Treats Yangming disease syndrome, characterized by sweating, thirst with desire to drink water, a pulse that is surging, large, floating, and slippery, aversion to cold absent but aversion to heat present.
Shigao (Gypsum Fibrosum) (crushed, wrapped in cotton cloth) one jin; Zimu (Anemarrhenae Rhizoma) six liang; Gancao (Glycyrrhizae Radix et Rhizoma) two liang; Jingmi (Oryzae Sativa Granum) six he. The above four ingredients,
with ten dou of water, boil until the rice is cooked and the decoction is ready, remove the dregs, take one sheng warm, three times a day.
[Collected Annotations] Ke Qin said: In Yangming, pathogenic factors transform into heat from the exterior, thus there is no aversion to cold but rather an aversion to heat; heat steams and escapes outward, thus hot sweat occurs spontaneously; heat scorches the stomach interior, thus there is thirst with desire to drink water; pathogenic factors are abundant and excess, thus the pulse is slippery, yet since it remains in the meridians, it also presents as floating. Generally speaking, Yangming belongs to the Stomach, which externally governs the muscles. Although there is intense heat, it has not formed an excess condition internally, so ultimately bitter-cold substances alone cannot treat it. Shigao (Gypsum Fibrosum) is acrid and cold; acridity can resolve muscle-layer heat, while coldness can overcome stomach fire. Its nature is heavy and descending, while its acridity moves outward, possessing both internal and external capabilities; therefore, it serves as the Sovereign herb. Zimu (Anemarrhenae Rhizoma) is bitter and moistening; bitterness purges fire, while moisture nourishes dryness; therefore, it serves as the Minister herb. Gancao (Glycyrrhizae Radix et Rhizoma) and Jingmi (Oryzae Sativa Granum) are used to harmonize the Middle Jiao (Central Palace), and they can also drain fire within the Earth element, adhering to the sweet nature of sowing and reaping. When cold agents meet them, their coldness is moderated; when bitter drugs meet them, their bitterness is balanced, allowing the descending properties to linger within the body. With these two as Assistant herbs, there is no concern that the great cold substances will damage the Spleen and Stomach. Once the decoction enters the Stomach, it is distributed to the Spleen and Lungs; fluids and essence are distributed throughout the body, and severe irritability and thirst can be eliminated. Baihu (White Tiger) is the Metal Spirit of the West; naming the formula after it implies that when Autumn Metal is in command, scorching summer heat naturally resolves. Furthermore, adding Renshen (Ginseng Radix et Rhizoma) tonifies the Middle and benefits Qi while generating fluids, coordinating with the tonifying effects of Gancao (Glycyrrhizae Radix et Rhizoma) and Jingmi (Oryzae Sativa Granum), counteracting the coldness of Shigao (Gypsum Fibrosum) and Zimu (Anemarrhenae Rhizoma), draining fire without injuring Earth, representing a strategy of absolute security.
Treats Taiyang (Sunshine Meridian) affected by summer-heat, characterized by sweating, aversion to cold, feverish body, and thirst; this is known as Ye (Heatstroke).
Shigao (Gypsum Fibrosum) one jin; Zimu (Anemarrhenae Rhizoma) six liang; Gancao (Glycyrrhizae Radix et Rhizoma) two liang; Jingmi (Oryzae Sativa Granum) six he; Renshen (Ginseng Radix et Rhizoma) three liang. The above five ingredients, prepare and administer the decoction using water as described previously.
[Collected Annotations] Zhao Liang said: Sweating, aversion to cold, feverish body but without thirst indicates Fengzhong (Wind Strike). Sweating, aversion to cold, feverish body, and thirst indicate Zhongye (Summer-heat Stroke). Their symptoms are similar, distinguished solely by the presence or absence of thirst. However, Shanghan (Cold Damage) and Fengzhong (Wind Strike) both present with slight aversion to cold at the back, and sometimes with wind aversion accompanied by thirst; these are also treated with Baihu Jia Renshen Tang (White Tiger Decoction plus Ginseng). This is because fire scorces Lung Metal; the Lung governs Qi. When the Lung is injured, Wei Qi (Defensive Qi) becomes deficient; when Wei Qi is deficient, the exterior is insufficient, leading to sweating, fever, and aversion to cold. The *Neijing* states: "Heart transfers heat to the Lung, transmitting it as Ge Xiao (Diaphragmatic Consumption)." Ge Xiao results in thirst; all these are caused by Minister Fire injuring the Lung. Thus, it can be known that the key lies in rescuing the Lung. Shigao (Gypsum Fibrosum) can treat fire-heat in the Sanjiao (Triple Burner), with efficacy extending beyond clearing the Lung; it retreats fire within the Lung, hence it is used as the Sovereign herb. Zimu (Anemarrhenae Rhizoma) also clears Heart fire within the Lung and nourishes the source of Water; Renshen (Ginseng Radix et Rhizoma) generates fluids and benefits the injured Qi, serving as the Minister herb. Jingmi (Oryzae Sativa Granum) and Gancao (Glycyrrhizae Radix et Rhizoma) tonify Earth to support Metal, serving as Assistant herbs.
