When the pathogenic factor of Summerheat invades the human body, it often takes advantage of deficiency to enter. During the peak of summer heat, people often have heat stagnating internally, and their Yuan Qi (Original Qi) is damaged first. If they are occasionally constrained by external pathogens, causing internal heat to fail in being dispersed, Qi will be further exhausted and Body Fluids further injured, resulting in the onset of Heat disease. The manifestations include fever and aversion to cold, thirst and irritability, red face and dry teeth, dark urine, and a flooding yet empty pulse. This belongs entirely to a syndrome of Heat in both the exterior and interior.
If the tongue coating is white, there is thirst but no sweating, indicating the pathogen is in the Qi Level with an Excess pattern at the exterior, Fangfeng Qiao Sang Xing Tang (Wind-Proofing Forsythia Mulberry Leaf Apricot Kernel Decoction) should be used as the primary formula. If the tongue body is red, there is thirst but no sweating, indicating the pathogen is in the Blood Level with an Excess pattern at the exterior, Fangfeng Qiao Sang Xing Tang should be modified by removing Jin Yin Hua (Lonicerae Flos), Xing Ren (Armeniacae Semen Amarum), Hua Shi (Talcum), and Dou Chi (Sojae Semen Praeparatum), and adding Sheng Di Huang (Rehmanniae Radix), Dan Pi (Moutan Cortex), and Bai Shao (Paeoniae Radix Alba). If there is profuse sweating and extreme thirst, with a large, empty, and knotted (Kou) pulse, indicating Heat in the Qi Level with a Deficiency pattern at the exterior, Renshen Bai Hu Tang (Ginseng White Tiger Decoction) should be used. If the tongue body is red, there is thirst and profuse sweating, indicating Heat in the Blood Level with a Deficiency pattern at the exterior, Jiajian Sheng Mai San (Modified Pulse-Nourishing Powder) should be used.
If the pathogen enters the Yangming channel, presenting with a flooding and slippery pulse, red face, body heat, dizziness, aversion to heat but not aversion to cold, yellow and slippery tongue coating, desire for water, vomiting after drinking water, pain upon pressing the hypochondrium/epigastrium, short urine, and constipation, this indicates Water accumulation in the chest. Xiao Xian Xiong Tang (Minor Inverted Chest Decoction) with Zhi Shi (Aurantii Fructus Immaturus) added should be used. If the pathogen enters the Yangming channel, presenting with a slippery and rapid pulse, loss of appetite, lack of hunger, inability to defecate, congealed turbid phlegm, and epigastric fullness/stuffiness, Banxia Xi Xin Tang (Pinellia Heart-Draining Decoction) should be used, modifying by removing Renshen (Ginseng Radix), Gan Jiang (Zingiberis Rhizoma), Da Zao (Jujubae Fructus), and Gan Cao (Glycyrrhizae Radix et Rhizoma), and adding Zhi Shi (Aurantii Fructus Immaturus) and Xing Ren (Armeniacae Semen Amarum). If the pathogen enters the Yangming channel, dampness has transformed, leaving only heat accumulation, presenting with dry mouth and throat, desire for water, redness of the face and eyes, dry and yellow tongue coating, and deep and solid pulse, Xiao Cheng Qi Tang (Minor Order the Qi Decoction) should be used with equal parts of each herb to purge the heat. This is a specific method for treating Summerheat-Heat.
If latent Summerheat combined with food stagnation leads to vomiting and diarrhea, it still belongs to a Cold syndrome and should be treated according to the methods for Huoluan (Cholera-like disorder). If in the summer month one contracts Cold damage (Shanghan), but there is no internal heat, it should still be treated according to the methods for Shanghan.
