[Formula Song] The Dubmen abscess arises in the calf of the thigh, while the Jimen abscess occurs on the medial aspect of the thigh near the knee. Both conditions present with redness, swelling, burning heat, and pain, resulting from damp-heat accumulation in the Bladder Meridian and Spleen Meridian.
[Annotation] These two conditions are both caused by the coagulation of damp-heat. The Dubmen abscess (Abscess at the Calf) arises in the calf of the thigh and belongs to the Foot-Taiyang Bladder Meridian; the Jimen abscess (Abscess at Jimen Point) arises on the medial aspect of the thigh near the knee and belongs to the Foot-Taiyin Spleen Meridian. In the initial stage, if there is redness, swelling, burning, and pain, Shen Shou Wei Sheng Tang (Divine Grant Health-Preserving Decoction) is appropriate. If there is burning swelling accompanied by constipation, irritability, aversion to cold, and a rapid pulse, this indicates excessive heat within; Nei Shu Huang Lian Tang (Internal Draining Coptis Decoction) or Shuang Jie Gui Jin Wan (Double Resolving Golden Pill) should be used for purgation. If there is swelling and pain accompanied by alternating chills and fever, a deep and weak pulse, chest and abdominal distension, but normal appetite, Bing Su San (Betel Nut and Perilla Powder) is appropriate. Once the swelling, pain, and alternating chills and fever have subsided, switch to Xiao Yao San (Free and Easy Wanderer Powder). If the swelling and pain show no change in color, accompanied by chills and fever, poor appetite, and fatigue, this is due to Liver deficiency with damp-phlegm sinking downward; Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) added with Fu Ling (Poria), Ban Xia (Pinellia Rhizome), and Bai Shao (Peony Root) is appropriate. If sexual intercourse occurs during the illness, leading to hard swelling and obstruction of urination and defecation, Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia) added as Niuxi (Achyranthes Root) and Che Qian Zi (Plantago Seed) is appropriate; once urination and defecation become unobstructed, continue taking Bu Zhong Yi Qi Tang. For other treatments, refer to the sections on Carbuncles and Ulcers.
Formula: Bing Su San (Betel Nut and Perilla Powder)
Composition: Bing Lang (Areca Seed), Zi Su Ye (Perilla Leaf), Xiang Fu (Cyperus Rhizome), Mu Gua (Chaenomelis Frizctus), Chen Pi (Citrus Reticulatae Pericarpium), Da Fu Pi (Areca Peel), each 1 qian; Qiang Huo (Notopterygii Rhizoma et Radix) 0.5 qian; Mu Xiang (Aucklandiae Radix) 0.3 qian. Add three slices of Sheng Jiang (Fresh Ginger), three cun of Cong Bai (Scallion White), and two zhong of water; decoct to one zhong. Take on an empty stomach.
[Formula Song] Bing Su San treats abdominal distension and qi stagnation, eliminating the Jimen abscess on the inner thigh. It contains Xiang Fu, Mu Gua, Chen Pi, Da Fu Pi, Mu Xiang, Qiang Huo, Bing Lang, and Zi Su.
Other Formulas: Shen Shou Wei Sheng Tang, Nei Shu Huang Lian Tang, Shuang Jie Gui Jin Wan (see section on Carbuncles); Xiao Yao San (see section on Upper Back Hand-Da Carbuncle); Bu Zhong Yi Qi Tang (see section on Ulcers); Liu Wei Di Huang Wan (see section on Facial Freckles).
[Formula Song] Leg You Feng (Wandering Wind of the Leg) arises encircling the leg, presenting with redness and swelling like clouds, accompanied by burning heat and pain. This results from the struggle and stagnation between Ying (Nutritive) and Wei (Defensive) qi interacting with wind-heat. It is appropriate to use Bian Shi (stone lancet) therapy to bleed out bad blood and administer Shuang Jie Qing (Double Resolving Clearing Agent).
[Annotation] This condition presents as sudden redness and swelling on the medial and lateral aspects of both legs, shaped like piled clouds, with burning heat and pain. It is caused by the struggle and stagnation of Ying and Wei qi combined with wind-heat. Whenever this condition occurs, first apply Bian Shi therapy to release bad blood, then take Shuang Jie Tong Sheng San (Double Resolving Holy Guardian Powder), followed by Dang Gui Nian Tong Tang (Angelica Pain-Relieving Decoction) to clear and resolve. Externally apply beef slices to draw out wind-toxin, which is very effective.
Formula: Dang Gui Nian Tong Tang (Angelica Pain-Relieving Decoction)
Composition: Dang Gui (Angelica Sinensis Radix), Qiang Huo (Notopterygii Rhizoma et Radix), Yin Chen Hao (Artemisiae Scopariae Herba), Cang Zhu (Atractylodis Rhizoma, soaked in rice water and stir-fried), Fang Feng (Saposhnikoviae Radix), each 1 qian; Ku Shen (Sophorae Flavescentis Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma, stir-fried with soil), Sheng Ma (Cimicifugae Rhizoma), each 0.7 qian; Ge Gen (Puerariae Lobatae Radix), Ze Xie (Alismatis Rhizoma), Ren Shen (Ginseng Radix), Zhi Mu (Anemarrhenae Rhizoma), Huang Qin (Scutellariae Radix), Zhu Ling (Polyporus), Gan Cao (Glycyrrhizae Radix et Rhizoma), each 0.5 qian; Huang Bo (Phellodendri Cortex) 0.3 qian. Use two zhong of water, decoct to eight fen. Take before meals.
[Formula Song] Dang Gui Nian Tong Tang treats Leg You Feng, containing Qiang Huo, Ren Shen, Er Zhu (Cang Zhu and Bai Zhu), Sheng Ma, Yin Chen, Ge Gen, Gan Cao, Huang Qin, Zhi Mu, Huang Bo, Ku Shen, Fang Feng, Ze Xie, and Zhu Ling.
Other Formula: Shuang Jie Tong Sheng San (see section on Lip Wind).
