Acne (small boils), also known in the world as heat boils. Wang Bing's commentary states: Size like a sour jujube or a bean, red in color with pus and blood inside.
The *Huangdi Neijing* records: When sweat meets dampness, acne and prickly heat arise; prickly heat refers to sores and boils.
When laboring sweat is exposed to wind, cold forces its way into the body, stagnating in the skin leads to acne. Acne is red in color with pus and blood inside.
Xuanfu Wan (Inula Pill)
Ingredients: Xuanfuhua (Inulae Flos, 3 liang), Fangfeng (Saposhnikoviae Radix), Baizhi (Angelicae Dahuricae Radix), Ganju (Chrysanthemi Flos), Nanxing (Arisaematis Rhizoma), Banxia (Pinelliae Rhizoma), Shigao (Gypsum Fibrosum), Chuanxiong (Ligustici Chuanxiong Rhizoma), Chenpi (Citri Reticulatae Pericarpium), Baifuzi (Typhonii Rhizoma) (each 0.5 liang); Xieshao (Scorpio tail), Jiangcan (Bombyx Batryticatus) (stir-fried, each 3 qian).
Preparation and Administration: Grind the above ingredients into fine powder, form pills with ginger paste size of paulownia seeds. Take 30-50 pills after meals, washed down with ginger decoction.
Rash resembles Taiyang disease (exterior syndrome), appearing suddenly and disappearing quickly; the world mistakenly calls it "Sha zi" (sand children/prickly heat rash). Prurigo (Jie) resembles rash but lasts for months or years, varying between dry and wet, with different degrees of itching and pain; this is what the *Neijing* calls Yin Zhen (nettle rash). Tinea (Xuan) also lasts for years, characterized by dryness without moisture, itching without pain.
Zhu Danxi stated: Rash belongs to Heat, associated with Phlegm in the Lung. Treatment should clear Lung Fire and descend Phlegm, or resolve the exterior and induce sweating; there are cases where purgation is applicable. For rash at the Exterior level, use Xiaodu Yinzi (Disinfecting Decoction) or Fangfeng Tongsheng San (Wind-Protecting Universal Salvation Powder); for rash at the Interior level, use Da Chaihu Tang (Major Bupleurum Decoction) or Sishun Yinzi (Four-Smooth Decoction); for deficiency patterns, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). These treatments follow the same principles as treating Taiyang disease.
The *Taiping Shenghui Fang* records: Treating Wind-Rash entering the abdomen, causing body stiffness and dry tongue. Use Manqingzi (Brassicae Napus Seed) 3 liang ground into powder. Take 1 qian bi per dose with warm wine.
There are two types of Rash according to the Six Qi (Yunqi) theory:
First is Heat. The classic says: When Fire is depressed and erupts, the people suffer from ulcers and prickly heat. It also says: If the yearly Wood Qi is insufficient, Dryness prevails greatly; subsequently,炎暑 (hot summer) retaliates, leading to flowing fire, sores, ulcers, and prickly heat. It also says: When Shaoyin (Ministerial Fire) retaliates, the people suffer from prickly heat.
Second is Wind. The classic says: If the yearly Water Qi is insufficient, Dampness prevails greatly; subsequently, great storms erupt, causing rashes to develop in the muscles.
Li Dongyuan's Fangfeng Tongsheng San (Wind-Protecting Universal Salvation Powder): Treats wind-heat sores and tinea that do not heal for a long time. (Formula see Epilepsy section.)
Zhu Danxi Case: Zhu Yuanjun, over thirty years old, suffered from chronic Yin Zhen (nettle rash), body numbness, and purple complexion. Prescribe Fangfeng Tongsheng San added with Niubangzi (Arctii Fructus), ground to extremely fine powder. Take 2 qian per dose, add half a cup of water and ginger juice until spicy, decoct, and drink hot before meals.
Mahuang Yin (Ephedra Drink): Treats sores on the upper body, either itching or painful, with yellow fluid oozing, forming crusts, spreading across the three Yang meridians, with small roots and red swelling. This is a Damp-Heat pattern.
Ingredients: Mahuang (Ephedrae Herba, 0.5 liang, remove roots keep nodes), Fangfeng (Saposhnikoviae Radix, 0.5 liang), Qianghuo (Notopterygii Rhizoma et Radix), Shigao (Gypsum Fibrosum, 6.5 qian, crushed), Huangqin (Scutellariae Radix, 4 qian), Huashi (Haliotidis Concha, 1 liang), Chenpi (Citri Reticulatae Pericarpium), Ziping (Urticae Herba, each 7.5 qian), Niuzi (Arctii Fructus, 7.5 qian), Suosha (Amomi Fructus Rotundus, 2.5 qian), Cang'ercao (Xanthii Herba, 3.5 qian), Cangzhu (Atractylodis Rhizoma, 0.5 liang), Sheng Gancao (Glycyrrhizae Radix et Rhizoma, 3.5 qian), Boheye (Menthae Haplocalycis Herba, 1.5 qian), Jingjie (Schizonepetae Spica, 2.5 qian).
Preparation and Administration: Cut the herbs. Take 6 qian weight per dose, add 1.5 zhong of water, boil vigorously to reduce to 0.6 portion. Add 4-5 drops of good wine, strain, and take hot. Sweating throughout the body is required for the sores to settle. In severe cases, after taking about 100 doses, observe where the sweat comes out; if it flows from top to bottom past the feet, the sores will heal spontaneously.
Ju Fang Xiaodu San (Disinfecting Powder from the Imperial Formulary): (Danxi said: Xiaodu San regulates Qi, it is an exterior-resolving medicine.)
Ingredients: Fangfeng (Saposhnikoviae Radix, 1 liang), Gancao (Glycyrrhizae Radix et Rhizoma, 2 liang), Jingjiesui (Schizonepetae Spica, 3 liang), Shuniuzi (Arctii Fructus, 4 liang).
Preparation and Administration: Grind into coarse powder. Take 3 qian per dose, add 1 cup of water, decoct to 0.7 portion, strain, and take warm after meals.
Chu Yushi Case: Treating skin wind-heat, nettle rash all over the body. Use Niubangzi (Arctii Fructus) and Fuping (Urticae Herba) in equal parts, mix with Bohe Tang (Mint Decoction) to take 2 qian, twice daily.
Zhu Danxi Case: He Xiaoguan had sores, yellow urine. Use Tongsheng San 1.5 qian decocted, taken with Huangjing Wan (Polygonati Rhizoma Pill).
Yang Sangeguniang had sores, fever in the afternoon, aversion to cold during the day, emaciation, and poor appetite.
Prescription: Baizhu (Atractylodis Macrocephalae Rhizoma, 2 qian), Lianqiao (Forsythiae Fructus, 1 qian), decocted to take 30 Huangjing Wan (Polygonati Rhizoma Pills).
Zhang Congzheng (He Jian School): Wind sores often resemble Big Wind Disease (Leprosy-like conditions). Soak Mulberry leaves, steam overnight, dry in sun, grind to powder, take 2 qian bi with water decoction. (From *Experience Formula*)
*Taiping Shenghui Fang*: Treating Wind, nettle rash all over the body, painful sores. Use a small amount of Bai Jiangcan (Bombyx Batryticatus), roast until yellow, grind finely, take with wine for immediate cure.
Zhang Congzheng (Zihe) Case: Han Jiqing from Yingzhou developed wet sores from thigh to foot, large ones like coins, small ones like beans. Scratching breaks them, fluid oozes, resembling insects crawling in socks. Recurs repeatedly, leaving concave scars, unhealed for over ten years. Zihe laughed and said: This is Wet Sore. Caused by water-dampness, hence mostly on the feet. Used Zhouche Wan (Boat Vehicle Pill) and Junchuan San (River-Dredging Powder) to purge violently more than ten times, curing it instantly.
*Shan Ju Ben Cao*: Nettle rash entering the abdomen can be fatal. Use Can Sha (Bombyx Batryticatus residue) decocted strongly, wash the affected area.
*Yan Yi*: Someone suffered from fine wind-heat rashes all over the body, unbearable itching and pain, extending to head, calves, navel, abdomen, and private parts. Excessive saliva and phlegm, insomnia. Use Kushen (Sophorae Flavescentis Radix) powder 1 liang, Zaojiao (Gleditsiae Sinensis Fructus) 2 liang, water 1 sheng, rub and filter to get juice, boil in silver/copper vessel to make paste, mix with Kushen to make paulownia seed-sized pills. Take 20 pills with warm water after meals, cured next day.
Zhang Congzheng (Zihe) Case: A woodcutter bought Kushen (Sophorae Flavescentis Radix) to treat scabies. Unaware of drug potency, just heard it works, boiled a bowl strong and drank it. Vomited a basin of saliva/phlegm shortly after. Scabies crusted over in 2-3 days.
Zhu Danxi: Yishang San (One-Up Powder) treats various scabies, painful or itchy, or those preferring hot water washing.

To treat scabies, first wash off crusts with strong decoction of Cang'ercao (Xanthii Herba) or Yang Bushicao (Herba which sheep refuse to eat), then apply the above medicine. Can cure in one application.
Pediatric Facial Eczema: Face completely ulcerated, no intact skin, pus flowing profusely, unresponsive to many medicines. Use aged wax pig lard (unsalted), apply externally for miraculous effect. (From Pediatric Section.)
