The newborn's mouth contracts like the opening of a bag, making breastfeeding difficult; the tongue is stiff, lips are cyanotic, and there is foaming saliva. The complexion is red and yellow, indicating extreme fetal heat; cold limbs signal a critical condition where life is hard to save.
If phlegm is excessive, Jiangcan San (Silkworm Powder) is appropriate; if bowel and bladder movements are obstructed, Zishuang Wan (Purple Frost Pill) must be administered; for convulsions with fever and fright, Longdan Tang (Gentian Decoction) is highly effective; for spasms and grasping movements, Cuofeng San (Wind-Grasping Powder) brings peace.
[Note]
"Cuo Kou" refers to the contraction of the mouth and lips resembling a bag's opening, preventing suckling, accompanied by a stiff tongue, cyanotic lips, and yellow-red complexion. This results from heat accumulating in the Heart and Spleen (Xin Pi), transmitted from the mother during gestation.
This syndrome is a critical prognosis, requiring immediate treatment based on specific symptoms:
For cases with upward rushing Qi and excessive phlegm, Chensha Jiangcan San (Cinnabar and Silkworm Powder) is the primary formula.
For cases with constipation and urinary retention, Zishuang Wan (Purple Frost Pill) is the primary formula.
For cases with body heat and frequent fright, Longdan Tang (Gentian Decoction) is the primary formula.
For cases with limb spasms, Cuofeng San (Wind-Grasping Powder) is the primary formula.
If white foam is vomited and limbs become cold despite these treatments, even divine medicines will ultimately be ineffective.
(Cansha Jiangcan San) Formula: Chen Sha (Cinnabar, Cinabaris) 5 fen, processed by water flotation; Jiang Can (Bombyx Batryticatus, Stiff Silkworm) 1 qian, straight specimens, silk mouth removed, stir-fried; She Tui Pi (Boa Skin, Snake Slough) 1 qian, stir-fried; She Xiang (Musk, Moschus) 5 fen. Grind all into fine powder, mix with honey, and apply to the lips and mouth.
(Zishuang Wan) See formula under "Obstruction of Bowel and Bladder".
(Longdan Tang) See formula under "Lockjaw".
(Cuofeng San) Formula: Chi Jiao Wu Gong (Scolopendra Subspinipes Multipes, Centipede) half strip, roasted; [Missing Character] Teng [Missing Character] 1.5 qian; Zhu Sha (Cinnabar, Cinabaris), processed by water flotation; Zhi Jiang Can (Bombyx Batryticatus, Stiff Silkworm), baked; Quan Xie Wei (Scorpio Tail, Scorpio) 1 qian each; She Xiang (Musk, Moschus) 1 zi. If bathing the infant is not done carefully, allowing water to soak the umbilicus, or due to dampness from wet swaddling clothes, the umbilicus may weep and develop painful sores. In severe cases, it becomes inflamed, swollen, and ulcerated.
For Umbilical Dampness (Qi Shi), Shenqi San (Umbilical Dampness-Promoting Powder) is mandatory; for swelling and ulcers, Jinhuang San (Golden Yellow Powder) is most suitable. Treatment must follow this method; clinical application should not be doubted.
[Note]
When bathing a newborn, do not keep them in water for too long or wash arbitrarily. After swaddling, one must pay constant attention to prevent urine from soaking and eroding the umbilicus.
If negligence leads to the umbilicus remaining moist and unhealed, it is named "Umbilical Dampness," which requires application of Shenqi San.
If it becomes severely inflamed, red, and ulcerated, it is named "Umbilical Ulcer" (Qi Chuang), which requires application of Jinhuang San, thereby preventing the invasion of cold-damp pathogens into the interior.
(Shenqi San) Formula: Ku Fan (Alumen Exsiccatum, Burnt Alum) and Long Gu (Fossilia Ossis Mastodi, Dragon Bone) 2 qian each, calcined; She Xiang (Musk, Moschus) a small amount. Grind together into fine powder and sprinkle directly into the umbilicus.
(Jinhuang San) Formula: Chuan Huang Lian (Coptidis Rhizoma, Coptis Rhizome) 2.5 qian, calcined; Hu Fen (Cerussa, Basic Lead Carbonate) and Long Gu (Fossilia Ossis Mastodi, Dragon Bone) 1 qian each. Grind together into powder and apply to the affected area.
