There is a condition known as "Zaiguai" (Disaster and Anomaly). What is meant by this? It refers to situations where a patient, who should feel cold, instead presents with heat; or who should present with heat, instead feels cold. The patient may need to vomit but does not, or needs to have diarrhea but does not. They may need to sweat but do not, or should be delirious but remain silent. They may need to sleep but cannot, or should drink water but refuse. All these phenomena belong to "Zaiguai". This is caused by the five Zang organs' Qi failing to follow and coordinate with one another. If "Sini" (Four Reversals/Cold Extremities) occurs, it is considered incurable. "Sini" refers to a state where both the ruling Qi (Zhuyun) and the seasonal Qi (Kyun) are out of season, indicating severe disharmony.
Diseases originating in the Six Fu organs belong to the Yang system. When Yang pathogenic factors arise, they may manifest above or below, inside or outside, or accumulate at the extreme depths of the middle burner. Some patients may exhibit singing and laughing, others weeping and crying, others running wildly, others groaning in pain, others curling up in submission, others acting arrogantly as if superior or virtuous, others remaining awake unable to sleep, others able to eat but suffering from constipation, and others unable to eat but experiencing spontaneous diarrhea. Some can speak with clear voices, while others cannot speak and have muffled voices. Although the manifestations vary, they all stem from dysfunction of the Six Fu organs. Treatment should accord with the patient's inclination: if they favor unblocking, use unblocking methods; if they favor obstruction, use obstructing methods; if they desire water, assist with fluids; if they desire cold, assist with ice/cold agents. Do not violate the patient's preferences, nor forcibly suppress them. By following this compliance, ten cases will yield ten cures, and hundred cases will yield hundred cures; no disease will remain uncured.
Diseases originating in the Five Zang organs all belong to Yin. Their onset may present as hemiplegia (Piangu), flaccidity syndrome (Wei), exterior cold with interior heat, or exterior heat with interior cold. Other symptoms include abdominal distension, clenched and contracted hands and feet, deviation of the mouth and eyes, numbness of the skin, difficulty walking, stiffness and rigidity of the body, incessant vomiting and diarrhea, unrelenting pain, sudden aphasia, or long-term loss of voice. The patient may lie silent and motionless, resembling a corpse. Such presentations are entirely due to Yin excess. When Yin is excessive, Yang must be deficient; when Yang is excessive, Yin must be insufficient. As stated in previous discussions: if Yang is deficient, aid it with Fire Essence (warm/hot therapies); if Yin is deficient, supplement it with Water Mother (cool/cold therapies). Therefore, if the patient favors sweating, induce sweating; if they favor warmth, apply warming methods; if they favor heat, apply hot methods; if they favor fire therapy, use fire; if they favor decoctions, administer herbal soups. The application of warm, hot, decoction, and fire therapies must be appropriate. Do not force treatment against the natural inclination. In this way, ten thousand cases will be completely resolved. The methods of Water and Fire represent true Yin and Yang. The principles of healing and rescue should be thoroughly understood.
