When carbuncles and sores are severe, the initial sign is xiao ke (wasting-thirst syndrome). At the onset, some may appear like small boils, while others cause severe pain; these are subtle signs that require careful observation. To distinguish between different conditions, press firmly on the affected area; if the depression rebounds immediately upon release, it indicates excess. After carefully weighing the pros and cons, moxibustion should be performed immediately. First, apply 200-300 moxa cones directly on the lesion, followed by 100-200 cones around the periphery. For small lesions, moxa the periphery; for medium ones, six points; for large ones, eight points. A higher number of cones is not a concern. Simultaneously, apply topical medications to resolve the swelling. Internally administer Bu Nuan Tang San (Warming and Tonifying Powder); if ineffective, use cold medicines and external cold compresses. If still ineffective, use hot plasters and open the wound to vent heat.
For large, hard carbuncles without pus, or those half-hard and half-soft indicating pus formation, puncture at the thinnest area where pus is present. The method of puncture should be from the lower part upwards against the flow of pus to facilitate drainage. If the pus is deep and hard to see, use a Pai Zhen (drainage needle); if the surface is thick, use a Huo Zhen (fire needle). If there is no obvious external abscess, press repeatedly on the upper area; internal dull pain indicates pus presence. If pressure causes more pain than before, the pus inside is ripe. Once pus is drained, heat will not proliferate rapidly; otherwise, the condition will worsen.
