There are cases where internal diseases are about to heal, followed by the emergence of external symptoms. There are also cases where external symptoms are about to resolve, but subsequently trigger internal diseases. Furthermore, there are instances where mild conditions become severe, and during the severe stage, they revert to being mild, creating a cyclical alternation. We also observe cases where the disease has not been present for long, yet its transmission is unpredictable and its manifestations are irregular. All these arise from the early deterioration of the Zang-Fu organs, the dispersion and separation of Essence and Blood, which fail to consolidate and restrain, causing Shen (Spirit) to lose its foundation. Consequently, illusory and abnormal signs appear, and death can be expected immediately.
Realgar (Pishuang), originally possessing extreme heat and toxicity, is a prohibited substance. Once ingested in medicine, it causes the intestines and stomach to rupture, leading to immediate death. If applied to treat head lice, it causes redness, swelling, cracking, and pain on the head and face. If used to scrub scabies and leprosy, it causes whole-body muscle swelling and skin splitting, or flesh erosion and skin peeling; mildly, it causes physical disabilities, and severely, it causes death. For those poisoned, internally administer various clearing and detoxifying medicines; externally, apply a powder made of Rhubarb (Sheng Jun), Mung Bean Powder (Lü Dou Fen), Indigo Naturalis (Qing Dai), and Coptis Rhizome (Huang Lian), mixed with old Tung Oil for application.
Some individuals who are naturally quiet and rarely angry suddenly become irritable and constantly active; this indicates Yin deficiency and impending Yang hyperactivity. Some individuals who are naturally active and prone to anger suddenly become less irritable and seek quietness; this indicates Yang insufficiency and Yin excess. Any emotions within the Seven Emotions (Joy, Fear, Love, Hate, etc.), as well as preferences in living arrangements, clothing, diet, and hobbies, that are completely opposite to one's usual state, are either omens of disaster or indicators of pathological mechanisms. Others should only accommodate their wishes and gently persuade them. One must not go against their nature, thereby exacerbating the condition.
The Brain Gate (Naohu) area has long been steamed by Damp-Heat rising upwards, while externally wrapped and constrained by Wind-Cold. The nose connects to the brain, so Qi becomes congested and obstructed. Foul-smelling pus oozes down intermittently. Just as fire burning under a pot causes the contents to change flavor, water vapor drips continuously; this condition is called Bi Yuan (Nasal Sinusitis). It is caused by Excess Fire in the Lung and Brain. Use Qing Fei Yin (Clear the Lung Decoction - see formula later), adding Pig Bile (one gallbladder) to take orally. the Gallbladder channel can connect to the brain; using a "sentient" substance to enter an "insentient" orifice can effectively control the disease. Self-efficacy is observed after taking it ten-plus times.
Patients suffering from Chancre (Xia Gan) and Drum Distension (Gu Zhang) must avoid salt and soy sauce, because the salty flavor consolidates Dampness, aids Fire, and condenses Phlegm, causing external symptoms such as swelling, pain, and rotting. However, eating bland food alone may cause the person to easily become weak and frail. Using Autumn Stone (Qiushi) as a substitute for salt in cooking is the most appropriate method.
