If a disease occurs first, followed by Qi rebellion (such as vomiting or hiccups characterized by upward rushing of Qi), treat the root cause. If Qi rebellion occurs first, followed by other diseases, treat the root cause. If coldness appears first, leading to subsequent disease, treat the root cause. If other diseases appear first, followed by cold symptoms, treat the root cause. If heat appears first, leading to subsequent disease, treat the root cause. If diarrhea appears first, leading to other diseases, treat the root cause; one must first regulate the diarrhea before treating other diseases. If other diseases appear first, followed by abdominal fullness (Zhongman), treat the branch manifestation (abdominal fullness). If other diseases appear first, followed by diarrhea, treat the root cause. If abdominal fullness appears first, followed by vexation in the heart, treat the root cause (abdominal fullness).
The term "Ke Qi" refers to exogenous pathogenic factors, while "Tong Qi" refers to endogenous Qi or similar pathogens. In cases of difficulty with urination and defecation, treat the branch manifestation (promoting free flow of urine and stool); in cases where urination and defecation are unobstructed, treat the root cause. When the pathological condition is excess and vigorous, treat the root first and the branch later; when the condition is deficiency and weak, treat the branch first and the root later. Carefully observe the severity and mildness of the disease, and adjust treatment flexibly according to circumstances. For mild conditions, treat both root and branch simultaneously; for severe conditions, treat either the root or the branch exclusively. If difficulty with urination and defecation occurs first, followed by other diseases, treat the root cause (i.e., promoting free flow of urine and stool is considered the root).
This text contains no extraordinary elements, and its principles are quite simple, yet its phrasing is firm and concise, which is commendable. The usage of terms like "Ke Qi" and "Tong Qi," as well as "simultaneous action" and "exclusive action," is truly refined like hundred-times-refined gold.
In Jue syndrome accompanied by pain along the spine extending to the top of the head, with a heavy sensation in the head, blurred vision, and stiffness in the lower back and spine, puncture the blood collaterals at the popliteal fossa of the Foot-Taiyang Bladder Meridian. In Jue syndrome presenting with chest fullness, facial swelling, moist lips with drooling, difficulty speaking, and in severe cases inability to speak, select points on the Foot-Yangming Stomach Meridian. When Jue Qi rushes to the throat causing inability to speak, cold hands and feet, and constipation, select points on the Foot-Shaoyin Kidney Meridian. In Jue syndrome presenting with abdominal distension with sound, excessive cold qi, rumbling intestines, and difficulty in urination and defecation, select points on the Foot-Taiyin Spleen Meridian. For dry throat, heat in the mouth, and a sticky sensation like glue, select points on the Foot-Shaoyin Kidney Meridian. For knee pain, use the Dubi (ST35) point with a Yuanli needle; puncture during an attack and then at intervals. The needle should be as thick as a hair; puncturing the knee is without doubt. For Bi syndrome of the throat with inability to speak, select points on the Foot-Yangming Stomach Meridian; if speech is possible, select points on the Hand-Yangming Large Intestine Meridian. For malaria without thirst occurring every other day, select points on the Foot-Yangming Stomach Meridian; for malaria with thirst occurring daily, select points on the Hand-Yangming Large Intestine Meridian. For toothache without aversion to cold drinks, select points on the Foot-Yangming Stomach Meridian; for toothache with aversion to cold drinks, select points on the Hand-Yangming Large Intestine Meridian. For deafness without pain, select points on the Foot-Shaoyang Gallbladder Meridian; for deafness with pain, select points on the Hand-Yangming Large Intestine Meridian. For persistent epistaxis with profuse bleeding, select points on the Foot-Taiyang Bladder Meridian; if blood does not flow (text likely missing characters implying stasis or hemostasis), select points on the Hand-Taiyang Small Intestine Meridian. If ineffective, puncture below the Weizhong (BL40) point; if still ineffective, puncture the Xi Zhong point and release blood. For low back pain with cold sensations at the painful site, select points on the Foot-Taiyang Bladder and Foot-Yangming Stomach meridians; for heat sensations at the painful site, select points on the Foot-Jueyin Liver Meridian; for inability to bend forward or backward, select points on the Foot-Shaoyang Gallbladder Meridian. For internal heat with panting, puncture the blood collaterals at the popliteal fossa of the Foot-Shaoyin Kidney Meridian. For irritability, loss of appetite, and decreasing voice volume, puncture the Foot-Taiyin Spleen Meridian; for anger with excessive talking, puncture the Foot-Shaoyang Gallbladder Meridian. For scapular pain, puncture the Hand-Yangming Large Intestine Meridian and its full vessels to release blood. For neck pain with inability to look up or down, puncture the Foot-Taiyang Bladder Meridian; for inability to turn the head left or right, puncture the Hand-Taiyang Small Intestine Meridian. For lower abdominal distension extending upwards to the stomach and heart, with alternating chills and fever, and dysuria, select points on the Foot-Jueyin Liver Meridian. For abdominal fullness, constipation, abdominal enlargement, Qi rushing to the chest and throat, and coarse breathing, select points on the Foot-Shaoyin Kidney Meridian. For abdominal fullness, indigestion, rumbling abdomen, and inability to defecate, select points on the Foot-Taiyin Spleen Meridian. For heart pain radiating to the lower back and spine with urge to vomit, select points on the Foot-Shaoyin Kidney Meridian. For heart pain with abdominal fullness and tightness, and constipation, select points on the Foot-Taiyin Spleen Meridian. For heart pain radiating to the back with dyspnea, puncture the Foot-Shaoyin Kidney Meridian; if ineffective, select points on the Hand-Shaoyang Sanjiao Meridian. For heart pain radiating to lower abdominal fullness with wandering pain location and difficulty in urination/defecation, puncture the Foot-Jueyin Liver Meridian. For heart pain with only shortness of breath and insufficient respiration, puncture the Hand-Taiyin Lung Meridian. For heart pain located at the ninth vertebral segment, puncture there; after needling, massage the acupoint and the affected area; pain stops immediately. If not relieved, search above and below for the tender point, needle it, and pain stops immediately. For hypochondriac pain, puncture the arterial pulse at the Quzhou area of the Foot-Yangming Stomach Meridian until blood appears; pain stops immediately. If not relieved, massage the Renying (ST9) point along the meridian; pain stops immediately. For rebellious Qi moving upward, puncture depressions in the chest and arterial pulses in the lower chest. For abdominal pain, puncture the arterial pulses on the left and right of the umbilicus; after needling, massage; pain stops immediately. If not relieved, puncture the Qichuan (ST30) point; after needling, massage; pain stops immediately. For Wei-Jue syndrome (flaccidity and cold limbs), bind the extremities tightly, then quickly release them; do this twice daily. For numbness, perception returns within ten days; do not rest until recovery. For hiccups, stimulate the nasal cavity with grass to induce sneezing; hiccups stop. Alternatively, hold breath and rapidly guide Qi; hiccups stop immediately. A sudden fright can also stop hiccups. (Note: Yue refers to hiccups; the following describes common methods to stop hiccups; the original character 'Sui' is an error.)
This chapter contains numerous entries but is written in a mature, streamlined style without superfluous details, making each word solid and representative of authentic narrative prose. Many of the acupuncture methods described in these chapters belong to Miu Ci (lateral needling), which is still transmitted today.
