For women aged twenty to thirty, who are infertile despite having no other diseases, with regular or irregular menstruation, this is due to the failure of Yin Qi (vital energy associated with substance and cooling) to ascend and Yang Qi (vital energy associated with function and warming) to descend. Dusheng San (Single Holy Powder) may be used; after inducing vomiting to expel two or three sheng [approx. 0.6-1 liter] of cold phlegm, Dao Shui Wan (Water-Guiding Pill) and Yu Gong San (Yu's Meritorious Powder) should be administered to induce diarrhea three to five times, or even more than ten times; alternatively, Wuyou San (Worry-Free Powder) may be used to induce diarrhea over ten times. Subsequently, a diet of white porridge with scallions and vinegar should be consumed for three to five days. Once the Stomach Qi (digestive energy) is unblocked and the intestines are filled, Yuzhuo San (Jade Candle Powder) can be taken, supplemented by Guiling Baizhu Wan/San (Cinnamon Twig, Poria, and Atractylodes Macrocephala Pill/Powder). These two preparations lower Heart Fire (excess heat in the Heart system) and nourish Kidney Water (Kidney Yin), following the principle of Ji Ji (Mutual Interaction of Water and Fire); pregnancy will surely occur within a few months.
If a woman suffers from symptoms such as long bi (difficulty in urination), yin ni (urinary incontinence), or yi gan (dry throat), she will not conceive even with medication and acupuncture. This is because the Chong Mai (Penetrating Vessel), Ren Mai (Conception Vessel), and Du Mai (Governor Vessel) are diseased, which constitutes an incurable condition. Exterior syndromes presenting with interior signs are discussed in the Treatise on Heat Diseases.
A "half-birth" (spontaneous abortion/miscarriage) in women, commonly called xiao chan, refers to miscarriage occurring at three months, or four, five, or six months, because the male or female characteristics of the fetus have already formed. Causes may include sudden worry, fear, or rage, excessive grief; or physical exertion, trauma from blows or falls, exposure to wind-cold; or exposure to extreme heat. Formulas containing Gan Jiang (Dried Ginger), such as Hei Shen San (Black Divine Powder) and Wu Jin San (Black Gold Powder), must not be used internally as they violate the interior. Only formulas like Yuzhuo San (Jade Candle Powder) and Hejing San Tang (Harmonizing the Channels Decoction) should be used.
A "full birth" (term delivery) refers to childbirth after ten full months. If there is pain in the umbilicus and lower back, it is caused by retained blood and foul matter. The world generally mistakes this for deficiency-cold and treats it with drying-warmth methods, causing much harm. The Nan Jing (Classic of Difficult Issues) states: "All pain belongs to excess." Excess implies heat. Dao Shui Wan (Water-Guiding Pill) and Yu Gong San (Yu's Meritorious Powder) can be used to induce diarrhea five to seven times. One must strictly avoid immediately administering drying agents like Hei Shen San (Black Divine Powder) or Wu Jin San (Black Gold Powder). Treating it according to the protocol for half-birth is acceptable.
For postpartum Xin Feng (Heart Wind syndrome), Tiaowei Chengqi Tang (Stomach-Harmonizing Bowel-Purging Decoction) one or two liang [approx. 30-60g], plus Dang Gui (Angelica Sinensis) half a liang [approx. 15g], finely chopped, is decocted with three to four zhan [approx. 600-800ml] of water. After straining, divide into two doses. Curing occurs after profuse defecation of three to five times. If not cured, use Sansheng San (Three Holy Powders) to induce vomiting.
