[Verse] Bi Yong (Carbuncle of the Arm) and Bi Ju (Gangrene of the Arm) arise around the arm. Ping Zi Ying Ju (flat, purple, hard, and painful lesions) are Ju; Hong Zhong Yong (red, swollen, hot, and painful lesions) are Yong. This is caused by Ying Wei (Nutritive and Defensive Qi) and Wind Evil reversing within the muscle tissue. In severe cases, it leads to fist-like contraction and pain penetrating to the bone.
[Note] This condition occurring on the lateral side of the arm belongs to the Three Yang Meridians; that on the medial side belongs to the Three Yin Meridians. Lesions that are high-swollen, red, vibrant, with intense heat and pain, and rupture quickly are Yong (Carbuncle); those that are flat-depressed, purple-dark, hard, numb, and painful, with slow rupture are Ju (Gangrene). Both result from the uneven circulation of Ying Wei (Nutritive and Defensive Qi), contracting Wind Evil, which reverses in the muscle tissue. Initially, the shape resembles a millet grain, accompanied by aversion to cold and high fever; it is appropriate to administer Jing Fang Bai Du San (Schizonepeta and Saposhnikovia Decoction to Overcome Toxins) to induce sweating. If there is intense heat, pain, irritability, and fever, it is appropriate to administer Bai Zhi Sheng Ma Tang (Angelica Dahurica and Cimicifuga Decoction) to dissipate the swelling. When pus is about to form, it is appropriate to administer Tuo Li Tou Nong Tang (Decoction to Support the Interior and Penetrate Pus), and puncture with a needle when pus matures. If it is a Ju syndrome characterized by numbness and pain without redness or heat, this belongs to deficiency of both Qi and Blood. Regardless of whether it has ruptured or not, it is appropriate to administer Shi Quan Da Bu Tang (Decoction for Ten Complete Tonifications) to support the interior and expel toxins. After rupture, internal and external treatment methods follow those in the "Yong Ju Swelling Ulcer" and "Ulcerated Ulcer" sections. If the hand contracts into a fist and cannot extend, with pain penetrating to the bone, this indicates deep ulceration damaging the vessels, which is considered a Ni (Adverse/Intractable) prognosis.

[Verse] Wan Yong (Carbuncle of the Wrist) is formed by the condensation of Wind and Fire in the Three Yang Meridians, presenting as a carbuncle on the dorsal aspect of the wrist. High swelling with rapid rupture is a Shun (Favorable) pattern, easy to treat; rotting with exposed bone is a Ni (Adverse) pattern, difficult to cure.
[Note] This condition arises on the dorsal aspect of the wrist, belonging to the Hand Three Yang Meridians, formed by the condensation of Wind and Fire. High swelling, redness, and vitality, with rupture and pus discharge leading to pain reduction within fourteen days, constitute a Shun (Favorable) pattern, which is easy to treat. The wrist is an area where skin and flesh are thin. If the condition lingers for a long time without rupturing, or presents with diffuse swelling and flat collapse, and after rupture involves rotting with exposed bone, it is a Ni (Adverse) pattern, difficult to heal. Initially, administer Jing Fang Bai Du San (Schizonepeta and Saposhnikovia Decoction to Overcome Toxins) to induce sweating, and apply Tai Yi Zi Jin Ding (Taiyi Purple Gold Pill) externally. When pus matures and is about to rupture, treat according to the methods in the "Yong Ju Swelling Ulcer" and "Ulcerated Ulcer" sections.
