The Classics state: "When a disease originates from Yang and purgation is wrongly applied, heat enters inward, resulting in Jie Xiong (chest binding); when a disease originates from Yin and purgation is wrongly applied, Pi Qi (epigastric fullness) results." Yang refers to the Three Yang meridians, and Yin refers to the Three Yin meridians. When pathogenic cold factors are in the Three Yang meridians, purgation is contraindicated; applying purgation leads to the severe condition of Jie Xiong. Even if pathogens have entered the Three Yin meridians but have not yet condensed into excess patterns, clearing and resolving methods should still be preferred. If purgation is applied too early, Pi Qi may still result. Jie Xiong is a heavier syndrome, while Pi Qi is lighter. Generally speaking, hardness, fullness, and pain extending from the chest to the abdomen, which cannot be touched by hand, is called Jie Xiong; epigastric fullness and discomfort without severe pain is called Pi Qi. For Jie Xiong syndrome, Xiaoxin Xiongtang (Scutellaria and Trichosanthes Decoction) is used first; if the condition is solid and difficult to resolve, Daxin Xiongtang (Major Scrofula-Resolving Decoction) is used for attack. For Pi Qi syndrome, Banxia Xiexintang (Pinellia Heart-Mending Decoction) is the primary formula. There is also Shui Jie Xiong (Water-binding Chest) syndrome, caused by fluid retention, treated with Xiaobanxia Jia Fulingtang (Pinellia with Poria Decoction). Furthermore, Han Shi Jie Xiong (Cold-excess Binding Chest) syndrome is caused by condensation of cold qi, not resulting from erroneous purgation, and requires Baisan (White Powder) as the treatment. In all cases of Jie Xiong, Pi Qi, etc., if medication fails, it is due to turbid qi condensing and not dispersing; *Huo Ren* recommends Zhishi Lizhong Wan (Atractylodes Macrocephala and Immature Bitter Orange Pill to Regulate the Middle), which yields immediate recovery.
Originally a Taiyang Disease (Taiyang Disease), wrongly purged by the physician, resulting in hardness, fullness, and pain from the epigastrium down to the lower abdomen, which cannot be touched by hand, this is Jie Xiong, and Daxin Xiongtang (Major Scrofula-Resolving Decoction) should be taken. If there is no pain upon pressing, it is Xiaojie Xiong (Minor Scrofula-Resolving Syndrome), and Ben Fang (this formula, referring to Xiaoxin Xiongtang) should be taken. However, the principle of medication should proceed from mild to severe, akin to a probing method. The upper three ingredients (Trichosanthis Fructus, Coptidis Rhizoma, Pinelliae Tuber): use six zhong of water, first decoct Trichosanthis Fructus until the volume is halved, remove the dregs, add the other medicines, decoct to obtain two zhong, and take warm.
According to this formula, adding Zhi Shi (Immature Bitter Orange) 1.5 qian yields better effects.
