Shaoyang (Lesser Yang) follows its root, which is Ministerial Fire. Taiyin (Greater Yin) sits with its intermediate damp-earth. Jueyin (Terminal Yin) has fire as its home. Yangming (Bright Yang) takes dampness as its essence. Taiyang (Greater Yang) and Shaoyin (Lesser Yin) follow both root and branch.
Heat and cold energies wrap around each other. Wind follows fire, and sweating is appropriate. Dryness combined with dampness can be treated with purgation. If one can distinguish fire and dampness in ten thousand diseases, one can unlock the seamless lock of Huangdi (Yellow Emperor) and Qibo.
This poem collects matters that are difficult to know from previous texts. Referencing books such as *Yuanrong* and *Baoming Ji*, generally, physicians must base their medication on the "Seven Formulas" and "Ten Categories," making subsequent additions and reductions to align with the intent of sages, thereby achieving flexibility. Now, out of necessity, I list prescription examples for each syndrome. Wise readers should not mock my rigidity. Therefore, I append the original intent of Mr. Li Dongyuan's formula creation behind it, so that one may seek its purport as a model. Furthermore, Zhang Yuansu (Haizang) stated: When treating wind-stroke, medication can only support the disease; to achieve complete recovery, moxibustion and fire therapy are excellent. The *Neijing* states that acupuncture methods are more numerous than medication, which is the reason. The essentials are seen in the *Four Books of Acupuncture* and classics like *Zisheng*, so I will not repeat them.
The distinction between Shanghan (Cold Damage) and miscellaneous diseases is not fixed; one must first clarify the syndromes of yin/yang and interior/exterior.
[Yin Syndrome] The body is quiet and heavy, speech is silent, breath is difficult to distribute, eyes are unclear, nasal exhalation does not exit and inhalation does not enter, air from mouth and nose feels cold, food and fluids are not accepted, face fears cold as if scraped by a knife.
[Yang Syndrome] The body moves lightly, speech has sound, eyes are clear, nasal respiration enters and exits freely, air from mouth and nose is hot.
[Body Surface] Cool indicates disease in Yin meridians, named Yin Syndrome.
[Body Surface] Hot indicates disease in Yang meridians, named Yang Syndrome.
[Wind Injury] Nasal exhalation is coarse, mouth closed and not opening, because Lung Qi connects to Heaven.
[Food Injury] Mouth has no taste, fluids are not retained, nasal breath is even, because Spleen Qi connects to Earth.
[External Injury] Whole body is hot, starting from Taiyang. From exterior to interior, it starts with formless qi.
[Internal Injury] Hands and feet are uncoordinated, both hypochondriac regions are hot like fire, starting from Shaoyang. From interior to exterior, it starts with formed substance.
[Interior and Exterior Both Injured] Renying and Koukou pulses are both full, or firm upon lifting and pressing. Great exterior heat with aversion to cold, abdominal disharmony with excessive saliva. This is interior and exterior both injured. Upon diagnosis, first palpate palms and backs of hands. Heat in palms indicates internal injury; heat in backs of hands indicates external injury. Next, differentiate by pulse.
Syndrome and pulse differentiation for Shanghan leaves no doubt; attacking interior and releasing exterior requires detailed examination.
[Exterior Excess] Mahuang Tang. [Exterior Deficiency] Guizhi Tang. ○ Interior-Exterior Both Deficient: Sweating leads to cure.
Exterior of Fu-organs: Guizhi Mahuang Ge Ban Tang.
Interior of Fu-organs: Shaoyao Gancao Fuzi Tang.
[Interior Excess] Dachengqi Tang. [Interior Deficiency] Sini Tang.
[Middle Excess] Tiaoweichengqi Tang. [Middle Deficiency] Xiaojianzhong Tang. ○ Interior-Exterior Both Excess: Purging leads to cure.
Exterior of Six Fu-organs: Chengqi Tang. Interior of Five Zang-organs: Chengqi Tang, Didang Tang, etc.
[Exterior Syndrome Requires Sweating] Pulse floating, urgent sweating required, use Mahuang Tang. Pulse deep, slow sweating required, use Mahuang Fuzi Xixin Tang.
[Interior Syndrome Requires Purging] Pulse floating, slow purging required, use Xiao Chengqi Tang. Pulse deep, urgent purging required, use Da Chengqi Tang.
[Three Yang Syndromes] Sweating should be urgent, purging should be slow.
[Three Yin Syndromes] Sweating should be slow, purging should be urgent.
Heat at exterior: Huangqin. ○ Cold at exterior: Guizhi, Fuzi.
Heat at interior: Dahuang. ○ Cold at interior: Ganjiang, Liangjiang, Wuzhuyu, Fuzi. ○ Shanghan and Fengzhong, releasing exterior and attacking interior are originally different. For example, Mahuang Tang is an exterior medicine for Taiyang meridian; Wuling San is an interior medicine for Taiyang. All six meridians have exterior and interior medicines. Because diseases vary in interior/exterior, deficiency/excess, and depth of pathogenic qi, one cannot be rigid but must treat according to changes in condition. As seen in *Shanghan Lun* formulas and various changed disease formulas, separately listed in *Baoming Ji*.
Guizhi Tang corresponds to 28 syndromes, but only 14 formulas. Mahuang Tang corresponds to 10 syndromes, but only 5 formulas. Gegen Tang corresponds to 4 syndromes, but only 3 formulas. Qinglong Tang corresponds to 5 syndromes, but only 2 formulas. Chaihu corresponds to 20 syndromes, with 5 formulas. Chengqi Tang corresponds to 17 syndromes, with 3 formulas. Xianxiong Tang corresponds to 12 syndromes, with 3 formulas. Xiexin Tang corresponds to 9 syndromes, with 5 formulas. Didang Tang corresponds to 6 syndromes, with 3 formulas. Zhizi Tang corresponds to 5 syndromes, with 2 formulas. Fa Huang corresponds to 4 syndromes, with 3 formulas. Wuling corresponds to 15 syndromes, with 7 formulas. Ganjiang Sini corresponds to 15 syndromes, with 7 formulas. Yunqizi said: Generally, pulse is the life governor of man, so one prioritizes pulse over symptoms. Citing two methods of pulse-syndrome interaction, one should also consider temporarily, following the transformation of root and branch as the method. See *Yunqizi Maijue* in detail.
With sweat, do not take Mahuang; without sweat, how could Guizhi Tang apply?
[Shanghan] Sweating agents, use according to meridian. E.g., Taiyang uses Mahuang Tang, Shaoyang uses Xiao Chaihu Tang, Yangming uses Gegen Tang, etc. Stopping sweat agents, e.g., Taiyin uses Guizhi Tang, Shaoyin uses Sini Tang, etc.
[Miscellaneous Diseases] Sweating agents, use according to disease. E.g., Wind syndrome uses Xiao Xuming Tang, Paifeng Tang; Wind-heat uses Fangfeng Tongsheng San; Bi-Qi cold-damp uses Buhuanjin Zhengqi San, Wuji San, etc. Further, use according to Shanghan method and separate meridians, then there will be no harm from violating prohibitions. ○ Stopping sweat agents, e.g., Erjian Tang, Maijian San, Zhimu Fuling Tang, etc. Yang syndrome suddenly having string-like pulse, urgently resolve harmony, do not violate prohibitions.
For example, Shanghan Shaoyang syndrome: Hypochondriac pain, alternating chills and fever with vomiting, or coughing and deafness,寸关尺 (Cun, Guan, Chi) pulses all string-like, prohibit sweating, prohibit promoting urination, prohibit purging, suitable for Xiao Chaihu Tang. Soak shape to release sweat. Meaning: Do not violate meridian prohibitions.
Yin syndrome seeing deep-slow pulse, Zhengyang returning to Yin requires attention.
Use Zhengyang San, Fuzi, Ganjiang, etc., to warm. Or moxa.
Pulse disease should distinguish which meridian. If thirsty, follow the prescribed example.
Taiyang disease thirst, floating pulse no sweat, use Wuling San, Shihui, etc.
Yangming disease thirst, long pulse with sweat, use Baihu Tang, Liangge San, etc.
Shaoyang disease thirst, string-like pulse with vomiting, use Xiao Chaihu plus Gualougen, etc.
Taiyin disease thirst, fine pulse not desiring drink, even if drinking thinks of soup not water, etc.
Shaoyin disease thirst, deep pulse with spontaneous diarrhea, use Zhu Ling Tang, Sanhuang Tang, etc.
Jueyin disease thirst, faint pulse drawing drink, can give little by little.
Jie Xiong (Chest Binding) carefully use three Xianxiong formulas; Xu Pi (Empty Fullness) divide into five Xiexin formulas.
Shanghan causing epigastric hardness/fullness, resembling Rou Jing, suitable for Da Xianxiong Wan. Headache and neck stiffness wrongly purged, heart feels uncomfortable, evil moves to Diaphragm, thus Yang Qi sinks inside, epigastrium binds hard, hand cannot touch, suitable for Da Xianxiong Tang. If epigastrium painful on pressure, floating-slippery pulse, exterior syndrome not finished, suitable for Xiao Xianxiong Tang.
Re Pi (Heat Fullness) uses Dahuang Huanglian Xiexin Tang. Yin-Yang disharmony causing Pi, use Fuzi Xiexin Tang. Yin excess Yang deficiency causing Pi, use Banxia Xiexin Tang, Gancao Xiexin Tang, Shengjiang Xiexin Tang, select and use according to syndrome. It is not clearing Heart Heat, but clearing Heart Pi.
Abdominal pain syndromes differ in four seasons; Haizang's examples hide mysteries.
Zhongwan pain, Taiyin, use Lizhong, Jianzhong, Huangqi Tang, etc.
Navel pain, Shaoyin, use Sini, Zhenwu, Fuzi Tang, etc.
Lower abdomen pain, Jueyin, use Danggui Sini Tang.
Severe cases use Zhengyang, Huiyang, etc.
Miscellaneous disease abdominal pain, Siwu Kulian Tang, Jiuzhu Danggui, Wanzengsun Danggui Wan, Daoqi Tang, etc.
Four seasons abdominal pain, Shaoyao Gancao Tang, etc.
If abdomen not full add dates; if full do not add, this violates disease prohibition. Spleen deficiency fullness, Huangqi Tang, Shaoyao stops dampness.
If middle fullness do not eat sweet, two medicines using sweet lead to fullness. Spleen excess fullness, Pingwei San, Cangzhu dries dampness.
If large bowel constipation, Shi Pi (Solid Fullness), Nongpu, Zhishi.
If large bowel free, Xu Pi (Empty Fullness), Shaoyao, Chenpi.
Pi, Man, Zao, Shi (Fullness, Distension, Dryness, Excess) four syndromes complete, urgent treatment can apply Da Chengqi.
Taiyang Yangming, Da Chengqi Tang. If great fullness, chest and abdomen distended, like joined tiles, great excess means no stool.
Pi, Man, Zao, Shi four syndromes prepared then use it.
Shaoyang Yangming, Xiao Chengqi Tang. Use for Pi, Shi, Man.
Zhengyang Yangming, Tiaoweichengqi Tang. Excess without fullness, abdomen turns losing gas, has dry feces, no stool and delirium, Zao Shi Jian syndrome seen, can use Zhongjing's Xiao Chengqi Tang. Miscellaneous diseases same, named Sanwu Nongpo Tang.
Dahuang, Mangxiao for Da Shi, Zaoshi.
Floating: Hand-Foot Yangming, Large Intestine, Stomach.
Deep: Hand-Foot Taiyin, Lung, Spleen.
Pi, Da Man: Zhishi, Nongpu.
Da Chaihu Tang treats exterior re-having interior, or pulse floating, or headache, or aversion to wind, or aversion to cold, these four syndromes one or two remaining, this is passing meridian not resolving. Or delirious speech, or straight hand raising eyes, all interior urgency. Want to sweat, then interior syndrome already urgent; want to purge, then exterior syndrome still exists. Thus use Xiao Chaihu middle drug added flavors, harmonize Three Yang, make not violate Three Yang prohibitions. Use Shaoyao to pacify Taiyin below, let evil not enter inside. Use Dahuang to remove earth path不通 (blockage). Use Zhishi to remove epigastric pi-men. Or damp-heat spontaneous diarrhea steaming, exterior-interior syndrome already urgent, generally suitable to use.
Sanjiao (Triple Burner) cold-heat medicine suitability, Wet-Warm and Summerheat methods still different.
Upper Jiao Heat: Zhizi, Huangqin. Upper Jiao Cold: Chenpi, Nongpu, Huoxiang.
Middle Jiao Heat: Huanglian, Shaoyao. Middle Jiao Cold: Baizhu, Ganjiang, Dingxiang.
Lower Jiao Heat: Huangbai, Dahuang. Lower Jiao Cold: Rougui, Fuzi, Chenxiang.
One whole body all heat, one whole body all cold.
Digupi clears Kidney fire, generally treats heat outside. Earth is Yin, Bone is Inside, Skin is Outside.
Mudanpi treats Baozhong (Uterus/Mind Palace) fire, bone-steaming without sweat.
Zhimu clears Kidney fire, bone-steaming with sweat.
If dawn fever, heat in Xingyang (Moving Yang) division, Lung Qi governs it, thus use Baihu Tang to clear fire in Qi.
If dusk tidal fever, heat in Xingyin (Moving Yin) division, Kidney Qi governs it, thus use Digupi San to clear fire in Blood.
If clearing fire of each meridian, all belong to governing original meridian Qi-Blood medicines.
(Lung) Qi: Shigao, Blood: Huangqin. (Kidney) Qi: Zhimu, Blood: Huangbai.
(Liver) Qi: Chaihu, Blood: Huangqin. (Heart) Qi: Maidong, Blood: Huanglian.

Clearing cold of each meridian, all have governing original meridian Qi-Blood medicines.
(Liver) Qi: Wuzhuyu, Blood: Danggui. (Heart) Qi: Guixin, Blood: Danggui.
