome pathogen, causing slight back aversion to cold, while exterior lacks significant heat. Baihu Tang clears internal heat toxin; Renshen assists True Qi.
Cold Damage: After vomiting or purgation, unresolved for 7-8 days, heat binds in interior, interior-exterior both hot, occasional aversion to wind, severe thirst, dry tongue with irritability, desire to drink several liters of water: Baihu Jia Renshen Tang is indicated.
Examining this clause, exterior signs are heavier than before, yet Baihu Jia Renshen Tang is used. "Heat binds in interior, interior-exterior both hot" emphasizes importance. Heat binding in interior prevents exterior heat removal; evil fire latent inside causes aversion to wind, severe thirst, dry tongue, irritability, continuous drinking. Urgent need is generating fluids and clearing heat.
After taking Guizhi Tang, profuse sweating occurs, severe irritability/thirst unresolved, surging pulse: Baihu Jia Renshen Tang is indicated.
This was originally Warm Disease, mistaken for Cold Epidemic and treated with Guizhi Tang. If Cold Epidemic, sweating after decoction would resolve it. Not knowing it was Warm Disease, Guizhi misuse led to surging pulse, profuse sweating, unresolved irritability/thirst. Misusing Mahuang would cause Wind-Warmth, burning heat, spontaneous sweating. Profuse sweating injures Jin-Ye; Renshen saves fluids. ○Note: Guizhi treats wind invading from exterior; Shi Gao treats heat emitting from interior. Thus Baihu Tang is the precise formula for heat pathogen. Though a Yangming resolving drug, it also clears internal steaming heat, not just meridian heat. Ancients thought Shi Gao's acrid-cool nature resolves Yangming wind-heat; this case seems similar but is actually different. Formulas like Da Qing Long Tang, Yue Bi Tang, Ma Xing Shi Gan Tang, Ma Huang Sheng Ma Tang all use exterior drugs together. Ignoring that heat pathogen injures Stomach, if pathogen is in meridian, even with severe heat/thirst, Ge Gen Tang or Gui Zhi Jia Ge Gen Tang are used, without needing Shi Gao. Thus, Cold Damage misusing Baihu/Huangqin, Warm Disease misusing Guizhi/Mahuang, light cases become heavy, heavy cases become fatal.
Above are three examples of Three Yangs initiating Warm-Heat.
The Master said: Latent Qi disease is diagnosed by intuition. Within this month, if there is latent Qi, assuming old latent Qi exists, pulse must be checked. If pulse is weak/faint, throat pain resembling injury (but not Hou Bi/blockage) occurs.
Patient says: Indeed throat pain. Although so, now diarrhea is desired.
Winter cold damage, latent in meridians, erupts in Spring; hence diagnosis by intuition. "Within this month" refers to Spring Equinox period. Weak pulse indicates original Yuan Qi deficiency, erupting not in Yang but in Yin. Shaoyin channel circles throat; latent heat rising causes initial throat pain resembling injury. Kidney controls opening/closing; Yin meridian heat cannot vent outward, inevitably attacking posterior, causing diarrhea.
Shaoyin disease, throat pain after 2-3 days: Give Gancao Tang. If not cured, give Jiegeng Tang.
Evil heat lodges in Shaoyin channel, causing throat pain. Gancao Tang moderates the trend; Jiegeng Tang lifts the pathogen. In 2-3 days, heat erupts in channel without other symptoms, so this is usable. By 5-6 days, Shaoyin diarrhea, vomiting, etc., arise, making this method unusable. ○Yin pathogen disease erupts violently; thus Shaoyin latent Qi eruption causes throat pain. Zhang Zhongjing uses mild treatment. Gancao is sweet and mild, treating Shaoyin latent Qi urgent warmth. Sweet enters Spleen; Spleen mildness reduces Yin fire. Raw Gancao clears fire; used alone for specific effect. If not cured, latent pathogen force is strong, mildness insufficient; Jiegeng lifts pathogen, sending Yin pathogen from Yang branch to disperse via Yang, preventing downward intrusion into Yin. If treatment is improper, Yin Jin consumed by heat, using heat-clearing/Yin-saving drugs may be too late. ○Note: Throat pain is mostly Yin pathogen battling Yang. Yin pathogen usually carries Dragon Fire momentum, so many belong to Shaoyin. Only Yangming meridian disease has "dizziness, no aversion to cold, able to eat, cough, inevitable throat pain," which is wind-heat with fluid attack upward, not comparable to Yin pathogen. Warm/Wind-Warm diseases often have this due to Yang pathogen latent in Yin. Even direct Shaoyin throat pain, though Yin pathogen knots in clear Yang, is Shaoyin channel. Thus Zhang Zhongjing specially sets Tong Mai Si Ni Tang to open depressed微弱 Yang in Yin, adding Jiegeng to clear throat/diaphragm.
Shaoyin disease with diarrhea, throat pain, chest fullness, irritability: Zhu Fu Tang is indicated.
Diarrhea, throat pain, chest fullness, irritability: Shaoyin latent Qi erupts in Yin meridian but is actually a heat syndrome. Evil heat fills upper, lower, middle, everywhere; cold-purgative drugs are unusable. Zhu Fu Tang moistens Shaoyin dryness, similar to using black donkey hide. Yang deficiency uses Fuzi to warm meridian; Yin exhaustion uses Zhu Fu to moisten dryness; both share dispelling pathogen meaning. Comparing them yields half the understanding.
Shaoyin disease, after 2-3 days, irritability, inability to lie down: Huanglian Ejiao Tang is indicated.
Shaoyin disease, irritability after 2-3 days, indicates not transmitted heat, but latent Qi warming eruption. Irritability/insomnia in 2-3 days. But irritability without dryness differs from True Yang movement. True Yang movement first spreads Yin Qi, causing vomiting, diarrhea, cold limbs, then irritability/dryness and profuse sweating. Here, only irritability/insomnia without vomiting/diarrhea/cold limbs indicates Yang irritability, where True Yin is boiled by heat pathogen. Urgent need is saving heat and preserving Yin.
Shaoyin disease with diarrhea for 6-7 days, cough, vomiting, thirst, irritability, insomnia: Zhu Ling Tang is indicated.
Diarrhea for 6-7 days is when heat goes and cold rises. Vomiting/thirst/irritability/insomnia indicates not only heat boiling, but True Yin combined with water-fluid battling Yang, causing pathogen retention. Zhu Ling Tang benefits water and moistens dryness; treating diarrhea stops diarrhea naturally.
Shaoyin disease, after 2-3 days, dry mouth/throat: Urgently purge, Da Chengqi Tang is suitable.
Latent Qi erupting in Shaoyin is most urgent, different from Cold Damage transmitted heat. Dry mouth/throat after only 2-3 days; delaying purgation to 5-6 days leads to withering difficulty. Thus urgent purgation saves Burning Kidney Water. ○Note: Two Shaoyin urgent purgation cases: one transmitted heat extreme; one heat entering Stomach Fu; one Warm-Heat originating in Shaoyin. All are urgent, requiring emergency rescue of dying Kidney Water, same origin different branch as Yangming urgent purgation three methods.
Above are three examples of Shaoyin initiating Warm-Heat.
