Abdominal pain caused by smallpox is mostly due to the toxin of smallpox; clinical judgment should be exercised based on the specific presentation.
[Dan] The child of Wu Dianzi developed abdominal pain during the eruption of smallpox (Doudou).
Prescription: Dingxiang (Clove, two pieces), Guigui (Cinnamon Twig), Shaoyao (White Peony Root) (each 1 qian), Baizhu (Atractylodes Macrocephala Rhizome), Danggui (Angelica Sinensis Radix) (each 5 fen).
These ingredients constitute one dose for decoction and administration. Generally, for abdominal pain or body pain occurring at the initial stage of smallpox eruption, accompanied by a洪数 (Hongshu - surging and rapid) pulse, the method of releasing the exterior (Jiebiao) is used. Adding Shaoyao and Gancao (Licorice Root) to cooling herbs will gradually lead to improvement.
[Shi] After the eruption of sores and smallpox, if there is panting with urgency and profuse phlegm, Xinggan Tang (Apricot and Licorice Decoction) or Renshen Qingge San (Ginseng Clearing Diaphragm Powder) are appropriate.
Xinggan Tang treats irritability, panting, thirst, and restlessness caused by sores and smallpox.
Ingredients: Mahuang (Ephedrae Herba), Sangbaipi (Morus Alba Bark), Xingren (Prunus Armeniaca Seed), Gancao (Licorice Root).
The above ingredients are divided equally, ground into a coarse powder. Each dose is 3 qian, decocted in one cup of water, and taken after meals.
[Hai] For rashes causing dysfunction of Lung Qi, Zicao Gancao Zhike Tang (Arnebiae Radix, Glycyrrhizae Radix, and Aurantii Fructus Decoction) is used.
[Chen] Renshen Qingge San (Ginseng Clearing Diaphragm Powder):
Renshen (Ginseng Radix et Rhizoma), Sangbaipi (Morus Alba Bark), Chaihu (Bupleuri Radix), Danggui (Angelicae Sinensis Radix), Shaoyao (Paeoniae Radix), Zhimu (Anemarrhenae Rhizoma), Fuling (Poria) (each 0.5 liang); Baizhu (Atractylodis Macrocephalae Rhizoma), Huangqi (Astragali Radix), Zicao (Arnebiae Radix), Digupi (Lycii Cortex), Huashi (Talcum) (each 1.5 liang); Gancao (Glycyrrhizae Radix et Rhizoma), Jiegeng (Platycodonis Radix) (each 1 liang); Huangqin (Scutellariae Radix) (0.5 liang); Shigao (Gypsum Fibrosum) (1 liang).
The above ingredients are chopped. Each dose is 3 qian, decocted in water, and taken warm.
If there is phlegm-accumulation with high fever, chest oppression and stuffiness, and panting when lying down, Qianhu Zhike Tang (Peucedani Radix and Aurantii Fructus Decoction) is the primary treatment.
Qianhu Zhike Tang ingredients: Qianhu (Peucedani Radix), Zhike (Aurantii Fructus), Chi Fuling (Red Poria), Zhigancao (Prepared Licorice Root) (each 5 qian), Dahuang (Rhei Radix et Rhizoma) (dosage adjusted according to the child's condition).
The above ingredients are chopped. Each dose is 3 qian, decocted in one cup of water until reduced to 0.6 cups, and administered immediately.
If there is panting accompanied by abdominal fullness and difficulty in urination and defecation, promote diuresis first. If ineffective, use Xuanfeng San (Wind-Releasing Powder) to purge downward. (Method from Qian Yi.)
During the eruption of smallpox, if shortness of breath occurs, Muxiang San (Costus Powder) is the primary treatment.
[Hai] To treat rashes that have erupted or not yet erupted, presenting with constipation and dryness, or occasional thirst, Sijunzi Tang (Four Gentlemen Decoction) plus Gualougen (Trichosanthis Radix) and Jiegeng (Platycodonis Radix) is used.
Danggui Wan (Angelica Pill):
Danggui (Angelicae Sinensis Radix) (0.5 liang), Gancao (Glycyrrhizae Radix et Rhizoma) (1 fen), Huanglian (Coptidis Rhizoma), Dahuang (Rhei Radix et Rhizoma) (each 2.5 qian).
First, decoct Danggui into a paste, then mix it with the powders of the other three medicines to form pills. Take them alongside the main treatment, stopping when bowel movement becomes smooth.
[Hai] Zaobian Baixiang Wan (Date-Modified Baixiang Pill): Treats constipation in smallpox/rash.
Hongya Daji (Euphorbiae Pekinensis Radix, bone removed) (1 liang), Qingzhou Zao (Chinese Jujube, pitted) (10 pieces).
Decoct these in two cups of water until the water is completely evaporated. Remove the Daji, dry the dates by roasting, and prepare them into doses or pills as needed. Start with a small amount and increase gradually, stopping when bowel movement becomes smooth.
[Huofang] Xixin San (Heart-Washing Powder): Treats high fever in smallpox, difficulty in urination and defecation, delirium, and excessive thirst.
Ingredients: Dahuang (Rhei Radix et Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), Danggui (Angelicae Sinensis Radix), Mahuang (Ephedrae Herba, nodes removed), Baizhu (Atractylodis Macrocephalae Rhizoma), Shaoyao (Paeoniae Radix), Jingjie (Schizonepetae Spica) (all in equal proportions).
The above ingredients are chopped. The dosage depends on the severity of the illness. Decoct with Shengjiang (Zingiberis Rhizoma Recens) and Bohe (Menthae Haplocalycis Herba). Taking it warm stabilizes the condition; taking it hot causes loose stools.
[Chen] If there is no bowel movement for four or five days, a piece of fat and tender pork lard can be boiled in plain water, cut into bean-sized pieces, and fed to the child. This moistens the internal organs, facilitates the falling of scabs, and ensures no obstruction.
