A mass develops in the abdomen, initially the size of a chicken egg, gradually enlarging to resemble pregnancy. It feels firm upon palpation and is movable upon pushing. Menstruation continues to occur on schedule; this is known as Changtan (Intestinal Polyp/Mass).
In cases where the water pathways are obstructed, leading to swelling and fullness, the urgent treatment does not lie primarily in clearing the Lung. The fluids in the Bladder must await the descending action of Lung Qi to be transformed and distributed. Why not observe poultry and livestock? Animals with lungs urinate, while those without lungs do not; this illustrates the principle.
In cases of Gut Distension (Gu Zhang), if symptoms are relieved in the morning and worsen in the evening, it indicates Blood Deficiency. If relieved in the evening and worsened in the morning, it indicates Qi Deficiency. If there is constant urgency and distension throughout the day, it indicates both Qi and Blood Deficiency. If abdominal distension is caused by retained blood stasis, purgatives should be used to expel the dead blood. If abdominal distension is caused by food stagnation, herbs such as Muxiang (Vladimiria Root), Binglang (Areca Seed), Houpo (Magnolia Bark), and Xiangfu (Cyperus Rhizome) should be used to resolve the stagnation. If abdominal distension is caused by external cold constraining internal heat, Guizhi (Cinnamon Twig) and Mahuang (Ephedra) should be used to warm and disperse. In general, excess patterns require purgation or resolution followed by tonification; deficiency patterns require tonification or lifting, with tonification being the priority. If the pulse is deep but suddenly becomes large and floating at the surface, death ensues. Abdominal distension accompanied by alternating chills and fever resembling malaria results in death. Abdominal distension with bloody stools and a large pulse that intermittently ceases results in death. Generalized redness of the body is incurable. Abdominal distension with body heat results in death. Abdominal distension following diarrhea with visible blue veins on the abdomen results in death. Slippery diarrhea with persistent edema results in death. A solid and large pulse indicates a treatable condition; a weak and minute pulse indicates a difficult-to-treat condition.
This formula treats simultaneous deficiency of the Spleen, Lung, and Kidney, manifesting as generalized swelling, difficulty in urination, phlegm with wheezing and shortness of breath. No other medicine can save the patient.
