According to the *Treatise on Pathogenesis and Symptoms of Various Diseases* (Bingyuan Lun): The San pulse (Scattered Pulse) may appear surging and solid, or interrupted and insufficient resembling a dying pulse, or fine and rapid, or wiry and fast. This is because the pathogenic factors involved are not uniform. Since pulses have no fixed pattern, incompetent physicians cannot recognize them. However, when heat is excessive, the pulse becomes wiry and fast; when there are accumulations, it is surging and solid; in acute pain, it is interrupted. When the pulse is deep and rapid, the medicinal effect is difficult to manifest; when it is floating and large, the effect is easy to manifest. When the effect is difficult to manifest, one often does not perceive the movement of the medicinal power (heat). The medicine acts internally but has not yet manifested externally. To observe whether the medicine is effective: eating more is the first sign; breath sinking down with a harmonious and pleasant complexion is the second sign; itching of the head, face, and body is the third sign; slight aversion to wind is the fourth sign; drowsiness is the fifth sign.
Huangfu Mi and Vice Minister Xue described forty-two variations of symptoms triggered by Cold Food Powder (Han Shi San) along with methods for regulation and rescue. (Current examination reveals fifty-one variations.)
Huangfu Mi stated: Those who take Cold Food Powder with proper regulation will resolve within one month, or twenty days. Being able to tolerate warmth but not cold indicates resolution.
Those who lose regulation may suffer from splitting headaches due to consuming warm food after taking the medicine; purgation should be applied immediately.
Eyes may feel as if they will protrude due to heat invading the Liver; rapid purgation using cold properties will stop this.
Lower back pain feeling as if breaking occurs due to heavy clothing causing high body temperature; wash with cold water and apply cold stones.
Dizziness, vertigo, and fainting occur due to warm clothing and heat invasion; strike the head and wash with cold water.
Vice Minister Xue said: One must sleep alone; for travel and labor, washing with cold water leads to recovery.
Eye pain like needle pricks occurs because hot qi rushes to both eyes; diligently eat cold food and wash with warm urine in the early morning.
Blindness occurs due to warm diet and living conditions; remove clothes, exert oneself, wash, and consume frozen drinks and food; clarity returns shortly.
Edema of limbs, face, and eyes occurs due to warm diet and lack of exertion, causing medicinal qi to separate and gather with healthy qi; drink hot wine, eat cold food, exert oneself, and wash with cold water for improvement.
Tinnitus like wind sound and sweating occur due to over-exertion leading to improvement, irregular sexual activity, and qi rushing to the ears; maintain good diet, walk slowly, and regulate emotions to stop it.
A rotten egg smell from the nose occurs due to warm clothing; remove clothes and wash with cold water to stop it. (Some versions say: wash with cold water and wear thin clothes for recovery.)
Oral injury, stiff tongue, ulceration, dryness, and inability to eat occur due to insufficient food intake, deficiency of grain qi, and medicine accumulating in the stomach tube; urgently prepare Gardenia and Fermented Soybean Decoction (Zhizi Chi Tang), take three doses for recovery. (Some versions say two doses. Note: Zhizi Tang is in Volume 20 "Formulas for Dry Mouth".)
Swollen gums, ulcerated lips, loose painful teeth, and locked jaw (Jiache acupoint) occur due to unrescued heat invasion; open mouth facing the wind to let cold air enter the throat and gargle cold water for recovery.
Sore throat, nasal congestion, and clear runny nose occur due to warm clothing near fire; urge to remove clothes, wash with cold water, face the wind, and apply cold stones to throat and cheeks five or six times for recovery. Some versions say: remove clothes to get cold, stand facing wind, apply cold objects to throat for a moment to heal, no need to wash.
Cough, reverse flow of qi, throat injury, and clear blood discharge occur due to warm resting or eating warm food; drink cold water and apply cold stones to the outside of the throat.
Acute pain in one arm or leg occurs due to long sitting/lying on mats, stationary body temperature, and heat entering muscles attaching to bones; frequently press with cloth soaked in cold water, replace when warm.
Ulcerating stinging pain in both armpits occurs due to arms pressing against ribs; suspend hands away from ribs with an object and apply cold stones.
Chest and hypochondriac fullness, reverse qi, and dry retching occur due to hunger without eating, with medicinal qi steaming the diaphragm; urge cold food, frozen drinks, and cold wash for recovery.
Pain in limbs and joints wanting to relax, body sores and hard nodules occur due to long immobility in sleeping/living areas, sudden heat gathering in one place, or hard nodular pain. Severe cases develop abscesses; wash with cold water and apply cold stones. Mild cases dissipate in a meal time; severe cases require continuous daily washing until recovery. Washing has no limit, stopping only when recovered. Vice Minister Xue said: Sores, qi stagnation, swelling, and pain preventing movement indicate excessive self-exertion.
Abdominal distension feeling like bursting, severe cases breaking belts, occurs due to long immobility, lower heat, warm clothing, loss of diet/washing/exercise; urge walking, drink hot wine, eat cold food, wash with cold water, comb hair standing facing the wind.
Lower back pain feeling like breaking occurs due to heavy clothing and high body temperature; wash with cold water and apply cold stones. Vice Minister Xue said: Lower back pain breaking and eyes protruding indicate heat rushing to Liver, Diaphragm, Waist, and Kidneys, caused by extreme cold.
Foot pain like breaking occurs due to long sitting in warm areas; sit on cold beds, wash with cold water, and walk. Some versions say: sitting on a single bed is best.
Sores between toes occur due to warm shoes; remove shoes, wear wooden clogs, and wash feet with cold water for recovery. Vice Minister Xue said: Walk on cold ground and wash feet with cold water for recovery.
Skin and flesh hard as wood/stone, dry, and unable to bend occur due to hot diet and warm sleeping, long-standing accumulations not resolved, obstruction of Five Viscera, and blocked blood vessels; urge purgation, eat cold food, drink hot wine, and walk/exert oneself for recovery.
Body skin pain (or internal pain in some versions), shifting locations, resembling wind (or wandering wind in some versions), is caused by heat invasion, not true wind. Cold wash and cold application resolve it.
Soreness and pain in all joints occur due to thick bedding, warm quilts, and warm clothing not removed; bedding should be very thin (major principle), use single cloth without cotton, keep clothes thin and dirty (do not wear new clothes, use old wadding). Even in winter cold, frequently expose head to wind, apply cold stones to belt, do not tie tightly initially. If suffering from soreness/stuffiness, urge into cold bath; do not endure illness fearing bathing.
Joint stiffness preventing flexion/extension occurs due to long rest without exertion, medicinal qi prevailing, healthy qi knotting and not dispersing, stagnating in blood vessels; allow self-warming then cold wash for recovery. Self-warming means moving and exerting enough to generate heat, not wearing thick clothes near fire.
Surging pulse, interrupted/dying-like pulse, or fine/rapid/strong/fast pulse occur due to various causes. Pulses vary; doctors cannot identify them uniformly. Heat excess makes it wiry/fast; accumulations make it surging/solid; acute pain makes it interrupted. Cold Food Powder heat usually manifests thus; diligence in regulation is key.
When a person is exhausted but pulse continues, it is because medicinal qi prevails in all vessels while human qi is exhausted; only medicinal qi remains active, so the pulse continues but is not vital qi. After death, body remains warm like living muscle, abdomen roars, complexion unchanged, resembling a corpse after one or two nights. Moxibustion may lead to immediate death or survival depending on the weight of medicinal qi (heavy leads to death, light to life). Although moxibustion may save life, it is not the correct method for curing the disease and may cause disaster; caution is needed as many such cases exist.
Heart pain like cone stabbing occurs due to not eating when should, not washing when should, interlocking cold and heat, qi knotting and blocking, knotting in heart, lockjaw preventing breathing; pry mouth open and urgently give hot wine according to tolerance. Let wine circulate qi, belch, then soak in cold water, apply cloth towel to painful area, replace when warm, naturally resolves. Resolve quickly with cold; diet helps. If hating clothes, slightly warm then remove for recovery. Among pains, heart pain is most urgent; rescue is like rushing to fire/water.
Qi interruption and unconsciousness with locked jaw requiring others' help: hot wine is essential. If swallowing fails, pry teeth open and pour hot wine. If wine regurgitates, continue giving it. Over half a day, if wine goes down and qi flows, consciousness returns; if not, it kills.
Mental confusion after taking medicine occurs because warm medicine fights accumulated disease; major vomiting/purging is required, otherwise death. If not vomiting/purging but continuing, cold food/drinks resolve it. Vice Minister Xue said: If unconscious and eyes closed, pry teeth and pour hot wine. If vomited out, give again. Once wine qi flows down, recovery within half a day.
Dysuria occurs due to long sitting in warm areas or heat from horse saddles entering Bladder; large amounts of cold food, wash lower abdomen with cold water, apply cold stones for one day to stop.
Thick/frequent urination occurs due to hot food and items like wheat cakes/millet (or sheep cheese in some versions); wash lower abdomen with cold water to stop. If ineffective, cold immersion of genitals is better. If still unresolved, take Zhizi Tang.
Foul-smelling ulcerated scrotum occurs due to thick mats and lower heat; sit in cold water for recovery.
Difficulty defecating with abdomen hard like coiled snake occurs due to warm invasion and long accumulation of dry feces; take cold Xiao Su Ruo paste (one to two sheng) to moisten and purge; if not, take purgatives. Vice Minister Xue said: Take Da Huang (Rhubarb) and Po Xiao (Mirabilite) to purge for recovery.
Thick/frequent stools indicate loss of regulation and impending death; difficult to treat. Purgation might save one in ten; without it, death is certain. Do not fear purgation even if it leads to death, as regret would follow.
Diarrhea resembling cold invasion occurs due to movement/eating/drinking violating heat rules. People doubt this, but it is caused by underlying stagnation/accumulation and heat violation; do not doubt. Urgently remove clothes, eat cold food, frozen drinks, and wash with cold water.
Involuntary defecation occurs due to long lower warmth and rising hot qi to Stomach/Lower Abdomen (Intestines?) causing incontinence; cold wash stops it.
Involuntary flatulence occurs due to warm invasion and untimely washing; cold wash inducing cold stops it.
Generalized edema preventing turning occurs due to long rest, no alcohol, medicinal qi sinking in skin, and blocked blood vessels; drink alcohol, cold wash, exert self, and walk for recovery. For those unable to walk, have strong people drag them; do not obey stubborn refusal; ensure joints soften before stopping. Do not over-recover. Over-recovery leads to extremity exhaustion and further disorder. If hot, wash again; some versions say drink hot wine and wash with cold water.
Excessive sleepiness without awareness occurs due to long sitting in heat/stuffiness; urgently rise and cold bath. Diet refines itself; wait for appropriate time to purge.
Insomnia at night occurs due to low food intake and internal heat; take Zhizi Tang and frequently consume cold food. Vice Minister Xue said: Take San Huang Tang (Da Huang, Huang Qin, Zhizi) and frequently consume cold food for natural sleep. (Note: This formula is in Volume 20 "Clearing Heat and Resolving Powder Activation".)
Nightmares, palpitations, and lack of control occur due to internal heat struggle, Five Elements conflict clashing with medicine; sufficient food stops it.
Contracting Cold Damage (Shang Han) or Warm Malaria (Wen Nüe) is caused by heat violation. For those who habitually take Cold Food Powder, even if long resolved, if pain recurs, rescue with Cold Food methods, never using cold. If contracting Shang Han or Wen Nüe, treat with usual medicines without fault, but do not drink hot medicines. Shang Han medicines clear heat; Malaria medicines clear accumulations; they do not conflict with Cold Food Powder, so they can be taken.
Trembling/chills like Shang Han or fever like Wen Nüe occurs due to fasting/hunger, lack of washing, long sitting/inactivity, or eating foul food; urgently cold wash and walk.
Chills, head shaking, and inability to support oneself occur due to low food intake, medicinal qi moving in skin/muscles, Five Viscera losing control, hundred vessels shaking and fighting healthy qi; force drinking hot wine to harmonize pulse, force cold food/drinks to settle viscera, force walking to adjust joints. Once wine circulates and food fills, joints adjust, then wash completely. "Complete" means clear mind and resolved illness. Vice Minister Xue said: Forceful washing dispels stagnation.
Feeling cold when removing clothes and hot when wearing them occurs due to lack of adaptation in dressing/undressing; wear slightly when cool, remove slightly when hot to stop. Washing brings clarity. Do not endure letting illness flare. Vice Minister Xue said: Not enduring washing leads to established illness.
Months of alternating chills/fever, shouting with open mouth, staring blankly, failing to recognize people: wash with over one hundred shi of water that day. If unresolved, it is due to inability to exert self, drinking cold wine, and eating warm food. Like a freezing person whose chest feels colder with cold rescue, worsening because qi freezes like ice; applying heat and drinking hot soup melts ice and dissipates qi, relieving the freezing person. Similarly, if medicinal heat gathers in heart causing chills, give hot wine urgently. Once cold resolves and qi flows, wine circulates to limbs, whole body warms, then wash with two dou of cold water to complete recovery.
Withdrawing to lie silently when medicine activates occurs due to abundant heat, low food, insufficient grain qi, and evil attacking nature; drink hot wine, cold wash, and self-exertion with food is good.
Vomiting immediately after eating, unable to stay settled, occurs due to accumulation urging purgation. Vice Minister Xue said: Urgently give Gan Cao (Licorice) drink.
Inability to digest cold food after long consumption occurs due to loss of taste; make white wine porridge, add one liang of hot Su (Perilla) food. If too stuffy, return to frozen drinks/cold food.
Aversion to food smelling like rot occurs due to warm clothing causing [internal heat]; urge purgation. If not purged, ten thousand rescues will fail. Vice Minister Xue said: Purge with San Huang Tang. (Note: San Huang Tang formula is in Volume 20 "Clearing Heat and Resolving Powder Activation".)
Alcohol not resolving, food not going down, alternating chills/fever, heating without washing and chilling after washing, lasting dozens of days (severe) or several days (mild), insomnia day/night, sorrow/anger, self-startled palpitations, panic/confusion/memory loss occurs due to long warm invasion, improper sleeping/living, eating warm food causing internal fullness with heat running with medicine, cold/heat struggling. Even with rescue methods, it cannot be resolved. I experienced this: diligently facing food, weeping, holding knife to stab myself, stopped by Shengqin forcing me away. Reflecting, I put down knife, forced food, drank cold water, and stopped; disaster was avoided by a hair's breadth.
*Pang Shi Lun* states: Signs of medicine activating include frequent stretching, headache/eye pain, body spasms, fright/palpitations, whole-body stiffness, ear fullness like cart noise, body heat worse than burning, needle-prick sensation, lockjaw/aversion to cold, confusion/unawareness of pain location, or abdominal heat like forging. These are activation signs. Severe cases: abdomen full/hard as stone, blue/black around mouth, blood in stool/urine, mostly pulseless. Only breath passes, chest is warm. Such diseases require massive flushing until recovery.
Medicine activation causing sadness/anger, opisthotonos (bow-like rigidity), wind-like appearance, blue/black face, mottled body requires massive water flushing (two to three thousand shi). After resolution, urgent purgation is needed. Initially, baths are disliked due to cold; once water volume resolves symptoms, adapt. When mind is calm, stop. If abdominal stuffiness, internal heat, thirst, silence, or desire to sleep occurs, medicine attacks internal Five Viscera; urgently take Qi Wu Zhizi Tang and bathe in fresh cold water externally. (Note: In Volume 20 "Clearing Heat..."; from *Duan Ju Fang*, named Huang Qin Tang.)
If lockjaw/spasms prevent self-exertion, use cart or pull/drag.
If trembling, pull/drag as above. If bowel movement is free, no other blockage, no heat signs, but sudden palpitation and speechlessness, urgently drink three sheng of warm good wine. If stuffy/hot, drinking water resolves it.
If bowel movement is difficult, slight abdominal fullness/qi, bitter thirst, and enlarged tongue preventing speech after onset, urgently drink Zhizi Tang.
If medicine activation is not resolved timely and chronic thirst persists, it is caused by excessive drinking. Purging resolves it.
Ge Zhichuan stated: Taking Wushi Huming Gengsheng and Zhongru Cold Food powders without proper regulation causes activation and diverse diseases. Sudden onset without gradual progression indicates heat dissipation; rescue promptly.
For any external limb/body pain deviating from normal, wash with cold water hundreds of times. For balanced heat, wet cloth with water and apply. Also wet cold stones and apply. Drink warm wine, walk leisurely.
For internal heart/abdomen pain, stuffiness, and confusion, urgently remove clothes, drink one to two sheng of cold/hot/warm wine. Gradually increase until slightly relieved, then eat cold meal. Heart pain is most urgent.
If flesh is cold and mouth locked, break teeth to pour hot wine to open.
If internal hard knots/heat generate various diseases, purge urgently. If heat causes oral ulcers, purge. If accumulations persist, aversion to food/cold, purge again.
*Xiahou Shi Lun* states: Any abnormal body sensations are medicinal qi. Observe ailments within one hour of taking medicine: mild relief, warm stomach wanting to vomit, cold/heat, pain/itch, slack/tight, dizziness/numbness, orderly/disordered. All sensations are from medicine. For warm stomach, small amount of cold water helps; increase gradually until resolved, then stop. Sensations may not be whole-body; may affect head/face/hands/feet/chest/back. Apply wet towel to affected areas; if unresolved, wash slightly. Washing resolves and stops.
*Cao She Rescue Method* states: Medicine activation causing headache, eye closing, aversion to food, vomiting after eating, unrest indicates excess; urge purgation. If headache/eye pain without aversion to food, it is Cold Food Powder disease, not necessarily pure excess; always combine with cold regulation. If excess, purging is necessary for recovery. Cold Food Powder heat kills medicinal heat; purgatives should be used based on ability to purge; do not over-purge if disease allows.
Judgment is based on disease onset. Before purging after taking medicine, do not drink alcohol (causes stuffiness/vomiting). After purging, low food intake and empty interior allow heat to invade vacuities, causing vomiting/diarrhea/abdominal fullness; resolve gradually with cold food; Zhizi Tang is the treatment. (Note: Formula in Volume 20 with modifications. Huangfu Zhizi Chi Tang from *Duan Ju Fang*.)
For medicine activation causing severe dizziness/head, loosen head knots, spread hair, fan. If improved but not fully calm, wash head. For severe heat/headache/red face, pour cold water on head; in summer heat, pour ice water. If unresolved, put ice in oil bag on head knot; remove water when improved.
For medicine activation affecting ears/eyes/mouth/teeth, tinnitus with discharge, frequent cold food, slow walking, bad breath, frozen drinks/cold wash. Oral sores/stiff tongue: take Zhizi Tang.
For medicine activation causing heart/abdomen pain, give hot wine. For lockjaw, pry mouth, soak hand towel in cold water, apply to painful area, replace when warm.
