Tian Gou (Heavenly Fishing) syndrome is caused by the accumulation of pathogenic heat in the Heart and Chest, leading to excessive phlegm-saliva and obstruction of Qi mechanism. Clinical manifestations include limb convulsions (Chi Zong), high fever, and symptoms resembling Jing Feng (Convulsive Disease). The infant's head and eyes look upward, resembling the shape of being fished. For treatment, if there is excessive phlegm-saliva accompanied by convulsive twitching, Jiulong Kongyan San should be used; if Jing Feng symptoms are prominent with wind pathogens, Niuhuang San should be used to dispel wind; if convulsions decrease but internal heat remains, the dosage of Renshen should be reduced, and Tengteng Yin should be used instead; if the fingernails appear cyanotic, Suhexiang Wan (Suhexia Pill) should be used.
[Note] Tian Gou syndrome in infants is caused by pathogenic heat and phlegm-saliva obstructing the chest and diaphragm, preventing their dispersion and unblocking. Upon onset, it presents with palpitations, high fever, eyes and mouth turning upward, Chi Zong (spasmodic contraction and relaxation) of hands and feet, and cyanotic fingernails. This condition resembles Jing Feng (Convulsive Disease), but the main characteristic is that the eyes often look upward, which differs slightly from ordinary Jing Feng. If phlegm is abundant with convulsions, the principal formula is Jiulong Kongyan San; if Jing Feng is severe with wind signs, the principal formula is Niuhang San; if convulsions are intense with obvious heat, the principal formula is Tengteng Yin; if all fingernails turn cyanotic, the principal formula is Suhexiang Wan.
(Jiulong Kongyan San) Formula: One Peripatus schultzei (Chijiao Wugong), coated with wine and roasted dry; Dripping Frankincense (Diluxiang) and Himalayan Beechwood (Tianzhuhuang), each 1 qian, ground together evenly; Winter Tea (Lacha), Realgar (Xionghuang), and Licorice Root roasted (Zhigancao), each 2 qian; Schizonepeta Spikelet fried (Jingjiesui) and Alum (Baifan), each 1 qian (calcined until dry); Mung Beans (Ludou), 150 grains, half raw and half cooked. All ingredients are ground into fine powder. Each dose is 5 fen, taken with Ginseng and Mint Decoction for mixing.
(Niuhuang San) Formula: Calculus Bovis (Niuhuang), 1 qian, finely ground; Cinnabar (Zhusha), 1 qian, finely ground using water flotation method; Moschus (Shexiang), 5 fen; Himalayan Beechwood (Tianzhuhuang), 2 qian; Scorpion tail (Xeshao), 1 qian; Uncaria rhynchophylla (Gouteng) [text obscured], 2 qian. All ingredients are ground evenly. Each dose is 1 zi, taken well water mixed.
(Tengteng Yin) Formula: Radix Ginseng (Renshen), Full Scorpion (Quanye, detoxified), Cornu Saigae Tataricae (Lingyangjiao), Gastrodia Rhizome (Tianma), Licorice Root roasted (Zhigancao), Uncaria rhynchophylla [text obscured]. Decoct in water and take orally.
[Note] Tian Gou syndrome belongs to internal heat and excessive phlegm; therefore, the use of Renshen should be reduced.
(Prescription Song)
For Tian Gou, Tengteng Yin must be used,
Continuous Chi Zong spasms cease not;
Ginseng, Saiga Horn, and Gouteng combined,
With roasted Licorice, Gastrodia, and whole Scorpion.
(Suhexiang Wan) See the section on Anorectal diseases for the formula.

Nei Diao (Internal Fishing) is a disease of the Liver caused by cold invasion. Symptoms include greenish stool, tidal convulsions resembling Jing Xian (Epilepsy/Convulsions), stooping posture, abdominal pain, vomiting of saliva, and red blood vessels/spots entangled in the eyes. If convulsions are severe, Tengteng Yin should be used; if there is urgent crying and abdominal pain, Muxiang Wan should be used; if limbs are cold, nails are cyanotic, and lips/mouth are black, Yangzang San should be used to warm the middle and tonify, potentially preserving life.
[Note] Nei Diao syndrome is mostly due to pre-existing Liver pathology combined with external cold invasion. Its symptoms include greenish stool and tidal convulsions, which occur at specific times; stooping abdominal pain refers to bending the waist in pain; vomiting of saliva occurs. Although this condition resembles Jing Xian, the presence of red blood spots in the eyes is its distinguishing feature. If convulsions are severe, the principal formula is Diaoteng Yin; if there is urgent crying and abdominal pain, the principal formula is Muxiang Wan; if limbs are cold, nails are cyanotic, and lips/mouth are black, the principal formula is Yangzang San. However, when Nei Diao reaches this stage, it is a critical condition of Cold damaging the Middle Jiao with Yin excess, generally considered untreatable. Using this formula for rescue may perhaps preserve life.
(Diaoteng Yin) See the Tian Gou section for the formula.
(Muxiang Wan) Formula: Myrrha (Moyao), Calcined Costus Root (Muxiang), Fried Fennel (Huixiang), Uncaria rhynchophylla [text obscured], Full Scorpion (Quanye), and Frankincense (Ruxiang), each in equal parts. First, grind Frankincense and Myrrha evenly, then add the powders of other herbs and mix thoroughly. Take a small amount of garlic, grind it finely, and mix with the powder to form pills the size of Toona seeds. Dry them in the sun. Each time, take two pills, administered with Gouteng Decoction.
(Yangzang San) Formula: Angelica Sinensis (Danggui), Agarwood (Chenxiang), Baked Costus Root (Muxiang), Cinnamon Bark (Rougui), Ligusticum Chuanxiong (Chuanxiong), each 0.5 liang; Clove (Dingxiang), 2 qian. Grind all into fine powder. Each dose is 1 qian, taken with light Ginger Decoction.
