陈曰∶胃主纳,饮食不下,胃有病也。脾主化,饮食不消,脾有病也。
Dietary irregularity and overexertion constitute internal injury (Internal Injury), or result from hunger, excessive fullness, and sexual excess. Wind-cold pathogenic factors invading the human body are termed external contraction (External Contraction); once clearly differentiated and treated appropriately, no harm will ensue. A tight pulse at the Renying point (Left Cun position) accompanied by heat on the dorsal hand, with alternating chills and fever without interruption, indicates an external pathogen. Chills, absence of sweating, and nasal congestion distinguish this as an external contraction syndrome. In contrast, internal injury presents with a surging pulse at the Qikou point (Right Cun position), heat in the palms, slight aversion to wind, intermittent chills and fever, loss of taste, weak Qi, and lethargic speech. Chills due to internal injury resolve with warmth, whereas chills from external contraction persist even near fire. When external and internal conditions coexist, both pulse and symptoms must be carefully differentiated. Internal injury is a deficiency pattern requiring urgent tonification; external contraction is an excess pattern with different treatment principles. One may either tonify first or disperse first; the sequence must not be chosen arbitrarily. Yi Qi Tang (Qi-Supplementing Decoction) taken with fresh ginger and jujube harmonizes Qi and disperses mild cold. To rescue Kidney Yin deficiency, use Phellodendri Cortex (Huangbai) stir-fried with wine; to nourish Heart Blood, add Carthami Flos (Honghua). If spontaneous sweating occurs, remove Cimicifugae Rhizoma (Shengma) and Bupleuri Radix (Chaihu); for insomnia at night, add Ziziphi Spinosomi Semen (Suanzaoren) stir-fried with ginger. For headache, use Ligustici Chuanxiong Rhizoma (Chuanxiong) and Viticis Fructus (Manjingzi); for thirst, Puerariae Radix (Gegen) is efficacious. For vertex headache, use Libanodanis Resina (Xben). For palpitations and fright, use Ziziphi Spinosomi Semen (Zaoren) and Poria (Fushen) (for severe cases, add Polygalae Radix (Yuanzhi), Platycladi Semen (Bairen), and Acori Tatarinowii Rhizoma (Changpu)). For food stagnation, add Malto (Maiqu), Crataegi Fructus (Shanzha), and Ziziphi Spinosomi Semen (Shishi - *Note: Text says 'Shishi' likely referring to specific digestion aid or typo for Shenqu/Maiya context, but literal translation used*); for diarrhea, add Alismatis Rhizoma (Zexie), Paeoniae Radix Alba (Baishao), and Poria (Yunling). For chest stuffiness, use Coptidis Rhizoma (Chuanlian) and Immature Bitter Orange (Zhishi); for phlegm, Pinelliae Rhizoma (Banxia) and Poria (Fufu) serve as chief herbs. For weakness in the legs, use Stephaniae Tetrandrae Rhizoma (Fangji) and Saposhnikoviae Radix (Mugua); for seminal emission, use Longgu and Muli. For body heat, use Notopterygii Rhizoma et Radix (Qianghuo), Saposhnikoviae Radix (Fangfeng), Ligustici Chuanxiong Rhizoma (Xiong), and Angelicae Dahuricae Radix (Zhi) (add if wind-cold headache is present); for rising fire, use Anemarrhenae Rhizoma (Zhimu), Phellodendri Cortex (Baiyuan), and Scrophulariae Radix (Yuanshen). Coptidis Rhizoma (Lian) and Scutellariae Radix (Qin) clear internal heat; Chrysanthemi Flos (Juhua) and Rehmanniae Radix Preparata (Shudi) treat eye pain.
The Spleen and Stomach are the foundation of acquired constitution (Acquired Foundation), receiving and transforming food to nourish Ying and Wei Qi. The secret of health lies in dietary moderation; damage to the Spleen and Stomach leads to physical weakness. Symptoms include chest and abdominal fullness, acid regurgitation, belching, aversion to food, and continuous abdominal pain. Severe cases may involve fever and headache, but unlike Taiyang disease (Shanghan), the body does not ache. A normal pulse at the Left Guan position and a tight pulse at the Right Guan position indicate that Xiang Sha Ping Wei San (Fragrant Sand Regulate the Stomach Powder) is effective. Use Ligustici Chuanxiong Rhizoma (Chuanxiong), Immature Bitter Orange (Zhishi), and Pogostemonis Herba (Huoxiang), decocted with water and fresh ginger; dietary restrictions apply during treatment. For digesting meat, use Crataegi Fructus (Shanzha); for fruits, Radish (Luobo); for flour products, Raphani Semen (Laifu); for sticky rice, Arecae Semen (Binglang) and Massae Fermentatae Medicamentosa (Shenqu). For rice stagnation, use Shenqu and Malto (Maiya); for raw/cold foods, use Fresh Ginger (Shengjiang), Citri Reticulatae Pericarpium (Qingpi), Trichosanthis Radix (Gualu), and Tsaoko Fructus (Guoguo). For fish injury, use Olive (Ganlan), Zanthoxyli Fructus (Jiaozhu), and Perillae Folium (Zisu); straw can help digest beef. Musk (Shexiang) can eliminate vegetable and fruit stagnation; grapes, plums, and Amomi Fructus (Baokou) resolve alcohol injury. If complicated by phlegm, use Pinelliae Rhizoma (Banxia) and Fresh Ginger (Shengjiang); if complicated by Qi stagnation, Cyperi Rhizoma (Xiangfu), Amomi Fructus (Sharen), and Aurantii Fructus Immaturus (Zhike) are beneficial. If complicated by cold, use Perillae Caulis (Sugeng), Puerariae Radix (Gegen), and Bupleuri Radix (Chai); after eating cold food, consume Tsaoko Fructus (Caokou), Cinnamomi Ramulus (Guizhi), and Magnoliae Officinalis Cortex (Houpo). For fluid retention (Shang Yin), combine with Si Ling Tang (Four-Poria Decoction); add fresh ginger juice when administering for vomiting. Poria (Fuling), Alismatis Rhizoma (Zexie), and Atractylodis Macrocephalae Rhizoma (Baizhu) treat Spleen deficiency; for diarrhea, use Myristicae Semen (Roukou), Plantaginis Semen (Cheqianzi), and Atractylodis Macrocephalae Rhizoma (Baizhu). Food stagnation lingering long transforms into damp-heat; Scutellariae Radix (Qin), Coptidis Rhizoma (Lian), and Rheum Palmatum Radix et Rhizoma (Dahuang) are indispensable. Add Baizhu and Zexie, while removing Huoxiang, Sharen, and Atractylodis Rhizoma (Cangzhu). Danxi stated that initial exposure to dietary cold requires warm dispersion and warm purgation. Over time, it becomes stagnant heat; prolonged heat generates fire, making warm methods contraindicated. One should use acrid-cool releasing methods; acrid-cold regulation of the Middle Jiao easily dispels pathogens. Mild cases require reducing food intake; severe cases require eliminating stagnation. Dongyuan's profound theories should never be neglected.
Mr. Chen said: The Stomach governs reception; if food cannot go down, the Stomach is diseased. The Spleen governs transformation; if food is not digested, the Spleen is diseased.
