If fever lasts for one day and then suddenly erupts densely, resembling silkworm eggs, with a grayish-white color and dense distribution, accompanied by body heat, abdominal distension, incessant diarrhea and thirst, warmth of the head but coldness of the feet, or if the pustules are purple-black and dry/withered, the prognosis is fatal.
If spots appear immediately upon initial fever in the regions of the Taiyang (Greater Yang) and Taiyin (Lesser Yin) meridians, such as the temporal angle, hairline, Yintang (between eyebrows), Sikong, Tianting (forehead center), and Fangguang areas, with red-purple coloration, red eyes, cracked lips, phlegm rattle in the throat, and hoarseness, this indicates Qi stagnation and Blood coagulation, with pathogenic factors recklessly invading Yang positions; the prognosis is poor.
Pustules appearing on the lips indicate a poor prognosis.
Pustules appearing on the gums indicate a poor prognosis.
If upon onset the pustule tops do not rise at all, resembling water blisters or burns, this signifies exhaustion of both Qi and Blood; death will inevitably occur after nine days due to itching and collapse.
If pustules have erupted but fever recurs repeatedly after subsiding, the prognosis is poor.
If the flesh appears as a continuous patch of red-purple, the face resembles an orange peel, and there is no distinction between the pustule base and surrounding tissue, the prognosis is fatal.
Pustules appearing only below the waist, with none above the waist, indicate a poor prognosis.
If pustules are white with thin skin, smooth and moist, easily ruptured, lacking any redness at the base, and grow to the size of a vegetable bean within three to five days, these pustules will absolutely not suppurate. Later, they become sacs of clear fluid; rupture leads to certain death. Do not administer purgative medicines rashly just because they look appealing.
If upon eruption the top is depressed with a black dot in the center like a needle puncture, the prognosis is poor.
If the body's pustules are evenly distributed except for being densely clustered around the mouth, named "Locking the Mouth," one must beware of death from starvation and fever around the 90th day.
If pustules are predominantly concentrated in the "Three Granaries" area, termed "Man Chest," one must beware of critical condition around the 90th day, characterized by loss of voice, abdominal pain, and teeth grinding.
If pustules are predominantly concentrated on the shoulders and back, named "Climbing Shoulders." The Yongquan acupoint on the soles and Jianjing acupoint on the shoulders are pathways for hidden water flow. All fluids moistening the skin rely on these spring waters, originating from the Kidney. When pathogenic toxins are excessive here, the water pathways are cut off. Since all Zang organs attach to the back, excessive density on the back injures Zang Qi. One must beware of death from fever, loss of voice, irritability, and extreme thirst.
If the onset is sparse, the base is plump and moist, the color is greenish-white, accompanied by high fever and coma, it is named "Reverse Foot"; death will occur within three to five days.
If initially, a single spot appears prominently and brightly on the Tianting, Fangguang, or Taiyang areas, but sinks and disappears shortly after, this is named "Thief Spot," analogous to spies scouting before a city siege; it is a definitive fatal sign.
If lower back pain occurs during initial fever, and severe pain persists when spots emerge, with spots resembling silkworm eggs, flushed face, coarse breathing, irritability, and confusion, death will likely occur on the fifth or sixth day, preceded by foul breath, purple-black macules on the body, cyanotic/black lips, and carbuncles on the tongue.
Jaundice caused by Stomach Heat, resembling orange color, accompanied by diarrhea, indicates a poor prognosis.
If pustules first appear on both sides of the scrotum, they will later turn black and sink. If red macules are visible beneath the skin at acupoints such as Tianting, Baihui, Jiguan, and Renying when illuminated by fire, the resulting eruption will be severe.
If spots appear while fever is not yet fully resolved, then disappear, then reappear and disappear again, this is called "Playing with Spots." When toxin-heat penetrates thoroughly, skin and muscles are unobstructed, facilitating easy eruption. Here, heat does not penetrate, so the "ground" is not warm, causing hiding and re-emergence. This indicates severe depletion of Qi and Blood, lacking strength to vent pathogens; treatment will be difficult.
If both pox and rash are extremely dense, and the rash does not resolve first, this is named "Hunting Pox," indicating a poor prognosis.
If purple-red macules or several black macules appear on the body before pox eruption, accompanied by epistaxis, lethargy, body heat, and restlessness, death will occur within three to five days.
If hands, feet, and face have erupted, but body heat and irritability persist, and specifically the ear lobes and back of ears lack eruptions, this is a ominous sign. Only if eruptions are sparse, vibrant red, moist, distinct, and evenly distributed throughout the body are harmless.
If legs ache and standing becomes impossible on the third or fourth day of eruption, it is ominous.
Delirium, hallucinations of seeing ghosts, craving cold water, and macules starting from the lumbar region during eruption indicate a poor prognosis.
Green or black macules resembling moles, or patches of green-black muscle tissue, lead to immediate death.
Sensation in muscles as if beaten by a staff indicates a poor prognosis.
Initial appearance of macules, bursting/cracked or swollen lips, and foul breath indicate "Stomach Rot Macules," which is untreatable.
Tongue retraction and scrotal contraction indicate death.
Eruption of pox following an initial unnamed swelling/toxin results in death in nine out of ten cases.
Incessant vomiting and diarrhea upon initial eruption, expulsion of roundworms from mouth/nose/stool, and refusal to eat indicate death.
Whole-body purple blisters that release black blood upon puncture indicate death.
Dense, sunken, irritable, screaming pox with a fishy odor emanating from the mouth indicates pathogenic fire roasting, leading to lung rot and stomach failure. It will progress to loss of voice, dry retching, and panting, causing death in seven days. The Classic states: Lung exhaustion leads to death in seven days.
Sunken pox tops with sores in the umbilical fossa are incurable (one in a hundred survives); this is Kidney Meridian Pox, leading to death via chills and teeth grinding.
Unceasing delirium, lethargy, anorexia, and cold extremities following eruption indicate death.
Lingering onset, predominantly blue complexion without heat indicates a critical condition.
