Diseases that are destined to recover will show a yellowish hue in the eyes, (indicating the movement of Stomach Qi.) However, if the eyelids suddenly sink and remain fixed, death is certain. (This signifies the exhaustion of the Five Zang organs.)
If blackness appears around the ears, eyes, mouth, and nose, once the pathogen enters, seven out of ten cases are difficult to save. (This is due to Kidney Water encroaching upon Stomach Earth.)
A yellow complexion with greenish eyes, accompanied by frequent indulgence in alcohol and sexual excess, indicates evil wind residing in the Stomach, causing the body to deteriorate. (This represents Wood overcoming Earth.)
A black complexion with white eyes signifies the failure of the Gate of Vitality (Mingmen). In extreme exhaustion, death approaches within eight days. (Seeing green first and then black corresponds to the concept in *Su Wen* that "if there is no return, there is no turning back"; when Shen departs, death occurs.)
If the complexion suddenly appears blue/green from a distance, but looks black up close, it arises abruptly and is difficult to handle. (This indicates the exhaustion of Liver and Kidney essence.)
A red complexion with white eyes, accompanied by fear of panting, determines survival or death after ten days have passed. (This represents Fire overcoming Metal.)
Yellow, black, and white hues appearing in the eyes, combined with ominous signs around the mouth and nose, indicate Water encroaching upon Spleen Earth.
A blue/green complexion with yellowish eyes, manifesting at Wu hour (11:00-13:00), requires observation for two additional days to assess the strength of the remaining condition. (This represents Wood overcoming Earth.)
Eyes lacking luster and blackened gums indicate the exhaustion of Heart and Liver. A pale complexion with blackened eyes also portends disaster. (This indicates the exhaustion of Lung and Kidney.)
A mouth resembling a fish's mouth that cannot close indicates Spleen exhaustion. Breath that exits but does not return signifies the flying away of life force, indicating prior exhaustion of Liver and Kidney.
Shoulder breathing (shallow respiration using accessory muscles), fixed staring gaze, cracked lips, facial edema with a dark/blackish color, delirious speech, confusion, muteness, corpse-like odor, and unconsciousness all suggest poor longevity. (These are signs of Heart exhaustion.)
When the philtrum is completely filled and the lips are cyanotic, death is inevitable within three days. (Wood overcoming Earth.)
Chronic redness on the malar cheeks indicates long-standing Heart disease; an open mouth with straight, uncontrolled breath indicates difficulty in sustaining life. (Exhaustion of Spleen and Lung.)
Swelling of the dorsum of the foot and toes, with knees swollen like a bowl, indicates that preservation of health is difficult within ten days. (Spleen exhaustion.)
Stiffness and extension of the neck tendons signify certain death. (Exhaustion of the Du Mai/Governing Vessel.) The absence of lines on the palms indicates a short lifespan. (Exhaustion of the Pericardium.)
Cyanotic lips, cold body, and urinary incontinence indicate Bladder exhaustion. Inability to eat or drink for four days indicates Liver exhaustion.
Cyanotic and black fingernails and toenails indicate that even after eight days, recovery is unlikely. (Exhaustion of Liver and Kidney.)
Pain in the spine and heaviness in the waist, making turning over difficult, indicates Bone exhaustion; prognosis can be seen within five days. Heaviness of the body, red urine that does not cease, indicates Muscle exhaustion; verdict can be reached within six days. Cyanotic nails with excessive shouting and cursing indicate Tendon exhaustion; survival beyond nine days is unlikely. Hair standing straight like hemp leads to death within half a day. (Small Intestine exhaustion.) Grasping at clothes and speaking before death suggests only one in ten might survive. (Heart exhaustion.)
