二便俱秘胎热极，木通散与紫霜丸，行热开结真神妙，口嘬之法悉如前。

小儿初生大小便不通者，为最急候，乃胎中热毒太甚而成也，
急用前口咂五心脐下法，再以木通散行其热，紫霜丸开其结，庶可望生，
若延至七日，谓之一腊，肚腹硬胀，常作呻吟，则难治矣。
(木通散)
车前子 扁蓄 瞿麦 木通 赤苓 山栀 滑石飞 黄芩 生甘草 大黄
引用灯心水煎服，或入薄荷同煎。
(方歌)
二便闭兮如何医，木通散用甚为奇，车蓄瞿通苓栀子，滑石甘草大黄宜。
(紫霜丸)
代赭石一两火煨醋浸三五吹研 赤石脂一两 杏仁六十粒炒去皮尖
巴豆三十粒去油膜 以上为末，饭糊如麻子大，口服三丸，白水下。

有因热毒肛门结，或者内合无隙通，清毒宜服黑白散，脂瞒簪通导法精。

小儿初生，肛门内合有二，一者热毒太甚，壅结肛门，
一者脂膜遮瞒，无隙可通，
如肛门壅结者，急服黑白散，外用苏合香丸，作枣核状纳入孔中，
取其香能开窍，又能润泽，大便一下，庶可望生，
如脂膜遮瞒，无隙可通者，先以金玉簪透之，刺破脂膜，
再以苏合香丸照前法导之，庶可挽回于万一耳。
(黑白散) 黑牵牛半生半炒 白牵牛半生半炒 大黄生 槟榔 陈皮各五钱
生甘草三钱 元明粉一两 以上除槟榔，不过火余五味，或晒或焙，
仍合槟榔为末，同元明粉入乳钵内研细，每服五分至六七分，温蜜汤调化。
(苏合香丸) 苏合香油五钱入安息香内 安息香一两另为末用无灰酒半筋熬膏
丁香 青木香 白檀香 沈香 荜茇 香附子 诃子煨取肉 乌犀镑 朱砂各一两水飞
薰陆香 片脑各五钱研 麝香七钱半 以上为细末，入安息香膏，炼蜜和剂，
圆如芡实大，空心用沸汤化下，酒下亦可。

初生门
Simultaneous constipation of both stool and urine is caused by extreme fetal heat; Mutong San (Wood Vetch Decoction) and Zishuang Wan (Purple Frost Pill) may be used. These formulas clear heat and resolve accumulation with miraculous efficacy, while the method of oral suction (kou zuo) follows the previously described techniques.
[Note]
Infants presenting with inability to pass stool and urine at birth represent a most critical condition, resulting from excessive heat-toxin in the fetus.
Immediately apply the previously mentioned methods of oral suction on the five centers and below the umbilicus, then use Mutong San (Wood Vetch Decoction) to circulate and clear heat, and Zishuang Wan (Purple Frost Pill) to resolve accumulation, so that survival may be hoped for.
If delayed until seven days, termed one "lunar fortnight" (yi la), the abdomen becomes hard and distended with frequent moaning, making it difficult to treat.
(Mutong San / Wood Vetch Decoction)
Plantaginis Semen (Cheqianzi), Polygoni Cuspidati Herba (Bianxu), Dianthi Herba (Qumai), Akebiae Caulis (Mutong), Poria Cum Radio (Chiling), Gardeniae Fructus (Shanzhi), Talcum (Huashishi, prepared by flying), Scutellariae Radix (Huangqin), Glycyrrhizae Radix et Rhizoma Recens (Sheng Gancao), Rhei Radix et Rhizoma (Dahuang).
Decoct with Junci Medullia (Dengxincao) water, or decoct together with Menthae Haplocalycis Herba (Bohe).
(Fang Ge / Formula Song)
How to treat closure of both stool and urine? The use of Mutong San (Wood Vetch Decoction) is quite unique, containing Cheqianzi (Plantaginis Semen), Bianxu (Polygoni Cuspidati Herba), Qumai (Dianthi Herba), Mutong (Akebiae Caulis), Ling (Poria), Zhizi (Gardeniae Fructus), Huashishi (Talcum), Gancao (Glycyrrhizae Radix et Rhizoma), and Dahuang (Rhei Radix et Rhizoma).
(Zishuang Wan / Purple Frost Pill)
Haematitum (Daizheshi), 1 liang, calcined over fire, soaked in vinegar three to five times, then finely ground; Hongshizhi (Red Halloysite), 1 liang; Xingren (Apricot Seed), 60 grains, stir-fried, skin and tips removed; Badou (Croton Seed), 30 grains, oil and membrane removed. Grind all into powder, bind with rice paste into pills the size of hemp seeds. Administer orally, three pills per dose, washed down with plain water.

Newborn Section
Some cases involve anal constriction due to heat-toxin, or internal fusion of the anus with no passage. Qingdu (Clear Toxin) should be taken via Heibai San (Black and White Powder), and the method of using Zhiman Zan (Zhiman Needle) for guidance is exquisite.
[Note]
In newborns, internal fusion of the anus presents in two forms: one is excessive heat-toxin causing congestion and knotting at the anus; the other is lipid membrane covering the opening with no gap for passage.
For anal congestion and knotting, immediately administer Heibai San (Black and White Powder), and externally apply Suhexiang Wan (Styrax Pill), shaped like a date pit, inserted into the aperture.
Utilize its aromatic properties to open orifices and provide lubrication; once stool passes, survival may be hoped for.
For cases covered by lipid membrane with no gap, first pierce it with Jin Yu Zan (Golden Jade Needle) to break the membrane,
then guide the passage using Suhexiang Wan (Styrax Pill) as described above, perhaps offering a slim chance of rescue.
(Heibai San / Black and White Powder)
Qian Niu Zi (Morning Glory Seed), half raw half stir-fried; Bai Qian Niu Zi (White Morning Glory Seed), half raw half stir-fried; Sheng Dahuang (Raw Rhubarb); Binglang (Areca Seed); Chenpi (Citrus Reticulata Peel), each 5 qian; Sheng Gancao (Raw Licorice Root), 3 qian; Yuan Ming Fen (Mirabilite), 1 liang. Except for Binglang (Areca Seed), the remaining five herbs are not subjected to high heat, but dried in sun or baked.
Grind all together with Binglang (Areca Seed) into powder, mix with Yuan Ming Fen (Mirabilite) in a mortar until fine. Take 5 fen to 6-7 fen per dose, dissolved in warm honey water.
(Suhexiang Wan / Styrax Pill)
Suhexiang You (Styrax Oil), 5 qian (contained within Anxi Xiang); Anxi Xiang (Styrax Resin), 1 liang, separately ground into powder; Wu Hui Jiu (Pure Wine), half jin, boiled into paste; Dingxiang (Clove); Qingmuxiang (Lithospermum Root); Baitanxiang (Sandalwood); Chenxiang (Agarwood); Bibo (Long Pepper); Xiangfuzi (Cyperi Rhizoma); Hezi Rou (Areca Nut Flesh, roasted); Wuxi Bang (Rhino Horn, shaved); Zhusha (Cinnabar), each 1 liang, water-ground; Xunluxiang (Frankincense); Pian Nao (Borneol), each 5 qian, ground; Shexiang (Musk), 7.5 qian. Grind all into fine powder, mix with Anxi Xiang paste (Styrax Resin Paste), and bind with refined honey.
Form into pills the size of Euphorbia seed. Take on an empty stomach, dissolved in boiling water; taking with wine is also acceptable.
