Liu's young son from Liaoning, aged seven, contracted Warm Disease (Wen Bing) in late spring.
Etiology: He went to a bathhouse to bathe; after sweating, he did not wait for his sweat to dry and immediately went out into the wind, thereby developing Warm Disease. At the onset of the illness, the physician was unaware of the condition. Instead of using acrid-cool herbs to resolve the exterior, warm-hot herbs were erroneously used to induce sweating. Consequently, after profuse sweating throughout the body, the burning heat intensified. Subsequently, another physician hastily administered Chengqi Decoction (Cassia Pill Formula) to purge the bowels, further aggravating the illness due to the absence of indications for purgation. From then on, they dared not take medicine lightly. After several days of delay, as the disease progressed, I was invited to examine the patient. The patient exhibited intermittent mental confusion and delirium, faint breathing with slight panting, and burning hot skin. Upon inquiry, the patient reported intense heat in the chest. The lips were dry, cracked, and contained coagulated blood. The tongue coating was thin and yellow, with a dry, black center. Frequent drinking failed to moisten the mouth. The pulse was wiry and forceful, beating nearly six times per cycle (approx. 90-100 bpm), but felt empty upon pressure. The left cun position was particularly unable to withstand heavy pressure. Bowel movements had not occurred since the administration of the previous purgative.
Diagnosis: This condition resulted from erroneous sweating and purging methods, which damaged the Qi transformation mechanism. Additionally, the prolonged presence of warm-heat pathogens depleted the Yin aspect. This presents as a pattern of Excess Pathogen and Deficient Healthy Qi. The appropriate treatment is a large dose of Baihu Jia Renshen Tang (White Tiger Decoction plus Ginseng). Baihu Tang clears the heat, while Renshen tonifies the deficiency. Several Yin-nourishing herbs are added to replenish the depletion of Yin fluids.
Prescription: Sheng Shigao (Raw Gypsum Fibrosum, 4 liang, crushed) Zhimu (Anemarrhenae Rhizoma, 1 liang) Ye Dangshen (Wild Codonopsis Radix, 5 qian) Da Sheng Dihuang (Fresh Rehmanniae Radix, 1 liang) Sheng Huai Shanyao (Fresh Dioscoreae Hypoglaucae Rhizoma, 7 qian) Xuanshen (Scrophulariae Radix, 4 qian) Gancao (Glycyrrhizae Radix et Rhizoma, 3 qian)
Decoct together in water to yield three large bowls. Administer warm in three doses. If cured, there is no need to finish the entire course; stop taking it once the fever subsides. Rice (Jingmi) is omitted from this variation of Baihu Jia Renshen Tang because Sheng Huai Shanyao can substitute for Jingmi to harmonize the Stomach.
Outcome: After completing three doses, the heat symptoms significantly decreased, and mental clarity returned. However, bowel movements remained unpassed, and the patient still felt heat in the chest. The pulse still appeared forceful. Therefore, Shanyao was removed from the original formula, and the remaining herbs were decocted again to yield three large bowls for slow, warm consumption. After consuming two bowls, bowel movements were achieved. The medication was then discontinued, and the patient made a full recovery.
