According to the *Douzhen Fang* (Formulas for Smallpox and Measles): For coughing before the eruption of smallpox/measles, use Shengma Tang (Cimicifugae Decoction). For headache, fever, aversion to cold, and coughing, use Shen Su Yin (Ginseng and Perilla Leaf Infusion). For vomiting of phlegm and saliva, use Baizhu Tang (Atractylodis Macrocephalae Decoction). For headache and coughing caused by seasonal pathogens, or residual toxicity causing coughing after the scabbing phase, use Xingxing San (Xingxing Powder). For failure of lesions to erupt, use Shengma Tang. For headache due to cold invasion, chest oppression, restlessness, and coughing, use Muxiang San (Costus Root Powder). For severe coughing with fever but no other symptoms, use Shengdihuang San (Rehmanniae Radix Powder). For coughing due to wind-heat, use Wuweizi Tang (Schisandrae Fructus Decoction). For throat discomfort, use Ganjie Fangfeng Tang (Licorice, Platycodon, and Saposhnikovia Decoction).
I believe that during the pre-eruptive or eruptive stage, these symptoms are caused by heat-toxin steaming upwards into the clear passages, disturbing Lung Qi. In such cases, Xingxing San is appropriate. After the lesions have erupted, however, the condition belongs to deficiency of Yuan Qi (Original Qi), which fails to secure the Couli (skin pores and interstitial spaces), allowing Wind pathogen to invade via the deficiency. In this case, Wuyi Gong San (Five-Agent Tonic Decoction) added with Jiegeng (Platycodon Root) and Wuweizi (Schisandra Fruit) should be used to tonify the Spleen and Lung.
A male patient presented with coughing, sneezing, red and white cheeks, red eyelids, and generalized erythema. I diagnosed this as a Heart-type rash. He was skeptical and did not take medicine; after more than ten days, the lesions became bright red and erupted successfully. I stated that since the lesions were bright red and erupting, no further medication was necessary. On the seventeenth day, he passed blood in his stool, and the pustules crusted over, leading to recovery.
A pediatric patient presented with bright red smallpox, high fever, severe coughing with phlegm, irritability, fever, and thirst. One dose of Renshen Qingge San (Ginseng Clear Diaphragm Powder) caused all symptoms to recede immediately. Daily consumption of celery juice led to scabbing within ten days.
A pediatric patient whose smallpox was gradually resolving still had a cough. The Lung pulse was large but forceless. Shen Su Yin was administered, and the cough gradually improved. Because the mother consumed alcohol, the cough recurred. Wuyi Gong San (Five-Agent Tonic Decoction) was modified with Sangbaipi (Morus Cortex), Xingren (Apricot Seed), and Zhizi (Gardenia Fruit). Both mother and child took the medicine and recovered.
A pediatric patient with smallpox developed coughing and expectoration on the twelfth day, accompanied by dull chest pain. The Lung pulse was rapid and slippery. I stated: "This is complicated by Fei Yong (Lung Abscess); Jiegeng Tang (Platycodon Decoction) should be used." The patient did not believe this. The next day, he indeed expectorated pus and blood. Treatment with Jiegeng Tang resulted in recovery.
A fourteen-year-old boy had coughing after smallpox resolution. The pulse was rapid but forceless. He took Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) in the morning and Liuwei Wan (Six-Ingredient Pill with Rehmannia) ingredients in the evening. After several doses, he gradually improved. After marriage, he experienced recurrent coughing and fever. He was treated again with the previous formulas and Bazhen Tang (Eight-Treasure Decoction), leading to recovery.
A pediatric patient nearing recovery from smallpox had coughing and a sallow-white complexion. His face turned red when coughing severely. Wuyi Gong San (Five-Agent Tonic Decoction) was used for regulation and tonification, resulting in recovery.
Shengdihuang San (Rehmanniae Radix Powder): Treats pediatric measles/rash with symptoms such as body heat, dry mouth, coughing, and irritability.

Usage: Take 3–5 qian per dose, decoct in water, and administer slowly. Increase dosage for older children. If phlegm-qi and internal rash-heat arise, use Jiegeng Gancao Fangfeng Tang (Platycodon, Licorice, and Saposhnikovia Decoction). If phlegm obstructs upwards, assist with Baolong Wan (Embrace Dragon Pill).
Jiegeng Tang (Platycodon Decoction): Treats chronic cough damaging Lung Qi, expectoration of bloody or foul-smelling sputum, expectoration of pus and blood, and Lung Abscess.
