The Yellow Emperor asked Qibo, saying: When Zhou Bi (Generalized Bi Syndrome) occurs in the body, the pain migrates up and down following the meridians. The symptoms on the upper and lower left and right sides correspond with each other, with no gap between them. I wish to know if this pain is within the blood vessels or between the fen rou (muscle intervals). What causes this? When the pain shifts, there is not enough time to insert the needle; when the pain arises, there is not enough time to determine the treatment, yet the pain has already ceased. What principle accounts for this? I wish to hear the reason. (The above section generally raises the topics of up, down, left, and right together, distinguishing Zhou Bi from Zhong Bi, and then analyzes the manifestation of Zhong Bi according to left/right and up/down.)
Qibo replied: This is Zhong Bi (Clustered Bi Syndrome), not Zhou Bi.
The Yellow Emperor said: I wish to hear about Zhong Bi.
Qibo answered: Zhong Bi occurs at specific locations in the body. It alternates in onset and cessation, alternates in staying and rising. Symptoms on the right correspond to the left, and symptoms on the left correspond to the right. It does not circulate throughout the whole body but rather alternates in attack and rest.
The Yellow Emperor said: How should it be treated with acupuncture?
Qibo replied: To treat this condition, even if the pain has ceased, one must still needle the site where the pain occurred, so as to prevent recurrence.
The Yellow Emperor said: Excellent. I wish to know what Zhou Bi is like.
Qibo replied: Zhou Bi resides within the blood vessels, moving upward along the meridians or downward along the meridians. It cannot correspond to specific locations on the left and right sides independently like Zhong Bi.
The Yellow Emperor said: How should it be treated with acupuncture?
Qibo replied: If the pain moves from the upper part to the lower part, first needle the lower part to intercept the pathogenic factor, and subsequently needle the upper part to eliminate the disease. If the pain moves from the lower part to the upper part, first needle the upper part to intercept the pathogenic factor, and subsequently needle the lower part to eliminate the disease.
The Yellow Emperor said: Excellent. How is this pain generated? Why does it bear this name?
Qibo replied: Wind, cold, and dampness pathogens invade the exterior and lodge between the fen rou (muscle intervals). They press upon the fluids, causing them to condense into foam (mo). When this foam encounters cold, it aggregates. Aggregation exerts pressure on the fen rou, causing them to split. Splitting results in pain. When pain occurs, the spirit (shen) concentrates on the affected area. Concentration of the spirit generates heat. Heat alleviates the pain. Once the pain is alleviated, qi stagnation or reversal (jue) occurs. Qi reversal leads to the emergence of Bi syndrome in other areas, manifesting in this manner.
The Yellow Emperor said: Excellent. I have grasped its underlying principle. (Note: There may be thirteen characters missing here: "There are matters not yet understood; I wish to hear them completely." Qibo replied...)
Qibo continued: This pathology is internally not within the zang-fu organs and externally has not manifested in the skin. It resides solely between the fen rou, preventing the true qi (zhen qi) from circulating fully throughout the body. Therefore, it is named Zhou Bi. Thus, when treating Bi syndrome, one must first palpate and trace the six meridians of the lower limbs, observe their deficiency or excess conditions, and check for large collaterals where blood is knotted and obstructed, or where the pulse is sunken and empty due to deficiency. Then, regulate these conditions. Apply warming moxibustion or hot compresses (wu) to unblock the meridians. For cases involving muscle twitching, hardness, or twisting, employ traction and guidance to promote smooth flow.
The Yellow Emperor said: Excellent. I have grasped both the principle and the practical methods.
