Accumulation of wind-heat internally, with frequent onset of palpitations and fright, twitching and convulsions of sinews, obstruction and difficulty in the throat, dryness and constipation of the intestines and stomach, mania and extravagance, etc., are primarily treated by this formula.
Liver fire transforms into wind, heart fire transforms into heat. Heart heat causes palpitations; liver heat causes convulsions. Obstruction and difficulty in the throat indicate that the Lung also has fire; dryness and constipation of the intestines and stomach indicate that the Spleen also has fire; mania and extravagance refer to being arrogant and transgressing propriety. The Classics state: "Delirious speech is a loss of willpower"; it also states: "The Kidney stores the will." By this reasoning, the Kidney also has fire. This is what is meant by "one water failing to overcome five fires." Therefore, Coptis Rhizome (Huanglian) is used to drain Heart fire, Scutellaria Root (Huangqin) to drain Lung fire, Indigo Naturalis (Qingdai), Genti Root (Longdancao), and Aloe (Luhui) to drain Liver fire, Rheum Radix et Rhizoma (Dahuang) to drain Spleen fire, and Phellodendron Bark (Huangbai) to drain Kidney fire. The urgency to drain the fire of the five z-organs arises because the fire is so intense that it nearly depletes fluids; thus, draining fire serves to preserve yin. Angelicae Sinensis Radix (Danggui) is used to nourish the yin of the five z-organs during times of hyperactive fire; Aucklandiae Radix (Muxiang) and Moschus (Shexiang) are used to promote the movement of qi in the five z-organs amidst the use of harsh purgatives. For cross-reference, see the Cough section.
Agastache Herba (Huoxiang) (2 qian), Gardenia Fruit (Zhizi) (1 liang, stir-fried until black), Gypsum Fibrosum (Shigao) (5 qian), Licorice Root (Gancao) (3 liang), Saposhnikoviae Radix (Fangfeng) (4 liang).
Latent fire in the Spleen causing dryness of the lips and mouth is primarily treated by this formula.
The lips are the external manifestation of the Spleen; the mouth is the orifice of the Spleen. Therefore, dryness of the lips and mouth indicates Spleen fire. Bitterness can drain fire, hence Gardenia Fruit (Zhizi) is used; cold nature can overcome heat, hence Gypsum Fibrosum (Shigao) is used; fragrance can awaken the Spleen, hence Agastache Herba (Huoxiang) is used; sweetness can moderate the Spleen, hence Licorice Root (Gancao) is used. Saposhnikoviae Radix (Fangfeng) is used for its ability to disperse Spleen qi and ascend-disperse latent fire. One might ask why Coptis Rhizome (Huanglian) is not used? I reply: Coptis Rhizome (Huanglian) is bitter and drying; given the symptom of dry lips and mouth, it is unsuitable. Therefore, only the bitter yet moistening nature of Gardenia Fruit (Zhizi) is appropriate. Another question: Since dryness is disliked, why is Saposhnikoviae Radix (Fangfeng) not removed? I reply: Li Dongyuan has already stated that Saposhnikoviae Radix (Fangfeng) is a moistening agent among wind-dispelling herbs, which is why ancient practitioners carefully selected and used it.
