Stool and urine are normal, the Shaoyin pulse is faint; treatment should tonify deficiency and descend fire. For blood deficiency, Siwu Tang (Four Substances Decoction) is used, added with Jiegeng (Platycodonis Radix), Jingjie (Schizonepetae Herba), Zhimu (Anemarrhenae Rhizoma), and Huangbai (Phellodendri Cortex); for qi deficiency, Sijunzi Tang (Four Gentlemen Decoction) is used, added with Gancao (Glycyrrhizae Radix et Rhizoma), Jiegeng (Platycodonis Radix), Xuanshen (Scrophulariae Radix), and Shengma (Cimicifugae Rhizoma). In severe cases, Ganjiang (Zingiberis Rhizoma) and Fuzi (Aconiti Lateralis Radix Praeparata) are added as guiding agents, administered slowly. If phlegm is excessive, Erchen Tang (Two-Cured Decoction) ingredients are used, adding one green fish gallbladder. The gallbladder is first stuffed with glutinous rice, dried in the shade, then powdered, and taken mixed with ginger juice. Alternatively, emesis can be induced to expel phlegm, or Qianmin Tang (Thousand-Mill Decoction) may be used. For those who have previously taken cooling medicines causing diarrhea, or whose voice has been damaged, Michuan Jiangqi Tang (Secret Transmission Descending Qi Decoction) is used to rescue them. For sudden contraction of wind-cold causing throat constriction and obstruction, Zigeng Banxia Tang (Purple Stem Pinellia Decoction) and Zhufu Tang (Pig Skin Decoction) are used; for Kidney Cold Damage and Yin syndrome, Bangui Tang (Half-Cinnamon Decoction) and Honey-processed Fuzi (Honey-fried Aconite) are used. Commonly, Ganjie Tang (Licorice and Platycodon Decoction), Lige Tang (Diaphragm-Relieving Decoction), Bingmei Wan (Ice Plum Pill), Xijiao Hupo Gao (Rhinoceros Horn and Amber Paste), or single Baicaoshuang (Ligninum Carbonisatum) powdered and honey-pilled into pill size of a bullet (each dose: three pills dissolved in fresh drawn water) are used. Generally, pure cooling medicines should not be used for throat diseases. Herbal remedies may show immediate effect, but upper heat remains while middle cold arises; toxic vapors enter the abdomen through deficiency, causing high swelling on the chest, upper asthma and lower diarrhea, cyanosis of fingernails and toenails. If after seven days the patient cannot eat at all and the mouth opens like a fish's mouth, death ensues.
Inducing vomiting to draw out phlegm is a truly swift method.
Jiao method: Apply green fish gallbladder powder three times; redness and swelling dissipate immediately. Vomiting method: Use winter green fish gallbladder, insert Boren (Alum) inside. At time of use, add a little roasted salt mixed with Baicaoshuang (Ligninum Carbonisatum) and vinegar. Dip duck feather in the medicine to induce vomiting until phlegm is exhausted. If no fish gallbladder is available, use half a jin (approx. 500g) of Baifen (White Alum) and ten seeds of Badou (Crotonis Fructus), stir-fry together with Boren (Alum), remove Badou (Crotonis Fructus), and use this to induce vomiting of phlegm with divine efficacy. After vomiting, blow Jin Suoshi (Golden Lock Key) powder into the throat. Regularly taking Ganjie Tang (Licorice and Platycodon Decoction) is best. If the jaw is tightly closed, use the following opening drugs, or Erxian San (Two Immortals Powder). For unconsciousness, use Yizi San (One-Character Powder).
In acute emergencies, a single needle insertion brings relief;
"Fire depression should be vented" refers to sweating, but throat sores contraindicate sweating. The method that harms patients least is pricking with bi needles to bleed, which embodies the principle of sweating; if much blood is released, the disease improves. For needle sores, mix ginger juice with cooked water and sip occasionally. For those afraid of needles, prick gently and tactfully. Blood blisters at the Guan area (upper throat) are most suitable for pricking. If none are visible below the Guan area, have the patient hold a mouthful of water, sharpen a reed tube, insert it into the nostril, and prick to bleed—this is excellent. However, Kidney Cold Damage and internal throat swelling contraindicate needling. Instead, burn Shechuangzi (Cnidii Fructus) in a bottle, let the patient inhale the smoke into the throat for immediate cure.
Toxic accumulation blocking the airway can still be rescued by opening the gate.
Xionghuang Jiedu Wan (Realgar Detoxification Pill), Longpo Podu San (Borneol Breaking Toxin Powder), Yuyaochi ( Jade Key), or press oil from Badou (Crotonis Fructus) onto paper. Roll the oiled paper into a strip, light it, blow it out, and smoke the fumes into the nose. Saliva will flow from mouth and nose within an instant, and the jaw will open naturally. Another prescription: Wrap Badou (Crotonis Fructus) meat in cotton, secure it, and insert it into either the left or right nostril. If both sides are affected, block both nostrils; patency is achieved immediately. This is because Badou (Crotonis Fructus) treats "running horse throat obstruction" (acute epiglottitis/throat spasm) by using heat to attack heat, relying on the principle that heat promotes circulation.
Loss of voice due to throat disease is difficult to treat.
For throat obstruction with loss of voice, Michuan Jiangqi Tang (Secret Transmission Descending Qi Decoction) is used, removing Chenpi (Citri Reticulatae Pericarpium) and adding Huangqin (Scutellariae Radix). For wind-cold loss of voice, Ganjie Tang (Licorice and Platycodon Decoction) is used, adding Hezi (Terminalia Chebulae Fructus) and Mutong (Akebiae Caulis), mixed with Shengdi juice (Rehmanniae Radix Juice) to moisten, or Hesanzi (Terminalia Chebulae Powder). For blood deficiency with heat, coughing and hoarseness, Qingdai (Indigo Naturalis) and Hafan (Tridacnidae Concha) are mixed with honey to dissolve in the mouth, or Runfei Wan (Moisten Lung Pill) and Mizhi Jian (Honey Fat Decoction) are used. For ordinary unclear voice, Jiawei Guben Wan (Modified Root-Securing Pill) is used. For internal injury and deficiency with throat sores and loss of voice, there is no treatment. For bone splinter impaction, Xiaosha (Natrii Sulfas) is powdered and mixed into Longnao Jisu Wan (Borneol Mint Pill), formed into bullet-sized pills, held in the mouth to dissolve; three to five pills will naturally digest the splinter. For fish bone impaction, eat olives or take olive pit powder mixed with flowing water; externally, use otter claws to scratch/pull it down. Details are found in Volume Seven, Emergency Prescriptions.
