When Liver Wood Qi acts recklessly, symptoms such as pain in the chest and hypochondrium, bitter taste in the mouth with dry tongue, alternating chills and fever accompanied by vomiting, irritability, fullness and obstruction of the four limbs, dysuria or difficult defecation, tendon spasm, and acute abdominal pain occur. This is caused by the invasion of the "unconquered" element (Metal overcoming Wood).

When Lung Metal is affected by pathogenic factors, it is due to the deficiency of Spleen and Stomach failing to generate Lung Metal; this is a disease affecting the "generated" element (Mother affecting Child). Consequently, there are cough, shortness of breath, rebellious Qi rising, inability of the skin and hair to resist cold, mental fatigue with thirst, and a sorrowful, unhappy demeanor. These are all manifestations of insufficient Yang Qi and excess Yin Qi, representing an excess of substance (Yin) but insufficiency of function (Yang).

Kidney Water counteracts and insults Spleen Earth, which is the "conquered" element acting recklessly. Symptoms include saliva production and clear nasal discharge, excessive sputum, polyuria, and aversion to cold. If the Fire of Spleen Earth retaliates, and the three meridians are invaded by pathogens, one may experience inability of the feet to support the body, pain in the soles preventing walking, bone weakness and lack of strength, somnolence, coldness in both testicles, dull abdominal pain, hallucinations (hearing and seeing things that are not there), and pain throughout the waist, spine, back, and hypochondrium.

[Sun Simiao] Commentary: Wang Fenglao, Vice Director of the Bureau of Palaces, was critically ill. His son, Kesheng, should have submitted a memorial posthumously requesting hereditary privilege. Sun Simiao was summoned and stated, "The Vice Director has passed away; he cannot be treated." Kesheng told Sun Simiao, "Although recovery is difficult, can we delay for three days to await the return of the imperial decree?" Sun Simiao replied, "The Vice Director's condition indicates that Liver Qi is about to cease, while Spleen Earth Qi is relatively excessive. When Spleen Earth and Fire become vigorous, Liver Qi will truly cease. Between Chen and Si hours today, if we slightly drain Spleen Earth Qi to prevent it from overcoming Liver Wood, Liver Qi will not yet cease. Three days later, if the morning decree arrives, he will die of Qi exhaustion on that day." Physician Sun's profound investigation into the waxing and waning of the five zang organs is exemplified by such cases.
