[Prescription Verse] Carbuncle of the hypochondrium presents with redness, swelling, and pain; a carbuncle (ulcerative sore) is hard, sunken, diffuse, cold, and non-red. Both arise from the knotting of fire and anger in the Liver and Gallbladder. Delayed ulceration with foul pus and cold deficiency indicates a ominous prognosis.
[Annotation] This condition arises in the soft rib area; the part with hard bone is called "rib" (Lei), and the soft flesh below the ribs is called "hypochondrium" (Ji Xie). Both carbuncles (Yong) and ulcers (Ju) are caused by the condensation of Liver and Gallbladder fire, often occurring in individuals with weak constitutions. Initially, it resembles a plum or walnut, gradually growing to the size of a bowl or basin, with red color, burning pain, and high swelling. After fourteen days, it ulcerates with thick pus, which characterizes a carbuncle. If it is hard, flat, diffusely swollen, numb and painful, without redness or heat, and ulcerates after more than a month with thin pus, it is an ulcer. If mismanaged, failing to resolve by the expected time and instead attacking to form pus, with swelling like a drum and bursting to release foul-smelling, watery pus, it is a adverse sign (Ni). For both carbuncles and ulcers during the initial swelling stage, Chai Hu Qing Gan Tang (Bupleurum Liver-Clearing Decoction) should be taken urgently to relieve depression and purge fire. If already formed, Tuo Li Tou Nong Tang (Support Interior and Permeate Pus Decoction) should be taken. When pus is mature and causing distending pain, a lying needle should be used to open it. After ulceration, the key is to drain remaining pus and tonify Qi and Blood. Note: Refer to the treatment methods for ulcerated sores in the Carbuncle and Ulcer section. Cases where tonification fails are difficult to treat.
Prescriptions: Chai Hu Qing Gan Tang (See Head Section: Temporal Carbuncle); Tuo Li Tou Nong Tang (See Head Section: Brain-Infiltrating Ulcer).
[Prescription Verse] Lung abscess (Fei Yong) results from lung heat compounded by wind injury, causing hidden chest pain due to lung carbuncle. Symptoms resemble cold damage but with severe dry cough, thick turbid sputum, and foul-smelling pus. Before ulceration, She Gan Ma Huang Tang induces sweating; if qi stagnation prevents lying down, Ting Li Da Zao Tang attacks the excess. After ulceration, thick pus with ability to eat is auspicious; thin pus with blood and inability to eat is ominous.
[Annotation] This condition stems from heat accumulation in the lungs combined with wind pathogen invasion, leading to long-term stagnation and abscess formation. It causes hidden pain in the Zhongfu acupoint (LU-1) within the chest, chills, rapid pulse, symptoms resembling cold damage, throat dryness and irritability without thirst, cough with wheezing and chest fullness, and expectoration of thick, sticky yellow sputum mixed with foul-smelling pus and blood. During the pre-ulceration phase, while pus is not fully formed and wind is stagnant in the exterior, the method should be dispersing. Use She Gan Ma Huang Tang (Belamcanda and Ephedra Decoction) to induce sweating. If there is qi stagnation, wheezing, and inability to lie down, urgently take Ting Li Da Zao Tang (Descurainia Seed and Jujube Decoction) to purge. If cough is accompanied by slight fever, vexation, chest fullness, and skin roughness like scales (Jia Cuo), indicating impending pus formation, use Qian Jin Wei Jing Tang (Thousand-Gold Reed Stem Decoction) to induce vomiting/drainage. If foul-smelling pus resembling rice porridge is expectorated, use Jie Geng Tang (Platycodon Decoction) to drain residual pus. If foul-smelling pus is expectorated with cough and chest fullness, use Waitai Jie Geng Bai San (External Treasury Platycodon White Powder) to open blockages. If hemoptysis occurs with afternoon fever and restlessness, use Jin Li Tang (Golden Carp Decoction) as the main treatment, along with drinking child's urine. If after ulceration, there is persistent hidden pain in the chest, diaphragm, and hypochondrium, dry mouth and throat, vexation, excessive thirst, spontaneous and night sweats, insomnia, and coughing up thick foul sputum, this indicates incomplete abscess drainage with interior deficiency. Use Ning Fei Jie Geng Tang (Lung-Calming Platycodon Decoction). If the abscess has ulcerated, wheezing and foul smell cease, turbid sputum clears, but only cough, dry throat, hemoptysis, slight hypochondriac pain, and inability to lie long remain, this indicates the ulcerated site has not yet healed. Use Zi Yuan Rong Tang (Aster Root Decoction) for clearing and tonifying; remove Ban Xia and add Shi Gao if thirst is severe. If after ulceration, cough persists, pus-phlegm remains, and the body is emaciated with weak Qi, use Qing Jin Ning Fei Wan (Clear Metal Lung-Calming Pill). Generally, for this condition, warmth of the body, fine pulse, sticky pus and blood, bright sputum color, good appetite, cessation of pus/blood, and moist bowels indicate a good prognosis. Rough palms, surging and rapid pulses after ulceration, shortness of breath, malar flush, dirty pus with white blood, loss of appetite, and dry constipation indicate a bad prognosis.
Zhongfu Acupoint (LU-1), also known as the Front-Mu point of the Lung, is located between the third rib above the nipple.
