Head sores are caused by the upward rushing of Qi due to disharmony among the Zang-Fu organs, and the upward pouring of toxic heat in the blood. Since children's Yin energy is insufficient while Yang fire is excessive, they are most prone to this condition. It is appropriate to internally administer a decoction containing Qiaofiao (Forsythiae Fructum), Jingjie (Schizonepetae Herba), Fangfeng (Saposhnikoviae Radix), Huafen (Trichosanthis Radix), Beimu (Fritillariae Bulbus), Xuanshen (Scrophulariae Radix), Chishao (Paeoniae Radix Rubra), Niubangzi (Arctii Fructus), and Shengdi (Rehmanniae Radix) to clear heat, detoxify, cool the blood, and harmonize the blood. Once the pathogenic factors have slightly subsided, external medications should be applied to the affected area. One must strictly avoid suddenly applying cold-natured topical agents to suppress the condition, as this may cause the heat-toxin to penetrate inward, leading to untreatable complications. This is because children's Zang-Fu organs are delicate, making pathogens easy to enter but difficult to expel.
For treating tinea capitis covering the entire head, as well as lesions on the hands, feet, body, genitals, and scrotum that are excoriated and oozing:
Huanglian (Coptidis Rhizoma, 5 qian), Shechuangzi (Cnidii Fructus, 2.5 qian), Wubeizi (Galla Chinensis, 1.25 qian), Qingfen (Calomelas, 25 tie). Grind into a fine powder. First, wash and wipe the area with a decoction of Jingjie (Schizonepetae Herba) and Congbai (Allii Fistulosi Bulbus), allow to dry, then mix the powder with refined sesame oil (Qingyou) and apply topically.
For treating pediatric alopecia areata (white baldness) characterized by hair loss, dryness, and pain:
Chen Xiangru (Elsholtziae Herba, 2 liang), Hufen (Lead Carbonate, 1 liang), Zhuzhi (Porcine Fat, 5 qian). Heat one large cup of water, decoct the Xiangru to extract three parts of the juice, remove the residue, mix with the porcine fat until uniform, and apply to the affected area frequently every day.
For sore throat, submandibular swelling, a surging pulse, and flushed face: Use Qianghuo Shengshi Tang (Notopterygii Rhizoma et Radix Decoction for Overcoming Dampness) with added Huangqin (Scutellariae Radix), Gancao (Glycyrrhizae Radix et Rhizoma), and Jiegeng (Platycodonis Radix), each 0.5 fen. Danxi treated bilateral cheek swelling using Xixin (Asari Radix et Rhizoma) and Caowu (Aconiti Radix Preparata) in equal parts, ground into powder, mixed with Bangfen (Concha Margaritiferae Striata), and applied with porcine fat. The patient held a white plum in the mouth near the cheek for an extended period; upon salivation and reduction of swelling, the sensation of swelling would first shift downward.
An ancient case involved a person who, after being startled, habitually scratched their cheeks with fingernails, causing skin breakdown. They experienced mental restlessness (Ao'bu), a rapid and excess pulse, and failed to respond to various medicines. Using Niuhuang Qingxin Liangge Wan (Calculus Bovis Clear Heart and Cool Diaphragm Pill) from the Huoyou Xinshu, both internal administration and external application led to recovery.
This formula is found in the Pediatric Dampness Section.
