Upper Volume External Treatment Methods - Explanation of Manipulation Techniques
[1] The "Palpation" (Mo) technique involves using the hand to carefully feel the injured area:
<1> To determine if there is a bone fracture, comminuted fracture, dislocation/subluxation, reduction, softening, or hardening.
<2> To assess whether the tendons are rigid, flexible, deviated, aligned, ruptured, wandering, thickened, everted, cold-congested, or heat-excess.
<3> As well as distinguishing between exterior and interior, deficiency and excess, and the newness or chronicity of the condition.
[2] First, palpate to determine if the injury resulted from falling/impact, joint subluxation/sprain, or blunt force trauma, then treat according to the diagnosis.
Upper Volume External Treatment Methods - Explanation of Manipulation Techniques
[1] The "Elevation/Reduction" (Duan) technique involves using one or both hands to grasp the site requiring reduction, adjusting the weight and force appropriately. It can be performed by elevating from bottom to top, pushing from outside to inside, vertically, or obliquely.
[2] Since bones have dislocated, they must be reduced manually; this allows the bone gaps to heal without waiting for a long time. The manipulation must be neither biased nor leaning, so that after recovery, there will be no risk of limb length discrepancy.
Upper Volume External Treatment Methods - Explanation of Instruments
Guolian (Cloth Bandage), made of white cloth, is used because other instruments are unsuitable for the affected area; only wrapping with cloth is appropriate, hence the name Guolian. Its length and width should be selected according to the severity of the condition.
Upper Volume External Treatment Methods - Head and Face
[1] The bone between the nostrils is called the nasal bridge bone (Bilianggu); extending downwards to the tip of the nose is called the Zhuntou (Nasal Tip).
[2] Injuries where both nostrils are sunken are treatable, and bleeding is not a hindrance.
[3] If the nasal bridge bone is depressed, apply Danggui Gao (Angelica Sinensis Ointment).
[4] If both nostrils are torn open due to impact or cut by sharp metal objects, apply Fengkou Yao (Sealing Medicine) to the wound, and externally apply Xiaodu Dingtong San (Detoxifying and Pain-Relieving Powder) to reduce swelling.
[5] If the nose is knocked off, use the Zhui Fa (Suturing/Fixing Method).
Note, Formulas:
[1] Fengkou Yao (Sealing Medicine)
Effective for treating traumatic injuries with broken skin and flesh, as well as throat or ear severance by sharp weapons, lip defects, abdominal wall rupture, and scrotal skin lacerations.
Mingjing Ruxiang (Frankincense), Moyao (Myrrh), Ercha (Catechu), Danggui (Angelicae Sinensis Radix), Shanpitan Tan (Carbonized Torreya Twig) each 1 qian; Shexiang (Moschus) 0.5 li; Pian Nao (Borneolum Syntheticum) 1 fen; Zhu Qiong Ning Ye (Leaf of *Ligustrum lucidum* or substitute with Ge Ye or Maotengzi Ye) 1 qian.
Each of the above ingredients is separately ground into fine powder, weighed and mixed uniformly. Add Shexiang and grind finely, then add Pian Nao and mix evenly. Store in a porcelain jar for future use.
[2] Xiaodu Dingtong San (Detoxifying and Pain-Relieving Powder)
Treats contusions and falls causing local swelling, hardness, and pain.
Wumingyi (Fossil Wood, stir-fried), Chuan Dahuang (Rhei Radix et Rhizoma) each 5 qian; Mu'er (Auricularia Auricula-Judae, stir-fried).
Grind all into powder, mix with honey water, and apply topically. If blood stasis is present, scarify to remove it before applying. If the tissue is necrotic, applying Danggui Gao (Angelica Sinensis Ointment) is particularly effective.
[3] Shenxiao Danggui Gao (Miraculous Angelica Sinensis Ointment)
This ointment promotes wound closure and tissue regeneration, draws out toxins, relieves pain, and treats various sores with excessive qi stagnation and suppuration.
Danggui (Angelicae Sinensis Radix), Huangla (Cera Flava) each 1 liang; Mayou (Sesamum Indicum Oil) 4 liang.
Fry Danggui in the oil until charred black, filter out the dregs, then add Huangla and stir rapidly until melted. Allow to cool and store in a porcelain container. Apply using old silk cloth when needed. An alternative formula uses Baila (White Beeswax).
[4] Zhui Fa (Suturing/Fixing Method)
The same method applies to ears bitten off.
Use human hair, place it in a Yangcheng pot, seal with salt mud, calcine into powder. Quickly dip the injured ear or nose into the medicine and reattach to the original site, securing firmly with soft silk cloth for efficacy. Historically, Chanter Jiang Huai had his nose bitten off by a donkey; a monk used this method to reattach it perfectly.
