Infants harbor heat in the abdomen, frequently groaning and pulling, sleeping restlessly. Straining causes Qi distension rushing to the navel root; if the navel appears vacuously enlarged, shiny, and floating, Omphal protrusion (Omphalocele-like condition) forms.
Quickly administer Xijiao Xiaodu Yin (Rhinoceros Horn Detoxification Decoction); Erdou San (Two Beans Powder) can reduce swelling and redness.
It is strictly forbidden to apply cold-cooling medicines on the navel; otherwise, ice-congealing effects will trap toxic heat, leading to ominous consequences.
[Note]
When infants have heat in the abdomen with no outlet for discharge, they frequently groan and pull, sleep restlessly, and strain. The Qi rushes into the navel area, causing the navel to suddenly swell, redden, become vacuously enlarged, shiny, and floating. This is named Omphal protrusion (Omphalo protrusion), caused by fetal heat, not by improper umbilical cord cutting.
Internally administer Xijiao Xiaodu Yin (Rhinoceros Horn Detoxification Decoction), externally apply Erdou San (Two Beans Powder), and the swelling will subside naturally.
It is strictly forbidden to apply cold-cooling medicines on the navel, fearing that cold coagulation of toxic heat will cause harm.
(Xijiao Xiaodu Yin [Rhinoceros Horn Detoxification Decoction]) Niu Bang Zi (Arctii Fructus, 牛蒡子) stir-fried and ground, Sheng Gan Cao (Glycyrrhizae Radix et Rhizoma, 生甘草), Jing Jie (Schizonepetae Herba, 荆芥), Fang Feng (Saposhnikoviae Radix, 防风), Jin Yin Hua (Lonicerae Japonicae Flos, 金银花). Decoct in water until cooked. Before administration, mix in fine powder of Xi Jiao (Rhinocerotis Cornu, 犀角) evenly and take orally.
(Formula Song)
Xijiao Xiaodu Yin adds Niu Bang Zi, combined with Gan Cao, Jing Jie, Fang Feng, and Jin Yin Hua. Grind Xi Jiao finely, mix evenly, and take orally; it excellently eliminates Omphal protrusion.
(Erdou San [Two Beans Powder]) Chi Xiao Dou (Adzuki Bean, 赤小豆) without removing skin, Dou Chi (Fermented Soybean, 豆豉), Tian Nan Xing (Arisaematis Rhizoma, 天南星) without skin and umbilicus, Bai Lian (Bletillae Rhizoma, 白敛) each one qian. Grind all above into fine powder. Use five fen of Ba Jiao Zhi (Musa Sapientis Juice, 芭蕉汁) to mix and apply around the four sides of the navel, twice daily.

Improper umbilical cord cutting leads to Umbilical Wind (Neonatal Tetanus), caused by contracting wind-cold-damp-water pathogens.
When about to manifest, Qu Feng San (Wind-Expelling Powder) is most effective; when fully formed with concurrent syndromes, differentiation must be clear.
For abdominal distension and constipation, use Hei Bai San (Black and White Powder); for pale face and cold limbs, use Li Zhong Tang (Regulate the Middle Decoction).
For abundant phlegm-saliva, use Jiang Can San (Silkworm Powder); for high fever and red face, use Long Dan Qing (Long Dan Clearing Decoction).
For vomiting and frequent crying, treat with Yi Pi San (Spleen-Nourishing Powder); for cyanotic lips, mouth contraction, and wind-grasping, use Cuo Feng San (Grasping-Wind Powder) to calm.
If the navel perimeter is cyanotic and the mouth is locked, it is incurable; encountering this within one week (Yi La) after birth ensures fatal outcome.
[Note]
The navel is the root of life for children, named Shen Que (Concentrated Gate), a point near the three Yin meridians. It prefers warmth and dislikes cold, prefers dryness and dislikes dampness.
If umbilical cord cutting strictly follows previous methods, how would Umbilical Wind arise?
Only due to lack of caution, allowing water-dampness, wind-cold qi to enter the navel,
the infant will inevitably suffer abdominal distension and navel swelling, crying day and night. This indicates impending Umbilical Wind; urgently use Qu Feng San (Wind-Expelling Powder) to treat.
If cold pathogen penetrates deeply and Umbilical Wind has formed, treatment should depend on concurrent signs and symptoms.
If abdomen is distended and hard, stool is unpassable, it is Wind combined with Excess; primarily use Hei Bai San (Black and White Powder).
If face is cyanotic, limbs are cold, urination and defecation are unformed, it is Wind combined with Deficiency; primarily use Li Zhong Tang (Regulate the Middle Decoction).
If phlegm-saliva is abundant, breathing is rapid and asthmatic, it is Wind combined with Phlegm; primarily use Chen Sha Jiang Can San (Cinnabar Silkworm Powder).
If body has high fever, face is red, mouth is dry, it is Wind combined with Heat; primarily use Long Dan Tang (Gentian Decoction).
If face is cyanotic, vomiting occurs, bending waist and crying frequently, it is Wind combined with Cold; primarily use Yi Pi San (Spleen-Nourishing Powder).
If mouth contracts, lips are cyanotic, convulsions do not stop, it is Wind combined with Fright; primarily use Cuo Feng San (Grasping-Wind Powder).
If the navel edge is cyanotic-black and the mouth is locked open, this is internal convulsion, an incurable condition. Umbilical Wind appearing within one week (Yi La) is also incurable.
One week (Yi La) refers to seven days. At seven days old, the infant's blood vessels are not yet consolidated; the disease has entered the zang-organs. Medical intervention is of no benefit.

(Formula Song)
For impending Umbilical Wind, use Qu Feng San; Su Ye, Fang Feng, Chen Pi, Hou Po, Zhi Ke, Mu Xiang follow.
Add Jiang Can, Gou Teng, and Gan Cao; adding fresh ginger makes it more efficacious and penetrating.
