Discourse states: "Ke" (cough with sound) refers to coughing without phlegm but with a sound. This is caused by damage to Lung Qi, resulting in a lack of clarity and descending function. "Sou" (cough with phlegm) refers to having phlegm but no sound. This is caused by Spleen dampness stirring up and generating phlegm. If there is both sound and phlegm in the cough, it is because the disturbance of Lung Qi has activated Spleen dampness. Therefore, Ke is accompanied by Sou. Spleen dampness refers to dampness injured in autumn accumulating in the Spleen. Thus, the classics state: "Injured by dampness in autumn, cough arises in winter." Generally, the qi of early autumn should be clear and肃 (descending/constricting). If this qi is disturbed in reverse, it must rush upward causing cough, and severely, it will stir Spleen dampness to generate phlegm. From this, we know that if there is no stagnant dampness in the Spleen, even if Lung Qi is injured, phlegm will not be generated. If there is phlegm with less cold and more heat, treatment should follow the syndromes of the Five Zang organs. For example, dampness in the Liver channel is called Wind Phlegm; dampness in the Heart channel is called Heat Phlegm; dampness in the Spleen channel is called Damp Phlegm; dampness in the Lung channel is called Qi Phlegm; dampness in the Kidney channel is called Cold Phlegm. Each should be treated according to the syndrome. Cough without Sou should be treated by moistening the Lungs with acrid-sweet herbs. Cough with Sou should prioritize treating phlegm. Therefore, using Arisaema Cum Bile (Nanxing) and Pinellia Rhizome (Banxia) to overcome phlegm will allow the cough to heal spontaneously.
