The general principles of the transmission and transformation of Cold Damage diseases have been detailed in *Commentary on the Treatise on Cold Damage* (*Shanghan Lun Zhu*) and *Essentials of Medical Practice* (*Xinfa Yaojue*). However, contemporary physicians treating four-season Cold Damage all employ methods such as Hejian's (Liu Wansu's) Two-Resolving method, which often yields miraculous effects; this is indeed a swift method for treating such conditions. I now compile empirical famous formulas such as Shuangjie San (Double-Resolving Powder) and Fangfeng Tongsheng San (Saposhnikovia Decoction to Unblock the Heroic), arranging them into rhymed verses, so that future students may know how to adapt flexibly. It is hoped that regarding Cold Damage, there will be adherence to both standard and variable approaches, leaving no therapeutic methods omitted.
Combining Shuangjie San (Double-Resolving Powder) with Tongsheng San (Unblocking Heroic Powder) and Liuyi San (Six-to-One Powder) addresses Zheng Shanghan (True Cold Damage) presenting with warm-heat characteristics in all seasons. The dosage is two xu (approx. 30g), decocted with scallion, ginger, and fermented soybean. This approach simultaneously induces sweating and purging, suitable for resolving both exterior and interior aspects. For those with strong constitution, double the dose; for those with weak constitution, halve it. If the condition does not resolve after initial administration, continuous intake will naturally bring peace. If insufficient sweating occurs without resolution, double the amount of Ephedrae Herba (Mahuang, 麻黄); if hard stool persists without resolution, double the amounts of Natrii Sulfas (Mangxiao, 芒硝) and Rheum Radix et Rhizoma (Dahuang, 大黄).
It is called "Shuangjie San" (Double-Resolving Powder) because it releases the exterior and attacks the interior; it is a combination of Fangfeng Tongsheng San (Saposhnikovia Decoction to Unblock the Heroic) and Liuyi San (Six-to-One Powder). Hejian formulated this to resolve winter-warmth, spring-warmth, summer-heat, and autumn-dryness affecting the four seasons. In cases of true Cold Damage where pathogenic factors remain unresolved in the exterior and interior of the Three Yang meridians, administer two xu as a dose, decocted with scallion, ginger, and fermented soybean. Wait for simultaneous sweating and purging to resolve the exterior and interior. For those with robust form and qi, use one liang and half (approx. 50g); for those with deficient form and qi, use five qian (approx. 15g). If the first dose fails to resolve due to lack of sweat, it indicates Exterior Excess; double the Ephedrae Herba (Mahuang, 麻黄) to induce sweating. If it fails to resolve due to hard stool, it indicates Interior Excess; double Natrii Sulfas (Mangxiao, 芒硝) and Rheum Radix et Rhizoma (Dahuang, 大黄) to purge. After two or three doses, resolution will inevitably come through sweating and purging. Modern people do not understand its subtleties, claiming Hejian overused cold-cooling herbs and that Zhang Zhongjing initially had no purging method for Cold Damage, leading them to discard it—a great pity. They fail to realize the method's divine swiftness, which rarely fails to achieve immediate results, and never produces complications of cold retention and glomus formation. Even if one or two cases do not resolve, it is not due to the inadequacy of the method, but because the pathogen has transmitted to the Yangming stage. Details of Fangfeng Tongsheng San (Saposhnikovia Decoction to Unblock the Heroic) are provided later.
If sweating and purging have been conducted yet the disease remains unresolved, it is due to incomplete sweating or transmission to other meridians. For internal heat with irritability and thirst, use Guiling Ganlu Yin (Cassia Twig and Atractylodes Macrocephala Drink to Transform Dampness). For severe cases, use Baihu Tang (White Tiger Decoction) combined with Huanglian Jiedu Tang (Coptis Decoction to Resolve Toxicity) to clear heat. For exterior heat with irritability, use Chaige Jieji Tang (Bupleurum and Kudzu Root Decoction to Release the Muscles). For exterior excess with high fever, use Sanhuang Shigao Tang (Three-Yellow and Gypsum Decoction) to calm. For interior heat with dark urine, use Liangge San (Cooling Diaphragm Powder) combined with Tianshui San (Heavenly Water Powder). For stomach excess with tidal fever and constipation, use Da Chaihu Tang (Major Bupleurum Decoction) or Chengqi Tangs (Chengqi Decoctions) to purge.
After taking Shuangjie San (Double-Resolving Powder), if sweating and purging have occurred but the disease persists, it is because sweating was incomplete or transmission occurred, and treatment was untimely. If the exterior is resolved but there is slight interior heat, irritability, and thirst, use Guiling Ganlu Yin (Cassia Twig and Atractylodes Macrocephala Drink to Transform Dampness) to harmonize the interior of the Taiyang meridian. If internal heat is extreme with high fever, severe irritability, and intense thirst, use Baihu Tang (White Tiger Decoction) combined with Huanglian Jiedu Tang (Coptis Decoction to Resolve Toxicity) to clear the interior heat of the Yangming meridian. If the exterior is unresolved and it has transmitted to Yangming, with body heat and irritability, use Chaige Jieji Tang (Bupleurum and Kudzu Root Decoction to Release the Muscles) to release pathogens from both Taiyang and Yangming. If there is exterior excess with no sweat, high fever, and irritability, use Sanhuang Shigao Tang (Three-Yellow and Gypsum Decoction) to clear heat from both exterior and interior. If there is interior heat with dark, difficult urination, use Liangge San (Cooling Diaphragm Powder) combined with Tianshui San (Heavenly Water Powder) to clear and harmonize. If there is stomach excess, tidal fever, and constipation with slight exterior signs, use Da Chaihu Tang (Major Bupleurum Decoction) to purge. Without exterior signs, use Chengqi Tangs (Chengqi Decoctions) to purge. Details of Guiling Ganlu Yin (Cassia Twig and Atractylodes Macrocephala Drink to Transform Dampness), Baihu Tang (White Tiger Decoction), Da Chaihu Tang (Major Bupleurum Decoction), and Chengqi Tangs (Chengqi Decoctions) are in *Shanghan Yaojue*. Details of Liuyi San (Six-to-One Powder) and Liangge San (Cooling Diaphragm Powder) are in *Zabing Yaojue*.
Fangfeng Tongsheng San (Saposhnikovia Decoction to Unblock the Heroic) treats wind-heat pathogens stagnating in the Sanjiao, affecting both exterior and interior. When qi and blood are not unimpeded and meridians are congested, the formula contains Gardeniae Fructus (Zhizi, 栀子), Forsythiae Fructus (Lianqiao, 连翘), Scutellariae Radix (Huangqin, 黄芩), Menthae Haplocalycis Herba (Bohe, 薄荷), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草), Angelicae Sinensis Radix (Danggui, 当归), Ligustici Chuanxiong Rhizoma (Chuanxiong, 川芎), Natrii Sulfas (Mangxiao, 芒硝), Rheum Radix et Rhizoma (Dahuang, 大黄), Paeoniae Radix Rubra (Chishao, 赤芍), Atractylodis Rhizoma (Baizhu, 白术), Gypsum Fibrosum (Shigao, 石膏), Talcum (Huashi, 滑石), Ephedrae Herba (Mahuang, 麻黄), Platycodonis Radix (Jiegeng, 桔梗), Saposhnikoviae Radix (Fangfeng, 防风), and Schizonepetae Spica (Jingjie, 荆芥). Reduce Natrii Sulfas (Mangxiao, 芒硝) and Rheum Radix et Rhizoma (Dahuang, 大黄) if diarrhea occurs; add Zingiberis Rhizoma Recens (Shengjiang, 生姜) and Pinelliae Rhizoma (Banxia, 半夏) if vomiting occurs. Remove Ephedrae Herba (Mahuang, 麻黄) and increase Cinnamomi Ramulus (Guizhi, 桂枝) if spontaneous sweating occurs.
This formula treats all wind-fire pathogens stagnating in the Sanjiao, exterior, interior, and meridians, preventing the free flow of qi and blood. It is suitable for initial symptoms of fever, headache, skin rashes, transmitted macules and jaundice, convulsions, irritability, thirst, insomnia, constipation, and difficult urination. Its efficacy is remarkable; users will appreciate its subtleties upon application.
Chaige Jieji Tang (Bupleurum and Kudzu Root Decoction to Release the Muscles) is indicated for concurrent disorders of the Three Yang meridians in all seasons, containing Bupleuri Radix (Chaihu, 柴胡), Puerariae Lobatae Radix (Ge Gen, 葛根), Notopterygii Rhizoma et Radix (Qianghuo, 羌活), and Zingiberis Rhizoma (Jiang, 姜). Add Angelicae Dahuricae Radix (Baizhi, 白芷), Scutellariae Radix (Kuang Qin, 枯芩), Gypsum Fibrosum (Shigao, 石膏), Paeoniae Radix (Shaoyao, 芍药), and Glycyrrhizae Radix et Rhizoma (Gancao, 甘草). Reduce Gypsum Fibrosum (Shigao, 石膏) if diarrhea occurs; add Pinelliae Rhizoma (Banxia, 半夏) and Notopterygii Rhizoma et Radix (Qianghuo, 羌活) if vomiting occurs.
This formula was created by Tao Hua to replace Gegen Tang (Kudzu Root Decoction). It is suitable for mild cases of concurrent disorders of Taiyang, Yangming, and Shaoyang meridians in all seasons. Modifications include removing Notopterygii Rhizoma et Radix (Qianghuo, 羌活) if Taiyang signs are absent, and removing Bupleuri Radix (Chaihu, 柴胡) if Shaoyang signs are absent. The ingredients are Bupleuri Radix (Chaihu, 柴胡), Puerariae Lobatae Radix (Ge Gen, 葛根), Notopterygii Rhizoma et Radix (Qianghuo, 羌活), Angelicae Dahuricae Radix (Baizhi, 白芷), Platycodonis Radix (Jiegeng, 桔梗), Paeoniae Radix Rubra (Chishao, 赤芍), Gypsum Fibrosum (Shigao, 石膏), Scutellariae Radix (Huangqin, 黄芩), and Glycyrrhizae Radix et Rhizoma (Gancao, 甘草). Reduce Gypsum Fibrosum (Shigao, 石膏) in diarrhea to avoid damaging interior deficiency; add Pinelliae Rhizoma (Banxia, 半夏) and Zingiberis Rhizoma Recens (Shengjiang, 生姜) in vomiting to descend rebellious qi.
Huanglian Jiedu Tang (Coptis Decoction to Resolve Toxicity), Zhizi Jinhua Tang (Gardenia Golden Flower Decoction), and Sanhuang Shigao Tang (Three-Yellow and Gypsum Decoction)
For yang toxicity with extreme heat, rashes, vomiting, irritability, thirst, groaning, delirium, and mania: If heat persists after purging but stool is soft, use Huanglian Jiedu Tang (Coptis Decoction to Resolve Toxicity) containing Coptidis Rhizoma (Huanglian, 黄连), Scutellariae Radix (Huangqin, 黄芩), Gardeniae Fructus (Zhizi, 栀子), and Phellodendri Cortex (Huangbai, 黄柏). If interior excess requires purging and stool is hard, add Rheum Radix et Rhizoma (Dahuang, 大黄) to Zhizi Jinhua Tang (Gardenia Golden Flower Decoction). If exterior excess requires sweating with no sweat, add Gypsum Fibrosum (Shigao, 石膏), Ephedrae Herba (Mahuang, 麻黄), Fermented Soybean (Danshi, 淡豉), and Allii Macrostemonis Bulbus (Congbai, 葱白) to form Sanhuang Shigao Tang (Three-Yellow and Gypsum Decoction). If diarrhea occurs, reduce Gypsum Fibrosum (Shigao, 石膏) and add Puerariae Lobatae Radix (Gegen, 葛根).
Qingdu Xijiao Yin (Clearing Toxin and Rhinohorn Decoction)
For conditions like yang toxicity with extreme heat, or persistent high fever after purging with soft stool, Qinglian Jiedu Tang (Coptis Decoction to Resolve Toxicity) is appropriate, consisting of Coptidis Rhizoma (Huanglian, 黄连), Scutellariae Radix (Huangqin, 黄芩), Phellodendri Cortex (Huangbai, 黄柏), and Gardeniae Fructus (Zhizi, 栀子). If interior excess requires purging and stool is hard, add Rheum Radix et Rhizoma (Dahuang, 大黄), named Zhizi Jinhua Tang (Gardenia Golden Flower Decoction). If diarrhea occurs, reduce Gypsum Fibrosum (Shigao, 石膏) and add Puerariae Lobatae Radix (Gegen, 葛根) to avoid damaging interior deficiency.
Xiaodu Xijiao Yin (Disinfecting Rhinohorn Decoction) treats exterior rashes; toxin congestion causes difficult throat swelling and pain. Ingredients include Rhinohorn (Xijiao, 犀角), Arctii Fructus (Niubangzi, 牛蒡子), Schizonepetae Spica (Jingjie, 荆芥), Saposhnikoviae Radix (Fangfeng, 防风), and Glycyrrhizae Radix et Rhizoma (Gancao, 甘草). For severe heat, add Forsythiae Fructus (Lianqiao, 连翘), Menthae Haplocalycis Herba (Bohe, 薄荷), Scutellariae Radix (Huangqin, 黄芩), and Coptidis Rhizoma (Huanglian, 黄连).
