The convulsions accompanying the eruption of measles (Rash) differ from ordinary cases. If there is phlegm rattling in the throat, it is not a true convulsion; one must distinguish the sequence of onset. If it appears during the initial fever and early eruption stage, it is not harmful; treatment should focus on dispersing and supporting the exterior, combined with cooling herbs. If it appears when the rash is fully erupted yet beginning to fade, it is difficult to treat. The general principle is to clear heat from the Upper Jiao (Upper Burner) and promote urination.
After the rash subsides, if coughing occurs, it is due to excessive fire causing dryness, which impairs the Lung Metal. Treatment should employ Qing Jin Jiang Huo Tang (Clear Metal Descend Fire Decoction), containing Huang Qin (Scutellariae Radix), Huang Lian (Coptidis Rhizoma), Shan Zhi (Gardeniae Fructus), Shi Gao (Gypsum Fibrosum), Hua Fen (Trichosanthis Radix), Bei Mu (Fritillariae Bulbus), Chen Pi (Citri Reticulatae Pericarpium), Sang Bai Pi (Mori Cortex), Xing Ren (Armeniacae Semen Amarum), Su Zi (Perillae Fructus), Gan Cao (Glycyrrhizae Radix et Rhizoma), Mai Dong (Ophiopogonis Tuber), and Gua Lou Ren (Trichosanthis Semen). If ineffective, add Ting Li (Lepidii Semen) and Zhi Ke (Aurantii Fructus). If voice loss occurs, base the prescription on Qing Jin Jiang Huo Tang and add Ting Li. If accompanied by irritability, abdominal distension, persistent diarrhea and dysentery, hoarseness, and cyanotic or blackish-purple lips, use Huang Lian Jie Du Tang (Coptis Decoction for Resolving Toxins), containing Huang Qin, Huang Lian, Zhi Ke, Gui Wei (Angelicae Sinensis Radix), Hong Hua (Carthami Flos), Chi Shao (Paeoniae Radix Rubra), Sheng Gan Cao (Raw Glycyrrhizae Radix et Rhizoma), and Da Huang (Rhei Radix et Rhizoma). Alternatively, use Gan Jie Tang (Licorice and Platycodon Decoction), containing Gan Cao, Jie Geng (Platycodonis Radix), Niu Bang Zi (Armeniacae Amarum Semen), Xuan Shen (Scrophulariae Radix), Lian Qiao (Forsythiae Fructus), Zhi Mu (Anemarrhenae Rhizoma), Xing Ren, and Gan Jiang (Zingiberis Rhizoma). If blood is present in the sputum, with panting, chest tightness, and confusion, use Liang Ge San (Cool the Diaphragm Powder), containing Huang Qin, Shan Zhi, Bo He (Menthae Haplocalycis Herba), Lian Qiao, Sheng Gan Cao, and Da Huang.
Thirst following the rash presents in two forms: deficiency and excess. If the tongue coating is prickly, lips are red like cinnabar, and bowel/bladder movements are obstructed, this indicates internal heat with both interior and exterior excess. Treatment requires cooling and regulating herbs such as Sheng Di (Rehmanniae Radix), Huang Qin, Hua Fen, Niu Bang Zi, Lian Qiao, Jing Jie (Schizonepetae Herba), Ge Gen (Puerariae Lobatae Radix), Jie Geng, Shi Gao, Huang Lian, plus Deng Cao (Junci Medulla). If lips and mouth are pale red, the tongue has no coating, and bowel/bladder functions are normal, this indicates internal deficiency. Treatment should employ Qing Jin Yi Fei Tang (Clear Metal Tonify Lung Decoction), containing Sha Shen (Glehniae Radix), Mai Dong, Sheng Di, Dan Shen (Salviae Miltiorrhizae Radix et Rhizoma), served with Mung Bean Soup or fried rice tea made with sesame seeds. If panting and abdominal distension occur, it signifies a critical condition (Bai Zheng); cold water must strictly be avoided to prevent fluid retention.
Absence of nasal discharge during the rash is caused by excessive fire scorching the Lung Metal. Treatment should employ Qing Jin Fei Run Tang (Clear Metal Moisten Lung Decoction), containing Zhi Mu, Huang Qin, Sheng Di, Hua Fen, Lian Qiao, and Lou Ren. Nasal bleeding (Bi Nu) accompanying the rash is due to stagnant fire in the Lung Metal rushing upward. Herbs include Ge Gen, Sheng Di, Shan Zhi, Bo He, Dan Pi (Moutan Cortex), Mao Gen (Imperatae Rhizoma), Xi Jiao (Rhinocerotis Cornu), Ling Yang Jiao (Saigae Tataricae Cornu), and Huang Qin. If these symptoms appear after the rash has fully erupted, use Xi Jiao Di Huang Tang (Rhinoceros Horn and Rehmannia Decoction) added with Yin Chen (Artemisiae Scopariae Herba) and Mu Tong (Akebiae Caulis) to direct fire downward for recovery. Xi Jiao Di Huang Tang consists of Sheng Di, Xi Jiao, Dan Pi, and Bai Shao (Paeoniae Radix Alba), added with Yin Chen and Mu Tong.
