Any condition of Shanghan (Cold Damage), Zhongfeng (Wind Strike), Wenbing (Warm Disease), Shiqi (Seasonal Epidemic), or Maoshu (Heatstroke) results from contracting the abnormal Qi of the four seasons. If one is infected by such pathogenic toxic Qi, the pathogen initially affects the Juyang Meridian (Taiyang Meridian), on the second day it affects the Yangming Meridian, and on the third day it affects the Shaoyang Meridian. In the first three days, the pathogen is in the exterior Yang; in the subsequent three days, it is in the interior Yin. In the *Neijing·Re Lun* (Treatise on Heat Diseases), these are collectively referred to as Shanghan. "Re Bing" (Heat Disease) refers to excessive heat throughout the body; "Shanghan" refers to disease caused by external contraction of Cold Evil. Regarding the chills and fever of Shanghan: Chills (aversion to cold) indicate exterior heat with interior harmony, hence the pulse is floating and large; Fever indicates interior heat with exterior harmony, hence the pulse is slippery and solid. This can be resolved using the Vomiting Method, primarily using Baxue Tang (Snow-Pulling Decoction). Decoct fresh ginger, scallion white, and fermented soybean together, combined with Tingli Kujiu Tang (Descurainia Seed and Vinegar Decoction), to expel the pathogen upwards. If the patient's pulse is deep and solid, or if there is constipation accompanied by wheezing, abdominal fullness, and delirious speech, do not be constrained by the number of days since onset; urgently attack the interior using Tongjie Wan (Unblocking Decoction Pill); if there is dryness and thirst in the stomach, use Dachengqi Tang (Major Order the Qi Decoction) to purge. One must absolutely avoid using hot-natured drugs such as Silver Powder, Radix Crotonis Preparata (Badou Frost), Semen Armeniacae Amarum (Xingren), and Genkwa Flos (Yuanhua); using them will inevitably lead to critical danger. Zhang Zhongjing said: "Regulating Shanghan lies in the principles of Sweating and Purging." One must clearly distinguish between exterior and interior conditions, and no case will remain uncured! A difference of a hair's breadth leads to an error of a thousand miles; this must be approached with extreme caution. After sweating or purging, one must strictly observe dietary restrictions, consuming light rice gruel to nourish the body; otherwise, the illness will recur.
Furthermore, discussing Shanghan after seven or eight days, presenting with tidal fever, abdominal fullness, jaundice, and macules: which meridians are responsible? The *Neijing* states that the Hand-Taiyin Lung Meridian, Foot-Taiyin Spleen Meridian, Foot-Yangming Stomach Meridian, and Hand-Shaoyin Heart Meridian are affected. Zhang Zhongjing stated that if both Cun-kou pulses at the wrists are floating and slippery, indicating phlegm turbidity attacking upward in the chest, Gouti San (Melon Sepal Powder) can be used to induce vomiting; after vomiting, treat according to the specific syndrome. Jaundice arises from Yangming Wind-Strike and Taiyang Dampness, where blood stasis struggles with retained food residues, causing yellowing. Decoct Zhi Zi Yin Chen Hao Tang (Gardenia, Capillaris, and Artemisia Decoction) and take Jiajian Wuling San (Modified Five-Ingredient Poria Powder) afterwards. If urination becomes smooth and rapid, with urine resembling the juice of soap pods, this indicates efficacy. Macules result from Heat affecting the Heart Meridian. Analyzing the principle of medication based on severity: for mild cases with red macules, Yue Tao Yin Zi (Prunus Juice Drink) can be used; for severe cases with purple macules, indicating abundant toxic Qi in the stomach, Da Qing Siwu Tang (Isatis Four Substances Decoction) and Yuanshen Shengma Tang (Scrophularia and Cimicifuga Decoction) are primary. Tidal fever and abdominal fullness indicate pathogenic heat in the Stomach; one should cleanse the pathogenic heat and moisten dryness, treating it urgently. In miscellaneous diseases, a deep and solid Cun-kou pulse also indicates pathology in the chest. Qixuanzi (Wang Bing) annotated: "If upper excess persists, use vomiting to deprive the pathogen, and the disease will heal." Black macules indicate a critical and difficult-to-treat condition. For jaundice and blood diseases, inquire about the ease or difficulty of urination. Additionally, verify if there is headache lasting several days; this indicates pathology in the Six Yang Meridians. The Three Hand-Yang Meridians run from hand to head; the Three Foot-Yang Meridians run from head to foot. This is the convergence of the Six Yangs. Chronic unrelenting pain can lead to blindness due to retained phlegm in the chest and diaphragm. First use Qianghuo San (Notopterygium Powder) to induce vomiting, then select and take Chuanxiong Shigao San (Ligusticum and Gypsum Powder) or Baihu Tang (White Tiger Decoction) for recovery.
Another method: For persistent headaches, decoct Lianxu Congbai Douchi Tang (Scallion White with Fermented Soybean Decoction with Roots), take frequently until vomiting occurs as a sign of effect; after vomiting, take Chuanxiong Bohe Tang (Ligusticum and Mint Decoction), a pungent-cooling agent clearing the upper jiao, such as Shufeng Wan/San (Wind-Dispersing Pills/Powders). Zhang Zhongjing said: Shanghan headache with a rapid Cun-kou pulse corresponds to this presentation.
For men suffering from yellow complexion, body heat, emaciation, alternating chills and fever resembling malaria, incessant salivation and coughing, or wheezing and abdominal fullness, facial and ocular edema, generalized body heat, or spontaneous sweating: The *Neijing* names this Shanghan Jialao Syndrome (Shanghan complicated with Deficiency Consumption). How to treat? For pathology located high, exploit its position to cause evacuation upwards. Use Fangji San (Stephania Powder) to induce vomiting; after vomiting, initially administer one dose of Tongjie Wan; subsequently choose from Renshen Huangqi San (Ginseng and Astragalus Powder), Danggui Yin Zi (Angelica Drink), or Jiajian Xiaochaihu Tang (Modified Minor Bupleurum Decoction). The *Neijing* states that male and female syndromes are often similar and share treatment methods. Following this regulation ensures efficacy.
For individuals suffering from chest and epigastric stuffiness, loss of appetite, high fever, dry mouth and tongue, and urinary and fecal obstruction: One physician diagnosed lower origin deficiency-cold and administered numerous doses of warming medicines without success. Another physician offered the same diagnosis and administered warming medicines for five to seven days, worsening the patient's weakness. Upon my examination, I asked about the duration, diet, and bowel/urinary status. The response was dark red urine and black stool. I diagnosed this as Shanghan Blood Stasis. Initially, two doses of Dahuang Shaoyao Tang (Rhubarb and Peony Decoction) were given, followed by two doses of Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction), and finally one dose of Tongjing Wan (Meridian-Unblocking Pill). When stools turned yellow, this was considered effective. After taking over ten doses of these medications, the disease subsided.
For men and women presenting with skin heat, muscle wasting, limb fatigue, headache, flushed cheeks, palpitations, dry lips and tongue, afternoon tidal fever, night sweats, thick sticky sputum, chest discomfort, occasional wheezing and coughing, five-palm heat, restlessness, reduced appetite, excessive thirst, and amenorrhea: What is this disease? The *Qibing Lun* (Treatise on Rare Diseases) states: Amenorrhea in women is Blood Stasis; Kidney Deficiency and insufficient Essence in men. Treatment involves descending Heart Fire and benefiting Kidney Water. First use Tongjie Wan to induce two or three bowel movements, then take Danggui Yin Zi, followed by Jiajian Wuling San, Muxiang Sanleng Wan (Aucklandia and Curcuma Pill), Renshen Huangqi San, or Xijiao San, selecting based on deficiency or excess. If expectorating pus or blood, or bleeding from bowels/bladder, indicating blood loss, do not use vomiting methods, and strictly avoid pungent-hot substances like Ginger and Aconite. Violating precautions for blood-loss syndromes by using sweating or warming-tonifying methods, or administering Spleen/Stomach tonics, may allow intake of food but prevents muscle growth, leading to critical deterioration into Xu Lao (Deficiency Consumption).
Physicians who fail to understand releasing the exterior and attacking the interior, indiscriminately prescribing decoctions, risk lives. It is strictly forbidden to perform burning moxibustion on Zhongwan, below the navel, Guanyuan, Qihai, Back-Shu points, and Zusanli during summer. Hundreds of moxa cones yield no effect and only harm the patient, which is tragic. Patients with Xu Lao must avoid meat, noodles, and pungent/sour foods. They should only consume foods aligned with Shennong's Dietary Therapy. Spinach, Malva, and cool water are permissible and encouraged to keep the intestines and stomach lubricated, preventing Qi and Blood stagnation and dryness. The *Classic* states: "When grain enters the stomach, the vessel pathways flow; when water enters the meridians, blood is formed." Failure to observe dietary restrictions causes gastric closure and stenosis, leading to gradual emaciation and death. All consumption disorders follow this principle. For those expectorating pus/blood, epistaxis, or hematochezia/hematuria, take Sanhuang Wan (Three Yellow Pill), Huanglian Jiedu Wan (Coptis Detoxification Pill), Liangge San (Cool the Diaphragm Powder) plus Platycodon, Angelica, Rhubarb, Peony, and Xijiao Dihuang Tang, prepared in large quantities and taken frequently in small sips. The *Neijing* states: "This refers to diseases where pathogenic heat injures the Liver and Heart." Adhering to this regulation ensures complete success.
For individuals over forty, whose chronic excessive talking has led to deficiency consumption, dark complexion, tasteless diet, chest stuffiness, limb fatigue, residual body heat, and obstructed urination/defecation: How to treat? The *Neijing* says: Do not use hot tonics. Men with Kidney Deficiency lack Water; most deficiency tonics use Aconite, Prepared Aconite, or Tianxiong, ignoring that the Kidney dislikes dryness. Women with Yin Deficiency lack Blood; most tonics use Yang agents, resulting in Yang excess and Yin deficiency. Do not use hot-natured drugs for tonification. On an empty stomach, take Jiabian Bawu Wan (Modified Eight-Ingredient Pill), Danggui Yin Zi, or Jian Gui Wuling San. For irritability and thirst, add Yi Yuan San (Benefit Origin Powder), named Danshen San (Light Percolation Powder). Also take Tongjie Wan or Xianren Wan, which have miraculous effects.
What is Zangdu Xiexue (Visceral Toxemia with Bleeding)? The *Shengqi Tongtian Lun* states: Pathogenic heat injures the Liver; following gluttony, tendons and vessels relax, leading to hemorrhoids. Pus and blood result from blood flowing with heat, entering the intestines and stomach irregularly, causing bloody diarrhea. If accompanied by high fever, it is difficult to treat; if the body is cool, it is treatable. First regulate the middle and eliminate blood stasis, purging two or three times; after purging, take Shaoyao Bapi Wan (Peony and Phellodendron Pill), Huanglian Jiedu Tang (Coptis Detoxification Decoction), Wuling San, and Yi Yuan San in equal parts, mixed with fresh water, 5-7 qian. Severe cases may recover by taking half a cup of Rehmannia Juice.
For dysentery with pus and blood and unceasing abdominal pain: Why? All pain and itching belong to Heart Fire. Use modified Tongjie Wan to purge according to deficiency/excess; then take Xiaoshi San (Dampness-Resolving Powder) with fresh ginger, jujube, and peony; after purging, take Wuling San mixed with fresh water. Alternatively, decoct Centella Asiatica (Dengxin Cao) soup and mix with Yi Yuan San, 5-7 qian. This disease strictly forbids greasy, fishy, and hot foods.
