Any person falling from a height and injuring the five Zang organs, resulting in loss of consciousness and blocked Qi, should be examined for movement in the bilateral Taiyang acupoints and the chest and hypochondrium. If there is movement, it can be saved. Immediately blow Tongguan San (Passing Obstruction Powder) into the nose. If sneezing occurs and voice can be produced, administer Lidong Wan (Lidong Pill), followed by Fuyuan Huoxue Tang (Recovering Vitality Blood-Invigorating Decoction) to eliminate blood stasis and generate new blood. If treatment is delayed, it cannot be saved.
In cases of high fall where the cervical vertebrae are fractured, causing the neck to insert into the thoracic cavity while the body remains mobile on both sides, apply the "lifting" technique. For injuries from beating where the head cannot be raised, apply the "supporting" technique. For injuries from falling where the body tilts left or right, apply the "aligning" technique. For injuries where the face looks upward and the head cannot droop, or where tendons are elongated with bone dislocation, or where tendons are gathered and stiff preventing the head from drooping, apply the four techniques of pushing, supporting, connecting, and aligning. At the bedside, inquire whether the injury resulted from overturning vehicles or horses, falling from heights, or heavy falls. Also ask about appetite. If the limbs are uninjured, spirit is unimpaired, or the patient can sit up and move, the injury is mild. If the patient is comatose and silent, or cries out in pain, with coagulated blood stasis, swelling, hardness, and distended tendons, the injury is severe. Administer Sanhuang Baola Wan (Three-Yellow Precious Wax Pill), Jiegu Zijin Dan (Bone-Knitting Purple-Gold Elixir), apply Wanling Gao (Ten-Thousand Miracles Plaster), and hot compress with Dingtong San (Pain-Stabilizing Powder). Manual techniques are detailed later.
The vertex bone, known as Tianlinggai (Heavenly Spirit Cover), presents as a three-pronged suture in men and a cross-shaped suture in women. Located at the highest point, it encloses the brain marrow and governs the whole body. In cases of sudden death due to falls or blows, where the body is rigid but breath remains at the mouth and nose, and the heart region is warm and pulsating, rescue is possible. The patient should be seated with knees bent (the *Yizong Jinjian* states "lying on side"). First, apply Hunyuan Gao (Primordial Paste) mixed with vinegar to the vertex. Then, ignite rolled straw paper and have the patient groan. Administer Bali San (Eight-Centigram Powder) or Lidong Wan (Lidong Pill) mixed with children's urine. Once Qi returns and Yang energy recovers, apply manual therapy: massage the chest, heart, and axillae, support the wrists, and frequently rub the pulse hollows of both hands. Injury causes tendon and vessel stiffness; frequent rubbing restores Heart blood and circulates vital pulses, enabling revival. Subsequently, take Jiegu Zijin Dan (Bone-Knitting Purple-Gold Elixir), wash off previous medicines with Sanyu He Shang Tang (Stasis-Dispelling and Injury-Healing Decoction), and reapply Hunyuan Gao (Primordial Paste). If stool is dry, indicating internal blood stasis, take Jiajian Suzi Taoren Tang (Modified Perilla Seed and Peach Kernel Decoction). For deafness, insert Daoqi Tongyu Ding (Qi-Conducting Stasis-Unblocking Cone) into the ear. Diet should consist of plain rice porridge; avoid anger, greasy foods, and wheat products. Rest in a quiet room without noise. For severe cases clinically dead, wrap the head with white cloth, gently tap the sole of the foot with a wooden stick, pull the hair to straighten the neck, stretch the meridians, apply Hunyuan Gao externally, and take Zijin Dan internally. If the skin is intact but bones are shattered, membranes pierced, blood flows inward, voice is hoarse, face is cyanotic, and lips black, it is incurable. If the vertex bone collapses, disturbing brain marrow, with bleeding from the seven orifices, body rigid and convulsed, unconscious and unaware, it is incurable. If bones are crushed and marrow leaks out, it is incurable. If flesh is torn and bleeding does not stop, it is treatable. Use hemostatic cotton to seal the wound, stop bleeding first, then take Buqi Yangxue Tang (Qi-Nourishing and Blood-Supplementing Decoction). Avoid wind and cold, abstain from anger and sexual activity, wait for scabbing. If ulceration and pus occur, wash off sealing medicine with Gancong Jian (Sweet Scallion Decoction), sprinkle Tieshan San (Iron Fan Powder), cover with Yuhong Gao (Jade Red Plaster), take Shufeng Yangxue Tang (Wind-Dispersing and Blood-Nourishing Decoction), follow washing and changing protocols, until healing and scabbing occur.
