[Formula Song] Jiehou Carbuncle (Carbuncle at the Adam's Apple) arises in the center of the anterior neck; Liver and Lung accumulated heat obstructs the throat, creating a perilous condition. If pus forms and is not rapidly punctured, ulceration penetrating the throat becomes difficult to treat.
[Annotation] This carbuncle occurs above the Adam's apple on the anterior neck, also known as Mengju (Fierce Abscess), due to its fierce toxic nature. The center of the anterior neck belongs to the Ren Mai (Conception Vessel) meridian, combined with accumulated heat and indignation from the Liver and Lung meridians. Severe swelling blocks the throat, preventing the intake of soup and water, which is terrifyingly dangerous. If pus forms and is not punctured, leading to internal ulceration through the throat, survival is unlikely. Initially, administer Huanglian Xiaodu Yin (Coptis Decoction for Eliminating Toxins); externally apply Erwei Baidu San (Two-Ingredient Antidote Powder). For management when approaching ulceration, refer to the sections on Carbuncle/Sore Ulcer and Ulcerative Sore.
Formulas: Huanglian Xiaodu Yin (see Baihui Carbuncle of the Head)
Erwei Baidu San (see Sore Ulcer Section)
[Formula Song] Jiahou Carbuncle (Parapharyngeal Carbuncle) arises on both sides of the throat; Liver and Stomach toxic heat generates this sore. Its treatment is the same as Jiehou Carbuncle, but phlegm obstruction causing frequent choking is particularly concerning.
[Annotation] This carbuncle is also called Jiaju (Parapharyngeal Abscess), arising on both sides of the Adam's apple, caused by fire-toxicity attacking upward from the Foot Jueyin Liver Meridian and Foot Yangming Stomach Meridian. Its treatment method is the same as that for Jiehou Carbuncle.
Attributes: Small Luoli (Scrofula) and large Lili involve the Three Yang Meridians, arising in the anterior neck, posterior neck, and lateral sides. Although named differently as Phlegm, Dampness, Qi, or Tendon types, they all originate from indignation and stagnant heat. One must carefully examine the lingering symptoms; if prolonged, it may evolve into consumption (Lao disease) with no therapeutic effect.
[Annotation] This condition is called Luoli if small, and Lili if large. It should be differentiated by meridians: if arising in the anterior neck, it belongs to the Yangming Meridian, named Tanluoli (Phlegm Luoli); posterior neck belongs to the Taiyang Meridian, named Shiluoli (Dampness Luoli); left and right sides of the neck belong to the Shaoyang Meridian, soft in texture and swelling upon anger, named Qili (Qi Lili); hard with tendon contraction is named Jinli (Tendon Lili); if continuous like a string of pearls, it is Luoli; or long like a clam, red and hard, painful like burning iron, with fierce swelling, named Madao (Hatchet). Luoli also includes Zimu Li (Mother-Child Lili) of varying sizes. There is Chongtai Li (Double-Tier Lili), where three to five nodules pile up on the lili, forming clusters. Those growing around the neck are named Shepan Li (Snake-Coiling Lili). Those resembling beans in a pod are named Suoxiang Li (Neck-Locking Lili). Arising at the root of the left ear is Fengwo Li (Honeycomb Lili). Arising at the upper root of the ear is Huidai Li (Bag Lili). Small and itchy is Fengli (Wind Lili). Red, swollen, and painful in the jaw is Yanyao Li (Swallow's Nest Lili). Extending to the chest and axilla is Guateng Li (Melon-Vine Lili). Arising in the soft flesh beside the breasts and between the thighs is Qi Yangli. Arising all over the body, diffuse swelling and soft, with a hard core inside the sac is Liuzhu Li (Metastatic Lili). A single one arising at the fontanelle is Danke Li (Single-Cavity Lili). One sac containing ten to twenty nodules is Lianzi Li (Lotus Seed Lili). Hard as a brick is Men shuan Li (Door Bolt Lili). Resembling a lychee is Shi Li (Stone Lili). Mouse-shaped is Shuli (Mouse Lili), also named Shuchuang (Mouse Sore). Among the above types, those movable upon pushing are rootless, belonging to Yang; external treatment should use acupuncture, plasters, or cauterization according to the syndrome. Those immovable upon pushing have deep roots, belonging to Yin, and are considered incurable. Puncture, bloodletting, and corrosive medicines are strictly contraindicated; misuse makes healing difficult.
Although Luoli manifests in various shapes and names, the pathology stems from the accumulation of Phlegm, Dampness, Wind, Heat, or Qi-Toxin, invariably accompanied by indignation, depression, stagnation, or unfulfilled ambitions. External Wind pathogen combined with internal Phlegm-Dampness binding in the meridians causes initial chills followed by fever, diffuse swelling with slight heat, normal skin color, easy resolution, ulceration, and healing; this is Wind-Toxin. Use Fangfeng Qianghuo Tang or Haicai Wan selectively. Excessive heat in weather, summer-dampness affecting the Three Yang Meridians, combined with rich food intake,酿结 (ferments and accumulates) to cause redness, slight heat, hard tubercles with slow swelling, difficult resolution, delayed ulceration and healing; this is Heat-Toxin. Use Shengyang Tiaojing Tang, Chaihu Lianqiao Tang, or Jiming San selectively based on severity. Contracting the killing qi of the four seasons causes sudden masses in the ears, neck, chest, and axilla, rapid violent swelling, red hot skin, chills and fever, dizziness, stiff neck, and pain; this is Qi-Toxin. Use Li Gao's Lianqiao Sanjian Tang or Sanzhong Kuijian Tang according to the syndrome. Liver injury from indignation and Blood deficiency failing to nourish tendons results in hard nodules and contracted tendons immovable upon pushing; this is Jinluoli (Tendon Luoli). Initially administer Shugan Kuijian Tang, followed by Xiangbei Yangrong Tang. Ingestion of sweat, insects, ant/mouse residues, stale water, or unclean tea causes lesions starting small and growing large, continuous like pearls, connecting three to five nodules, without chills/fever, initially painless but painful later; this is Poisonous Food Toxin. Use methods from Yang Family's Collection for Luoli, such as Prepared Spirit Chicken Eggs, selecting based on excess/deficiency for self-healing.
Shiluoli (Dampness Luoli) on both sides of the posterior neck belongs to the Bladder Cold Water meridian; external Wind pathogen condenses with Dampness, causing diffuse swelling and pain with normal skin color. As it approaches ulceration, skin turns red, slight heat and severe pain occur. Internal and external treatments should avoid cold/cool herbs. Initial swelling uses Fuzi Baidu Tang and external Shen Gong San; approaching or after ulceration, follow general Carbuncle/Ulcer protocols. Warmth throughout treatment is effective; violating cold/cool properties causes neck/back stiffness, sinking sores, internal toxic attack, and diarrhea, indicating adverse prognosis. Men with Luoli should not exhibit exposed Qingjin (Green Veins) of the Taiyang meridian, tidal fever, cough, spontaneous sweating, or night sweats; women should not show red threads in eyes, amenorrhea, bone-steaming fever, or five-palm heat. Presence of these in men or women indicates transformation into Chuang Lao (Sore Consumption), an adverse sign difficult to cure.
