*Chan Yu Bao Qing Ji Fang* (Collection of Formulas for Childbearing and Preservation) contains twenty-one treatises on childbirth that I have collected. The discussions are precise and thorough, leaving no aspect unexamined. Imperial physicians (physicians of the Imperial Medical Bureau) have extensively studied medical texts and mastered their subtleties; however, it is a pity that they possess the theories but lack the corresponding formulas. Mr. Guo Jizhong is a distinguished physician of his time, particularly skilled in treating obstetric conditions. Consequently, his pulse diagnosis and prescription of medicines frequently achieve remarkable results. One day, he expressed his willingness to attach his family's collected formulas to the end of these treatises, thus compiling this collection. It is truly an extraordinary book for saving the masses. The ancients said, "A son who does not study medicine is unfilial." Therefore, possessing formulas and treatises but not passing them down to the world—can this be called benevolent? Recorded by Li Shisheng of Puyang on the fifth day of the ninth month of the third year of Daguan.
(Compiled by Li Shizheng with treatises by Guo Jizhong and attached formulas)
Treatise One: Regarding heat diseases causing fetal death in utero, what is the cause?
Answer: This occurs when the mother suffers from a heat disease. After six or seven days, the internal organs become extremely hot, steaming and damaging the fetus, leading to its death. Since the dead fetus becomes cold and cannot expel itself, one should administer *Heishen San* (Black Divine Powder) to warm the uterus. Within moments, as the uterine qi warms, the fetus will naturally be expelled. How can one know if the fetus has died? Observe the color of the parturient woman's tongue; if it appears bluish-green, this is the sign. There are two formulas for *Heishen San*.
**Heishen San (Black Divine Powder)**
*Cinnamomi Ramulus* (Guizhi, remove coarse bark, avoid fire, chop) *Angelicae Sinensis Radix* (Danggui, wash, remove radix and rootlets, slice, dry-roast) *Paeoniae Radix* (Shaoyao, wash, chop) *Glycyrrhizae Radix et Rhizoma* (Gancao, honey-fry, chop) *Rehmanniae Radix Preparata* (Shengshengdihuang, wash with warm water, chop, avoid cold water) *Zingiberis Rhizoma* (Ganjiang, preferably use stir-fried fresh ginger instead. Each 1 liang) *Aconiti Lateralis Radix Praeparata* (Fuzi, 0.5 liang, processed, remove skin and umbilicus) *Phaseoli Semen Mungo* (Xiongheidou, 2 liang, small beans are preferred, stir-fry until scorched, remove skin)
Grind all ingredients into fine powder. Take 1 qianbi (approx. 3-5g) per dose, mixed with warm wine on an empty stomach.
**Puhuang Heishen San (Polygonum Puhuang Black Divine Powder)**
(This formula is mentioned in Treatise Eighteen; it treats all conditions before and after childbirth.)
*Phaseoli Semen Mungo* (Xiongheidou, 2 liang, small beans preferred, remove skin and weigh) *Angelicae Sinensis Radix* (Danggui, wash, remove radix and rootlets, slice, dry-roast) *Paeoniae Radix* (Shaoyao, wash, chop)
*Glycyrrhizae Radix et Rhizoma* (Gancao, honey-fry, chop) *Zingiberis Rhizoma* (Ganjiang, process, chop) *Cinnamomi Cortex* (Rougui, remove bark, chop, avoid fire) *Rehmanniae Radix Preparata* (Shengshengdihuang, wash with warm water, chop, dry-roast, avoid cold water)
*Polygoni Orientalis Spadix* (Puhuang, lightly stir-fry in silver or ceramic vessel until red, or fry in a paper-lined pot until paper turns yellow. Each 1 liang)
Grind all ingredients into fine powder. Take 2 qian per dose, mixed with warm wine on an empty stomach. For women over thirty or those who have had few births, omit Cinnamon Twig (*Guizhi*) and Dried Ginger (*Ganjiang*), replacing them with stir-fried fresh ginger (*Shengjiang*) and Safflower (*Honghua*). Use 2 liang each of ginger and safflower, washed and dried.
Treatise Two: Regarding retained placenta, what is the cause?
Answer: After delivery, blood flows into the placenta, causing it to swell and preventing expulsion. If treatment is delayed, the swelling fills the abdomen and rises upward, causing chest pain and shortness of breath; this is difficult to treat. Administer *Qiming Dan* (Miracle Life Pill) to rapidly disperse the blood within the placenta. Once the blood disperses and swelling subsides, the placenta will naturally fall without complication.
**Duoming Dan (Life-Snatching Pill)**
*Aconiti Lateralis Radix Praeparata* (Fuzi, 0.5 liang, processed, remove skin and umbilicus, slice) *Moutan Cortex* (Mudanpi, 1 liang, wash, chop) *Dryopteridis Rhizoma* (Ganqi, 1 fen, crush, stir-fry until smoke ceases)
Grind into powder. Mix with 1 sheng of strong vinegar and 1 liang of *Rhei Radix et Rhizoma* (Dahuang) powder, boil into a paste, and form pills the size of paulownia seeds. Take 50 pills with warm wine at any time.
Treatise Ten also states: Simply administering *Puhuang Heishen San* will drive out the blood from the placenta, allowing it to fall, similar to the case of cord rupture.
Treatise Three: Regarding dystocia (difficult labor), what is the cause?
Answer: A formed mass exists on the side of the fetus, called the "child pillow" (Erzhen). When birth approaches, this pillow ruptures, and the decaying blood wraps around the fetus, causing difficulty. Administer *Shengjin San* (Gold-Overcoming Powder) to expel the decaying blood, allowing natural delivery. This applies to breech presentation (Nisheng) and transverse lie (Hengsheng).
**Shengjin San (Gold-Overcoming Powder)**
*Moschus* (Shexiang, 1 qian, grind finely together) *Fermented Soybean* (Gushi, 1 liang, wrap in old tangerine peel, burn until red-hot, immediately grind with milk into powder)
Grind into powder. Take a red-hot metal weight, quench it in wine, and use this liquid to mix 1 qianbi of the powder for administration.
Treatise Eighteen also states: Simply administering *Puhuang Heishen San* resolves the decaying blood, enabling natural birth. It treats transverse lie, breech presentation, and retained placenta.
Treatise Four: Regarding postpartum blood syncope (Xueyun), what is the cause?
Answer: Postpartum, Qi and Blood are suddenly deficient. Without rest, Blood follows the ascending Qi, confusing the Heart Spirit. This causes visual disturbances (seeing spots). In severe cases, it leads to unconsciousness, locked jaw, coma, and cold breath. Physicians unfamiliar with this may misdiagnose it as "Dark Wind" (Anfeng); treating it as such will fail. Administer *Qinghun San* (Clear Soul Powder) for recovery.
**Qinghun San (Clear Soul Powder)**
*Lycii Cortex* (Zelan Ye, select clean pieces) *Ginseng Radix* (Renshen, remove radix. Each 1 fen) *Schizonepetae Spica* (Jingjie Sui, 1 liang, sun-dry) *Chuanxiong Rhizome* (Chuanxiong, 0.5 liang, wash, chop) *Glycyrrhizae Radix et Rhizoma* (Gancao, 2 qian, honey-fry, chop)
Grind into fine powder. Take 1 qianbi per dose, mix with half a small cup of hot water and half a small cup of warm wine, stir well, and administer urgently. Upon reaching the throat, eyes open, breathing stabilizes, and consciousness returns.
Treatise Eighteen also states: Simply administering *Puhuang Heishen San* leads to recovery.
Treatise Five: Regarding postpartum dry mouth and epigastric fullness, what is the cause?
Answer: Postpartum, the Uterus and Stomach are severely deficient, and Qi/Blood are unsettled. Eating noodles too early overwhelms the Stomach's digestive capacity. Noodle toxins accumulate in the epigastrium, steaming upward to the brain, causing dry mouth, thirst, and epigastric oppression. Misdiagnosing this as chest/phlegm stagnation and using purgatives is dangerous. Administer *Xianxian Wan* (Seeing Present Pill) for recovery.
**Xianxian Wan (Seeing Present Pill)**
*Curcumae Longae Rhizoma* (Jianghuang, slice form, chop) *Bannong Rhizoma* (Jingsanleng, bake, tear while hot, dry-roast) *Litsea Fructus* (Bichengqie, do not stir-fry with fire) *Citri Reticulatae Pericarpium* (Chenpi, remove white pith) *Alpiniae Officinarum Rhizoma* (Gaoliangjiang, chop, stir-fry) *Ginseng Radix* (Renshen, remove radix, slice) *Curcumae Zedoariae Rhizoma* (Peng'ezhu, bake, tear while hot, chop, dry-roast)
Grind these seven herbs into equal parts. Slowly boil chopped radish until soft, grind finely, and use the juice to make pills the size of paulownia seeds. Take 30 pills with radish decoction at any time.
Treatise Eighteen also states: Simply administering *Puhuang Heishen San* leads to recovery.
Treatise Six: Regarding postpartum alternating chills and fever, what is the cause?
Answer: Disharmony of Yin and Yang, combined with undispersed decaying blood, causes alternating chills and fever. Postpartum deficiency leads to Yin/Yang disharmony; Yin excess causes chills, Yang excess causes fever. If production labor damages organs, weak Blood fails to circulate freely, leading to stagnant blood entering the Lung (causing heat) or Spleen (causing chills). Misdiagnosing this as malaria is erroneous. For Yin/Yang disharmony, use *Zengsun Siwu Tang* (Modified Four Substances Decoction). For undispersed decaying blood, use *Duoxue Dan* (Blood-Removing Pill).
Question: How to distinguish between them?
Answer: Presence of stabbing pain indicates decaying blood; chills/fever without other symptoms indicates Yin/Yang disharmony.
**Zengsun Siwu Tang (Modified Four Substances Decoction)**
*Angelicae Sinensis Radix* (Danggui, wash, remove soil/radix/rootlets, slice, dry-roast) *Paeoniae Radix* (Shaoyao, wash, chop) *Ginseng Radix* (Renshen, remove radix, slice) *Chuanxiong Rhizome* (Chuanxiong, wash, chop. Each 1 liang) *Glycyrrhizae Radix et Rhizoma* (Gancao, 4 qian, honey-fry) *Zingiberis Rhizoma* (Ganjiang, 1 liang, preferably use fresh ginger)
Grind into powder. Take 2 qianbi per dose, decoct with 1 cup of water and 5 slices of ginger until reduced to 60%, strain, and drink warm at any time.
**Duoming Dan (Life-Snatching Pill)** (See Treatise Two)
Treatise Eighteen also states: Simply administering *Puhuang Heishen San* leads to recovery.
Treatise Seven: Regarding postpartum edema of limbs, what is the cause?
Answer: Decaying blood accumulates in the Five Zang due to deficiency, travels along meridians to the limbs, and remains there, rotting like water, causing facial and limb edema. Misdiagnosing this as fluid retention (Shuiqi) and using diuretics is harmful. Diuretics damage healthy Qi. Postpartum patients are already deficient; further depletion causes double deficiency and often death. Administer *Tiaojing San* (Regulating Menses Powder); as Blood moves, swelling subsides, and recovery occurs.
**Tiaojing San (Regulating Menses Powder)**
*Myrrha* (Moyao, grind) *Succinum* (Hupo, grind. Each 1 qian) *Cinnamomi Ramulus* (Gui, remove bark, avoid fire, chop) *Paeoniae Radix Rubra* (Chi Shaoyao, wash, chop) *Angelicae Sinensis Radix* (Danggui, remove radix/rootlets, slice. Each 1 qian) *Asari Radix et Rhizoma* (Xixin, remove leaves, chop) *Moschus* (Shexiang, grind. Each 0.5 qian)
Grind into powder. Take 0.5 qianbi per dose, mix with a little ginger juice and warm wine, and take.
Treatise Eighteen also states: Simply administer *Puhuang Heishen San*, followed by *Zhuan Yao* (Turning Medicine), which is *Heishen Wan* (Black Divine Pill) from Treatise Twelve, to purge foul matter and recover.
Treatise Eight: Regarding postpartum hallucinations (seeing spirits), what is the cause?
Answer: The Heart governs blood vessels. Childbirth depletes blood, weakening Heart Qi. Decaying blood accumulates and ascends to the Heart, disturbing it, causing irritability, restlessness, hallucinations, and confused speech. Misdiagnosing this as Wind Evil is incorrect. Administer *Tiaojing San* with added *Longnao* (Dragon Brain/Camphor) for sleep and calmness. See Treatise Seven for formula.
Treatise Eighteen also states: Simply administering *Puhuang Heishen San* brings peace.
Treatise Nine: Regarding postpartum muteness within one month, what is the cause?
Answer: The Heart has seven apertures and three hairs. Postpartum Blood/Qi deficiency leads to accumulation. Decaying blood blocks Heart apertures, clouding Shen (Spirit). Since Heart Qi connects to the tongue, blockage causes tongue stiffness and muteness. Administer *Qizhen San* (Seven Treasures Powder) for recovery.

Treatise Eighteen also states: Simply administering *Puhuang Heishen San* brings peace.
Treatise Ten: Regarding postpartum abdominal pain with diarrhea/dysentery, what is the cause?
Answer: Postpartum intestines/stomach are weak, inviting Cold Evil. If exposed to cold drinks/wind within a month, Cold invades, lodging in the blind membrane and spreading to abdomen/flanks, causing paroxysmal stabbing pain. If it enters Large Intestine, it causes watery diarrhea, borborygmus, bloody/mucous stools, distension, or wandering pain. Urgently administer *Tiaozhong Tang* (Regulate Middle Decoction) for immediate cure. Misdiagnosing as food stagnation and purging is dangerous.
**Tiaozhong Tang (Regulate Middle Decoction)**

Treatise Eighteen also states: Urgently take *Puhuang Heishen San*, then *Tiaozhong Tang*.
Treatise Eleven: Regarding postpartum generalized body pain, what is the cause?
Answer: Postpartum joints open, blood flows freely. With weak Qi, blood stagnates in meridians/muscles. Days of stagnation cause joint stiffness, tendon spasms, inability to turn back or move limbs, fever, and headache. Misdiagnosing as Taiyang Cold Damage and sweating leads to tendon twitching, cold extremities, and complications. Administer *Chentong San* (Following Pain Powder) to silently resolve it.

Treatise Eighteen also states: Simply administering *Puhuang Heishen San* cures it.
Treatise Twelve: Regarding postpartum constipation, what is the cause?
Answer: During childbirth, fluids and blood descend, exhausting intestines/stomach and depleting Jin-Ye (fluids), causing constipation. If over 5-6 days with abdominal distension, dry feces are stuck. Administer *Maren Wan* (Hemp Seed Pill) to moisten. Misdiagnosing as Heat and using Cold herbs consumes Yang, leading to critical danger.
**Maren Wan (Hemp Seed Pill)**
*Cannabis Semen* (Maren, grind) *Aurantii Fructus* (Zhiqiao, remove pulp, chop, stir-fry until light yellow) *Ginseng Radix* (Renshen, remove radix, slice) *Rhei Radix et Rhizoma* (Dahuang, bake, chop. Each 0.5 liang)
Grind into powder, mix with refined honey, form pills size of paulownia seeds. Take 20 pills with warm water on empty stomach; increase if no effect.
Treatise Eighteen also addresses: Postpartum hematuria and painful defecation.
Answer: Decaying blood enters Large/Small Intestines, causing obstruction, leading to painful urination and difficult defecation. Misdiagnosing as Wulin (Five Strangury) fails. Administer *Puhuang Heishen San*, then *Heishen Wan* to purge foul matter.
**Heishen Wan (Black Divine Pill)**

Treatise Thirteen: Regarding postpartum metrorrhagia (blood flooding), what is the cause?
Answer: Childbirth depletes energy; if exerted before meridians stabilize, violent bleeding occurs. Irregular diet (salty/sour) damages Ying/Wei, weakening Qi/Blood, causing flooding. If lower abdomen is full/painful, Liver Meridian is damaged (hard to treat). Urgently administer *Gu Jing Wan* (Stabilize Meridian Pill).
**Gu Jing Wan (Stabilize Meridian Pill)**
*Artemisiae Argyi Folium* (Aiye, select clean, chop, stir-fry yellow) *Halloysitum Rubrum* (Chishizhi) *Psoraleae Fructus* (Buguzhi, lightly stir-fry until fragrant) *Equiseti Herba* (Muzei, remove roots/nodes. Grind into powder, bind with aged rice decoction into paulownia-sized pills. Take 20 pills with warm wine before meals (rice decoction acceptable). If binding is hard, add a little snow sugar.
Treatise Eighteen also states: Simply taking *Puhuang Heishen San* and *Heishen Wan* cures it immediately.
Treatise Fourteen: Regarding postpartum abdominal distension/fullness and vomiting, what is the cause?
Answer: Decaying blood scatters to Spleen/Stomach. Spleen fails to transform essence, causing distension; Stomach fails to receive food, causing vomiting. Using standard distension/vomiting drugs harms Healthy Qi. Administer *Jusheng Tang* (Great Sesame Decoction) for cure.
**Jusheng Tang (Great Sesame Decoction)**

Treatise Eighteen also states: Urgently take *Puhuang Heishen San*, then *Heishen Wan* to vomit foul matter and find relief.
Treatise Fifteen: Regarding postpartum black vapor rising from mouth/nose and epistaxis, what is the cause?
Answer: Yangming Meridian is the sea of vessels, starting at nose, crossing forehead, encircling mouth. Postpartum Qi/Blood scatter, disrupting Ying/Wei, which scatter into meridians and cannot return. This causes black vapor and epistaxis. Due to postpartum deficiency-heat, this is termed "Stomach Extinction, Lung Failure" and is incurable. Emergency measure: Tie a crimson thread and two strands of crown hair to the middle phalanges of both middle fingers to stop bleeding. No medicine works; this is a talismanic suppression.
Treatise Eighteen also states: Postpartum black vapor from mouth/nose.
Answer: Decaying blood fills organs, rushes to head/eyes, cannot return, causing dry mouth/tongue and black epistaxis. This is "Heat Transformation," termed "Failed Lung," and is incurable.
Treatise Sixteen: Regarding postpartum rapid breathing/asthma, what is the cause?
Answer: Ying is Blood, Wei is Qi. Ying moves in vessels, Wei outside. Excessive blood loss during childbirth exhausts Ying Blood, leaving Wei Qi unanchored, gathering in Lungs, causing asthma. This is "Lone Yang, Absolute Yin," difficult to treat. If lochia is slow and decaying blood stagnates, steaming Lungs also causes asthma. Administer *Duoming Dan*; as blood clears, breathing stops. See Treatise Two.
Treatise Eighteen also states: Postpartum rapid breathing/asthma.
Answer: Decaying blood passes through Heart, turning in throat. This is incurable.
Treatise Seventeen: Regarding postpartum wind-stroke, what is the cause?
Answer: Rising early within 5-7 days, or sexual activity within 100 days, emotional turmoil, or moxibustion injury damages organs. Initial signs: dry eyes, locked jaw, muscle twitching, progressing to stiff neck/back. This is man-made, not spontaneous wind-stroke, and is incurable.
Treatise Eighteen: Regarding postpartum heart pain, what is the cause?
Answer: Heart governs Blood. Latent cold combines with postpartum deficiency, congealing Blood. Ascending Qi strikes Heart collaterals, causing pain. Treat with *Dayanmi Tang* (Great Rock Honey Decoction); as cold leaves, vessels warm, pain stops. Misdiagnosing as trauma worsens deficiency/cold. Severe cold transmitting to main Heart Meridian causes "True Heart Pain" (Zhenxintong), fatal within hours. Do not use mild medicines.
**Dayanmi Tang (Great Rock Honey Decoction)**
*Rehmanniae Radix Preparata* (Shudihuang, wash, soak in wine overnight, dry-roast, chop) *Angelicae Sinensis Radix* (Danggui, wash, remove radix/rootlets, slice, dry-roast) *Evodiae Fructus* (Wuzhuyu, remove stems) *Angelicae Pubescentis Radix* (Duhuo, wash, Sichuan origin best) *Paeoniae Radix* (Shaoyao, wash, chop) *Zingiberis Rhizoma* (Ganjiang, process, chop) *Glycyrrhizae Radix et Rhizoma* (Gancao, honey-fry, chop)

Treatise Nineteen: Regarding postpartum heat oppression transforming into Hongjiao (podagra/gout-like condition), what is the cause?
Answer: Childbirth causes Blood deficiency and heat. Combined with excessive cooling in spring/summer and damp earth vapors entering feet via footwear, this causes Hongjiao. Symptoms include heat oppression and pulling pain...
