[Lo Tianyi] Yang jaundice is characterized by red and scanty urination and defecation, along with fever. This results from the struggle between Spleen Earth and Heart Fire, constituting a Yang disease. The treatment principle should be to first promote diuresis (promote urination), followed by purgation (downward draining).
For treating jaundice accompanied by abdominal distension, Yinchen Hao Tang (Cinnamon Twig and Rhubarb Decoction) should be used to administer Wuling San (Five-Ingredient Powder with Poria). If the intention is to promote diuresis, remove Dahuang (Rhubarb Root and Rhizome); if the intention is to promote defecation, add herbs such as Dahuang (Rhubarb Root and Rhizome).
Yang syndrome patients may take this (referring to those with red face and thirst for water). (The formulas for Yinchen Tang and Wuling San are both recorded in Shanghan Lun.)
[Tang] Body pain, stiffness of the shoulders and back, scanty urination and difficult defecation, yellowing of the skin, face, eyes, teeth, and nails, urine turbid like house dust. If urination is free, it is easy to treat; if scanty and difficult, it is hard to treat. It is suitable to take Wuling San (Five-Ingredient Powder with Poria) added with Yinchen (Artemisiae Scopariae Herba), decocted and taken as a soup. It is also suitable to take Daochi San (Guide Out Redness Powder) added with Yinchen (Artemisiae Scopariae Herba); if there is fever, Xiaochaihu Tang (Minor Bupleurum Decoction) is suitable. For severe cases, Chengqi Tang (Order the Qi Decoction) is taken.
[Tian] Children present with steaming fever, irritability and fullness in the chest and diaphragm, skin yellow like green orange peel, and sclera similarly yellow. This is caused by damp-heat. It is suitable to use Jiajian Xiehuang San (Modified Draining Yellow Powder) as the main treatment.
This medicine can clear heat from Spleen Earth, restore Kidney Water, and descend Heart Fire.
Huanglian (Coptidis Rhizoma), Yinchen (Artemisiae Scopariae Herba) (each 5 fen); Huangbai (Phellodendri Cortex), Huangqin (Scutellariae Radix) (each 4 fen); Zhizi (Gardeniae Fructus), Fuling (Poria), Zexie (Alismatis Rhizoma) (each 3 fen).
Crush the above ingredients, combine into one dose, decoct with one large cup of water until reduced to six-tenths, strain, and take slightly warm before meals.
[Lo Tianyi] Yin jaundice presents with relatively normal urination and defecation, yellowing of the face, eyes, and body, and cold limbs. This is due to Spleen deficiency failing to control Kidney Water. Yihuang San (Yellow-Enhancing Powder) should be used to administer Shijunzi Wan (Quisqualis Pill). (The formula for Yihuang San is found in the Treatment Methods section; the formula for Shijunzi Wan is found in the Gan accumulation section.)
Pale yellow or white complexion indicates Stomach disharmony; Pingwei San (Stomach-Calming Powder) or Tiaozhong Wan (Middle-Regulating Pill) may be used. For thirst, Renshen Baizhu San (Ginseng and Atractylodes Macrocephala Powder) is indicated. (The formula for Pingwei San is in the Diarrhea section; Tiaozhong Wan is in the Abdominal Pain section; Renshen Baizhu San is in the Xiao Ke section.) Neonatal jaundice occurring within the first month (see Congenital Convulsion entry).
[Ming Yi Zhi Zhang] Drinking water without thirst, yellow complexion: Moxibustion on Yanggang acupoint (one moxa cone).
[Qian Yi] Similarities of Jaundice: When the skin and eyes are yellow, it is jaundice. Symptoms include body pain, shoulder and back stiffness, scanty urination and difficult defecation, general yellowing of the body, yellowing of the face, eyes, fingers, and nails, urine turbid like house dust staining clothes. Thirst indicates difficulty in treatment; this is Huangdan (Jaundice). This condition often occurs after serious illnesses. Another type, not following illness but presenting with slight yellowing, is Stomach Heat. Adults are similar. Those with yellow face, abdominal distension, geophagia (eating earth), and thirst have Pi Gan (Spleen Accumulation). Those born with yellowing have Taihuang (Fetal Jaundice). Books state that all types of Dan (jaundice) are heat syndromes, specifically those with deep yellow color. Pale yellow mixed with white indicates weak and disharmonious Stomach Qi.
