Banzhen (macules and papules) refers to a contagious warm epidemic disease occurring at that time. Macules originate from the neck, gradually spreading to the whole body; the skin on the limbs becomes smooth and forms patches, red like cinnabar. Upon close inspection, they appear as dense dots, with white sand spots appearing on the macules. If the head and face are spared, the toxic fire is particularly severe. Papules originate from the head and face, presenting as raised red points with granular, sharp tips. At the onset of Banzhen, there is high fever in the body. After two or three days, red spots appear all over the body: red indicates mild condition, purple indicates severe condition, and black indicates incurable. The symptoms include coma, delirium, irritability, fullness, and madness. If not treated promptly with acrid-cool herbs assisted by salty-cold agents, death may occur within a day. For severe warm macules with purple coloration and unclear mental state due to excessive toxic fire, Jiawei Qingdu Huaban Tang (Modified Clear Toxin and Resolve Macule Decoction) is the primary formula. If high fever subsides but true yin is severely damaged, Yangyin Fuye Tang (Nourish Yin and Restore Fluids Decoction) is used. For initial warm papules, Fangfeng Songji Baidu Tang (Saposhnikovia Relaxing Muscle and Resolving Toxin Decoction) is indicated. For cases requiring warm dispersal, Jingfang Touzhen Tang (Schizonepeta and Saposhnikovia Expelling Papule Decoction) should be used. If after the eruption of macules and papules, the cheeks and neck swell and hurt, indicating residual toxin not yet cleared, Xiaodu San (Detoxification Powder) is prescribed. In cases where warm disease causes sudden widespread eruption of red and purple patches, nasal flaring, rapid breathing, high fever with desire for coolness, and delirious speech, indicating excessively heavy toxic fire, Qingying Jiedu Tang (Clear Nutrient Level and Detoxify Decoction) is used. These are specific formulas for treating warm macules and warm papules; for cold macules and cold papules, other formulas should be selected.
Jiawei Qingdu Huaban Tang (Modified Clear Toxin and Resolve Macule Decoction):
Xijiao (Rhinoceros Horn, 3 qian, ground into fine powder and taken with decoction) Bohe (Menthae Heterophyllae Herba, 2 qian) Shigao (Gypsum Fibrosum, 4 qian, raw) Zhimu (Anemarrhenae Rhizoma, 3 qian) Daqingye (Isatidis Folium, 3 qian) Gancao (Glycyrrhizae Radix et Rhizoma, 2 qian, raw) Shengdi (Rehmanniae Radix, 3 qian) Danpi (Moutan Cortex, 3 qian) Jinyinhua (Lonicerae Japonicae Flos, 3 qian) Lianqiao (Forsythiae Fructum, 3 qian) Gengmi (Oryzae Semen, 3 qian). Decoct in water to obtain slightly more than half a cup, take warm. Children take half dose.
Yangyin Fuye Tang (Nourish Yin and Restore Fluids Decoction):
Shengdi (Rehmanniae Radix, 5 qian) Yuan Shen (Scrophulariae Radix, 3 qian) Maidong (Ophiopogonis Tuber, 3 qian) Gancao (Glycyrrhizae Radix et Rhizoma, 1 qian, raw) Beimu (Fritillariae Bulbus, 2 qian) Shashen (Glehniae Radix, 3 qian) Baishao (Paeoniae Radix Alba, 2 qian). Decoct in water to obtain slightly more than half a cup, take warm.
Fangfeng Songji Baidu Tang (Saposhnikovia Relaxing Muscle and Resolving Toxin Decoction):
Fangfeng (Saposhnikoviae Radix, 3 qian) Bohe (Menthae Heterophyllae Herba, 2 qian) Chantui (Cicadae Periostracum, 2 qian, heads and feet removed) Xingren (Armeniacae Semen Amarum, 3 qian) Baishao (Paeoniae Radix Alba, 3 qian) Danpi (Moutan Cortex, 3 qian) Lianqiao (Forsythiae Fructum, 2 qian) Jiegeng (Platycodonis Radix, 2 qian) Gancao (Glycyrrhizae Radix et Rhizoma, 1 qian, raw). Add Xian Lugen (Fresh Phragmitis Rhizoma, 1 liang). Decoct in water to obtain slightly more than half a cup, take warm. If body heat gradually subsides and papule toxin recedes, remove Fangfeng, Bohe, Chantui, and Xingren, add Shengdi 3 qian. Take 3-4 doses; once papules completely resolve, the body will naturally become cool and peaceful. Formulas for the Six Meridians of Plague can also be used.
Jingfang Touzhen Tang (Schizonepeta and Saposhnikovia Expelling Papule Decoction):
Jiesui (Schizonepetae Spica, 3 qian) Fangfeng (Saposhnikoviae Radix, 3 qian) Danggui (Angelicae Sinensis Radix, 3 qian) Baishao (Paeoniae Radix Alba, 3 qian) Chuanxiong (Chuanxiong Rhizoma, 3 qian) Xingren (Armeniacae Semen Amarum, 3 qian) Gancao (Glycyrrhizae Radix et Rhizoma, 2 qian). Indicated for cases requiring warm dispersal.
Xiaodu San (Detoxification Powder):
Bohe (Menthae Heterophyllae Herba, 1 qian) Baizhi (Angelicae Dahuricae Radix, 1 qian) Jiegeng (Platycodonis Radix, 2 qian) Gancao (Glycyrrhizae Radix et Rhizoma, 1 qian, raw) Tianhuafen (Trichosanthis Radix, 3 qian) Lianqiao (Forsythiae Fructum, 2 qian) Jiangcan (Bombyx Batryticatus, 2 qian) Beimu (Fritillariae Bulbus, 3 qian, crushed) Jinyinhua (Lonicerae Japonicae Flos, 3 qian). Add Zhuye (Phyllostachidis Folium, 15 leaves). Decoct in water to obtain slightly more than half a cup, take warm. Complete resolution occurs after several doses.
Qingying Jiedu Tang (Clear Nutrient Level and Detoxify Decoction):
Lingyangjiao (Saiga Tataricae Cornu, 3 qian) Shengdi (Rehmanniae Radix, 5 qian) Dong Sangye (Mori Folium, 3 qian) Bohe (Menthae Heterophyllae Herba, 2 qian) Danpi (Moutan Cortex, 3 qian) Baishao (Paeoniae Radix Alba, 3 qian) Jiegeng (Platycodonis Radix, 2 qian) Lianqiao (Forsythiae Fructum, 3 qian) Jinyinhua (Lonicerae Japonicae Flos, 3 qian) Yuan Shen (Scrophulariae Radix, 3 qian) Zhuye (Phyllostachidis Folium, 1 qian) Fangfeng (Saposhnikoviae Radix, 3 qian). Decoct in water to obtain slightly more than half a cup, take warm. If high fever persists and mental agitation with screaming continues after taking this, add Xijiao powder 1 qian and mix in Jinzhi (Gold Juice, 3-4 liang), drinking it as tea substitute. Once high fever subsides and mental state clears, and papules gradually recede, remove Xijiao, Lingyangjiao, and Bohe, add Maidong. Several doses will lead to recovery.
